Asthma Controller vs. Reliever Inhalers: What’s the Difference?

Short answer: A controller (also called a preventer or maintenance treatment) is taken on a regular schedule, or under a specific prescribed plan, to keep airway inflammation under control and reduce future risk. A reliever is taken when symptoms occur to provide rapid relief. They may be two different inhalers, or one prescribed inhaler may serve both jobs under an AIR or MART plan. The medicine, schedule and written asthma action plan on your prescription always determine which role your inhaler has.

Controller versus reliever: the difference at a glance

Question Controller or maintenance treatment Reliever
Primary purpose Control asthma over time, reduce symptoms and lower the risk of future flare-ups Relieve symptoms quickly when they occur
When it is used Usually every day or on another regular schedule; some anti-inflammatory regimens are prescribed only when needed As needed for symptoms, according to the written action plan
Typical medicine examples Often an inhaled corticosteroid (ICS), alone or combined with another long-acting medicine A short-acting beta2-agonist (SABA), or in some plans an ICS combined with rapid-acting formoterol
How many devices May be a separate daily inhaler or the same inhaler used in a MART plan May be a separate symptom inhaler or the same device used for maintenance in an AIR or MART plan
What it tells you Regular use is part of preventing poor control, even when you feel well Frequent or increasing use can signal that your asthma plan needs a clinical review

These are roles, not reliable color categories. Inhaler colors vary between countries and products, so identify the named medicine, the prescribed frequency and the instructions in your action plan rather than relying on a blue, brown or other color.

What a controller or maintenance inhaler does

Asthma involves inflammation and changing narrowing of the airways, not just the symptoms you notice at a particular moment. A controller is intended to influence that underlying pattern. Inhaled corticosteroids are the most familiar controller medicines; depending on the prescription, an ICS can be used alone or in a combination inhaler.

The Global Initiative for Asthma (GINA) defines maintenance treatment as “Asthma treatment that is prescribed for use every day (or on a regularly scheduled basis).” Its 2026 strategy report also says that “The term ‘maintenance’ describes the prescribed frequency of administration, not a particular class of asthma medicine.” That wording prevents a common mistake: maintenance describes how often treatment is prescribed, while the active ingredients determine what the medicine does.

People often use controller and preventer to mean regular treatment that addresses both current control and future risk. GINA’s terminology is more precise because an ICS-containing medicine can be prescribed as an anti-inflammatory reliever only when symptoms occur, and an inhaler can have both scheduled and as-needed roles. Do not infer your schedule from the medicine class alone.

Why regular use matters when you feel well

Symptoms can be quiet while airway inflammation remains active. A prescribed controller schedule is therefore not normally something to stop simply because breathing feels normal. The US National Heart, Lung, and Blood Institute (NHLBI) explains that a healthcare provider may prescribe controller medicines daily to help prevent asthma attacks and control symptoms. The exact dose, device and frequency depend on the individual prescription.

What a controller is not

  • It is not a substitute for the rapid-relief instructions in your action plan during an acute episode unless your plan specifically uses the same inhaler for both roles.
  • It is not safe to assume that every combination inhaler is a controller, a reliever or both. The active ingredients and local authorization matter.
  • It is not a reason to change, stop, double or share treatment without advice from the prescriber or pharmacist.

What a reliever or rescue inhaler does

A reliever is taken as needed for quick relief of asthma symptoms such as wheeze, chest tightness, cough or shortness of breath. GINA’s 2026 report defines a reliever as an “Asthma inhaler taken as needed, for quick relief of asthma symptoms.”

Traditional relievers are inhaled SABAs. They relax airway muscle quickly, which can make breathing feel easier, but they do not provide the same anti-inflammatory treatment as an ICS. Some newer plans use an anti-inflammatory reliever that combines an ICS with rapid-acting formoterol. That option can provide bronchodilation while delivering an anti-inflammatory medicine, but it is appropriate only when specifically prescribed.

NHLBI’s US patient guidance separates quick-relief medicines from long-term control medicines. It lists inhaled SABAs among common quick-relief medicines and corticosteroids, including inhaled corticosteroids, among long-term control medicines. Those are broad categories: instructions, side effects and suitability still depend on the exact product, strength, device and person.

Why increasing reliever use matters

A reliever is designed for episodes of symptoms, not to conceal a steadily worsening pattern. If you need it more often than your written plan allows, obtain advice from your asthma clinician. Increasing use, night-time symptoms, activity limitation or shorter relief may indicate that the control strategy, trigger management, inhaler technique or diagnosis needs review.

Do not respond by inventing a new schedule. Follow the action plan’s escalation steps and its emergency instructions. The NHS asthma guidance gives different response instructions for a traditional blue reliever and for AIR or MART inhalers; those directions should not be merged into one generic rule.

Do I need both a controller and a reliever inhaler?

Not necessarily. The answer depends on the regimen selected for you. There are three broad arrangements:

Two separate inhalers

One device is used on a regular schedule as the preventer or controller, and another is used when symptoms occur. This makes the two roles visually and operationally separate, but you still need to know the active ingredients and the exact instructions. The NHS describes this as a daily preventer plus a symptom reliever.

AIR: one inhaler used when symptoms occur

AIR means anti-inflammatory reliever. In GINA’s preferred adult and adolescent Track 1, low-dose ICS-formoterol is used as needed for symptoms at the relevant treatment steps. Because the inhaler includes an ICS, the reliever also supplies anti-inflammatory treatment when it is used. This is not the same as taking every ICS-formoterol product whenever you wish; the product, dose, age range and local approval must match the prescription.

MART: one inhaler for maintenance and relief

MART means maintenance-and-reliever therapy. The same prescribed ICS-formoterol inhaler is taken at scheduled maintenance times and additionally when symptoms occur. The scheduled doses provide the maintenance role; the extra as-needed doses provide the reliever role. Your action plan must specify the limits and escalation steps.

The NHS uses patient-facing language that distinguishes AIR, used when symptoms occur; MART, used every day and when symptoms occur; and two separate inhalers. These labels describe a treatment plan, not a universal property of a device.

Can one inhaler be both a preventer and a reliever?

Yes, but only under a regimen designed for that purpose. A single ICS-formoterol inhaler can be both maintenance and reliever in a MART plan, or an ICS-formoterol inhaler can be prescribed as an AIR reliever at certain treatment steps. In those plans, the inhaler’s role is defined by the schedule and action plan.

Not every combination inhaler can be used this way. GINA cautions that ICS-formoterol should not be used as the reliever when the maintenance combination contains a non-formoterol LABA. The NHS also notes that not every formoterol inhaler is prescribed as MART. A combination containing a different long-acting bronchodilator may be suitable as maintenance but not as your symptom reliever.

Never use the phrase “formoterol inhaler” as a complete instruction. Check the exact product name, the strength, the prescribed indication and the written plan. Licensing and age approvals differ by country, and guideline pathways do not replace an individualized prescription.

How GINA’s treatment tracks illustrate the roles

GINA’s 2025 adult and adolescent guidance makes the overlap concrete:

  • Preferred Track 1: an as-needed low-dose ICS-formoterol reliever is used at the relevant lower steps; at higher steps, the same class is used for scheduled maintenance and as-needed relief as MART.
  • Track 2: the reliever is a SABA or an ICS-SABA combination, while maintenance treatment follows the prescribed controller schedule.

These are guideline pathways, not instructions to select a treatment yourself. Local regulators may authorize different products, devices, ages or doses. Your clinician’s prescription and action plan take precedence.

Use your written asthma action plan as the decision rule

An asthma action plan translates a general diagnosis into instructions for your inhaler. NHLBI describes a written plan that includes triggers, signs of an attack, which medicines to take and when, when to call a healthcare provider or go to emergency care, and whom to contact.

Check that your plan states

  • The name and role of every inhaler.
  • Which inhaler is used regularly, and at what times.
  • Which inhaler is used for symptoms.
  • What to do if symptoms increase, including the plan’s maximum permitted doses or other escalation instructions.
  • When to contact your clinician urgently and when to call emergency services.
  • Any peak-flow or symptom zones your clinician has included.

Keep the current plan with your medicines and make sure household members, school staff or caregivers who may help you know where it is. Ask for clarification if the label, device instructions and plan appear inconsistent; do not resolve the discrepancy by guessing.

What to do when symptoms start

  1. Use the reliever named in your personal action plan. If your plan uses MART or AIR, use the prescribed ICS-formoterol inhaler; if it specifies a separate reliever, use that device instead.
  2. Follow the plan’s timing, repeat-dose and maximum-dose instructions exactly. Directions differ between blue-reliever plans and AIR/MART plans, so do not substitute a generic online schedule.
  3. Use the inhaler with the technique taught for that device. A puffer and a breath-actuated or dry-powder device do not work in the same way.
  4. Monitor the response described in your plan. If symptoms worsen or do not improve after the plan’s maximum dose, seek emergency help as directed by the NHS and your local emergency service.
  5. After an episode, arrange the follow-up specified by your clinician, particularly if reliever use has increased or the episode was more severe than usual.

Emergency symptoms can include severe or rapidly worsening breathlessness, inability to speak normally, exhaustion, bluish discoloration or poor response to the prescribed reliever. Your local emergency guidance and action plan should determine when to call; do not delay emergency care to experiment with another inhaler.

Technique, spacers and device compatibility

An inhaler works only if the medicine reaches the airways. Device technique is therefore part of the treatment, not a minor detail. Ask a pharmacist, nurse or clinician to watch you use the device and correct errors. Recheck technique when the device changes, after a long gap in use or whenever control worsens.

A spacer chamber is an accessory compatible with some pressurized metered-dose inhalers. NHS guidance mentions using a spacer with a blue reliever inhaler during an attack if one is available. A spacer is not a medicine and does not replace the prescribed inhaler. Compatibility, cleaning, mask fit and technique depend on the device; use the instructions supplied for your inhaler and spacer.

Dry-powder inhalers, soft-mist devices and pressurized inhalers have different loading, breathing and coordination requirements. Do not transfer a technique from one type to another without instruction. Store, clean and replace devices as directed, and check that a dose counter or indicator is readable.

Common mistakes and how to avoid them

Using the reliever as the only long-term strategy

Rapid symptom relief can feel reassuring, but a SABA-only pattern does not provide the controller treatment your clinician may have prescribed. If you are repeatedly relying on a reliever, contact your healthcare professional for a review rather than adding doses on your own.

Stopping a controller after improvement

Feeling better may mean the plan is working. It does not by itself authorize stopping or reducing a controller. Any step-down should be planned with the prescriber and recorded in the action plan.

Assuming every ICS-formoterol device is interchangeable

Products can differ in dose, inhaler design, authorization and age indication. A formoterol-containing maintenance inhaler is not automatically a MART reliever.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Using the wrong device during an attack

In a two-inhaler plan, the daily controller may not provide immediate relief. In a MART plan, the same device may be correct for both roles, but only because the plan says so. Keep the labels and plan together to prevent confusion.

Relying on color or someone else’s inhaler

Colors are not standardized internationally, and sharing medicine is unsafe. Use only the inhaler prescribed for you and confirm its role by the name and plan.

Side effects and questions to raise with a clinician

Side effects vary with the active ingredient, dose, delivery device and person. Do not assume that all medicines in a class have identical effects or instructions. Ask how to reduce local mouth or throat effects from an ICS, whether rinsing is advised for your product, and which symptoms should prompt a call.

Bring these questions to an appointment or pharmacy consultation:

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.
  • Which inhaler is my controller, which is my reliever, and is either device used for both?
  • What are the exact scheduled and as-needed instructions?
  • What is the maximum use allowed before I call for help?
  • Does my device require a spacer, priming, cleaning or a particular breathing technique?
  • What should I do if the inhaler is empty, unavailable or seems not to work?
  • Does this plan apply to my age, pregnancy status, other medicines and country of treatment?

Bottom line for choosing the right inhaler at the right time

A controller or maintenance treatment is about ongoing control and future risk; a reliever is about rapid relief when symptoms occur. The roles can be split between two devices or combined in an AIR or MART plan. Because not every combination inhaler is suitable as a reliever, the only dependable guide is the active ingredient, prescribed schedule and personal written asthma action plan. Follow those instructions, have your technique checked and seek urgent help when the plan says symptoms are not responding.
