Are Smart Contact Lenses Safe for Your Eyes? Risks, Regulation and Hygiene

Smart contact lenses are not automatically safe or unsafe. They should be treated as medical devices whose risk depends on regulatory authorization, professional fitting, engineering design, the prescribed wearing schedule and meticulous contact-lens hygiene. A 2025 review describes diagnostic, therapeutic and display lenses as an emerging field; it does not establish a single, universally approved consumer smart-lens model.

Until a named product has verifiable authorization and human clinical evidence, regard claims about sensors, displays or drug delivery as unproven for routine wear. The ordinary hazards of contact lenses—serious infection, corneal ulcer and, rarely, permanent vision loss—still apply, while electronics add further questions about oxygen flow, heat, power, wireless components, coatings and failure behavior.

What makes a smart lens different?

A conventional contact lens sits on the tear film and changes how oxygen reaches the cornea. A smart lens may add sensors, antennas, micro-displays, conductive traces, a battery or wireless power receiver, a surface coating, or a drug-delivery component. Each addition can alter thickness, oxygen transmissibility, wettability, optical quality, mechanical comfort and the way the lens must be cleaned or replaced.

“Smart” is a description of function, not a safety certification. A laboratory prototype, a device being tested in a clinical study and a product authorized for a specific medical indication have different evidence behind them. Marketing language cannot substitute for a regulator’s decision, a prescription and documented follow-up.

The ordinary contact-lens risks still apply

Infection and corneal ulcers

The U.S. Food and Drug Administration warns that wearing contact lenses can lead to eye infections and corneal ulcers, conditions that can progress quickly. Rare severe infections can cause blindness. A lens that contains electronics does not protect the cornea from bacteria, fungi, parasites or inflammation; an ill-fitting or contaminated smart lens may make detection and treatment more complicated.

Remove the lens and contact an eye-care professional promptly if you develop pain, marked redness, discharge, light sensitivity, blurred vision, swelling, burning, excessive tearing or unusual gritty discomfort. Do not wait to see whether symptoms disappear after a night’s sleep. Keep the lens and storage case available if the clinician wants to examine them.

Water exposure and Acanthamoeba

Water can carry organisms that adhere to a lens and cause severe keratitis. Do not expose lenses to tap, bottled, distilled, lake, ocean, shower, swimming-pool or hot-tub water. Remove them before bathing or swimming and use appropriate protective eyewear if your clinician permits lens wear in a specific activity. Never wet a lens with saliva; saliva is not sterile or a disinfectant.

Overnight wear and old solution

Sleeping in lenses increases the risk of microbial keratitis because the closed eyelid reduces oxygen and tear exchange. Do not sleep in a smart lens unless the prescribing clinician has specifically authorized that schedule for that exact product. Never “top off” solution in a case. Empty the old fluid, clean the case as directed, and fill it with fresh disinfecting solution.

Cases, rubbing and peroxide systems

Follow the product instructions for rubbing and rinsing, even when a solution is marketed as “no rub.” Air-dry the empty case and replace it every three months, or sooner if the clinician or manufacturer specifies. Hydrogen-peroxide systems must complete their neutralization cycle before a lens is inserted. Unneutralized peroxide can cause intense stinging and burn or damage the cornea; do not put disinfecting solution directly in your eyes.

Additional hazards introduced by electronics and advanced materials

Oxygen supply and physical fit

Adding circuitry or a thicker encapsulation can reduce oxygen transmissibility or change how a lens moves with each blink. Corneal swelling, persistent redness, foggy vision and discomfort can result even when the electronics work as designed. A fitting that is acceptable for a plain prescription lens may not be safe for a smart version. The clinician needs the exact material, thickness profile, oxygen-transmissibility data and maximum wearing time for the model being prescribed.

Heat, power and wireless components

A powered lens may use a battery, inductive coupling, radio-frequency link or another energy source. Ask how much heat the system can produce at the corneal surface, what happens if power is interrupted, and whether the device automatically enters a safe state. A fail-safe design should prevent a dead battery, software error or lost connection from creating a sharp edge, obstructing vision or delivering an unintended dose.

Surface coatings, sterility and chemical compatibility

Conductive layers, optical coatings and drug reservoirs must remain sealed and biocompatible throughout the approved replacement period. Cleaning agents that are safe for ordinary hydrogel or silicone-hydrogel lenses may damage a specialized coating or leave residues. Use only the solution and cleaning method listed for that product; do not experiment with homemade saline, tap water or a different disinfectant.

Display and drug-delivery failure modes

A display can create glare, visual distraction or a temporary blind spot if it shifts out of alignment. A therapeutic lens adds dosing questions: the amount released, release rate, remaining dose after cleaning and what occurs if the reservoir leaks. Ask whether the device has been tested under blinking, tears, temperature changes and the full approved wear period rather than only in a laboratory container.

Data and software considerations

Sensors may collect health or behavioral data and transmit it to a phone or other receiver. Privacy and cybersecurity are separate from corneal safety, but a software update or wireless failure should never alter a medically important setting without an explicit clinician-controlled process. Find out how firmware is updated, how errors are reported and whether the lens can be worn safely when its companion app is unavailable.

Regulatory status: prototype, investigational device or authorized product?

The 2025 review describes U.S. diagnostic and therapeutic smart lenses as devices that would fall under Class II or Class III medical-device regulation, depending on their risks and intended use. That classification does not mean every product described as “smart” has authorization. Check the exact model, manufacturer, indication, replacement schedule and labeling in the relevant regulatory database and prescribing materials.

Product stage What the evidence usually means What a wearer should do
Laboratory prototype Engineering performance may have been demonstrated in bench tests, models or short demonstrations; routine human safety is not established. Do not purchase or wear it outside an approved study.
Clinical investigational device Human testing is occurring under a defined protocol with eligibility rules, monitoring and informed consent. Use only through the authorized study team and follow its visit and reporting schedule.
Authorized medical device A regulator has reviewed the named product for a specific indication, design and labeling; authorization is not a guarantee of zero risk. Obtain the required prescription, professional fitting and follow-up for that exact indication.

No universal, broadly approved consumer smart-lens model was established by the 2025 review. Therefore, a product page that promises health monitoring or augmented-reality vision is not enough evidence to justify unsupervised wear.

How to assess a specific smart-lens product

Before accepting a prescription or enrolling in a study, ask for written answers to these product-specific questions:

Safety axis Information to verify
Authorization and indication Regulatory authority, authorization number or study identifier, exact model, intended use and approved age or health restrictions.
Fitting and follow-up Who performs the fitting, how soon the first follow-up occurs, which examinations are repeated and who handles urgent problems.
Oxygen and wear time Oxygen-transmissibility specification, maximum daily wear, replacement interval and conditions that require earlier removal.
Cleaning and storage Approved disinfectant, rubbing and rinsing method, case requirements, peroxide neutralization instructions and whether the lens can be reused.
Power and fail-safe behavior Power source, charging or coupling limits, heat testing, behavior after a power or connection failure and safe removal procedure.
Human evidence Peer-reviewed clinical results, number and duration of participants, adverse events, dropouts and whether the tested version matches the product offered to you.
Post-market record Recall notices, corrective actions, complaint channels and adverse-event reporting process for the exact model.

If the seller cannot provide these details, pause rather than treating uncertainty as proof of safety.

A safe handling and hygiene routine

  1. Wash first. Wash hands with soap and water, rinse thoroughly and dry with a lint-free method before every insertion, removal, cleaning or case-handling event.
  2. Inspect the lens. Check for tears, chips, deposits, clouding, a damaged edge or a displaced electronic component. Do not wear a damaged lens.
  3. Use only the approved solution. Follow the smart-lens manufacturer’s and clinician’s exact instructions. Never substitute tap water, bottled water, homemade saline or saliva.
  4. Clean as directed. Rub and rinse for the stated time when the instructions require it. Do not assume a “no-rub” label applies to a specialized lens unless its own labeling says so.
  5. Disinfect for the full time. Fill the case with fresh solution. If using hydrogen peroxide, wait through the complete neutralization cycle; inserting the lens early can burn the cornea.
  6. Store correctly. Close the case, keep it away from water and heat, and let it air-dry whenever it is empty. Do not add new solution to fluid that has already been used.
  7. Respect the schedule. Remove the lens at the prescribed time, never sleep in it without explicit authorization and replace it on the specified date even if it still feels comfortable.
  8. Keep a backup plan. Carry glasses and know how to turn off or remove the device if the display, sensor or wireless link behaves unexpectedly. Do not drive if vision is blurred or the lens is malfunctioning.

What to do when something feels wrong

Remove the lens immediately for pain, redness, discharge, light sensitivity, blurred vision, swelling, burning, excessive tearing or gritty discomfort. Contact an eye-care professional promptly, especially if symptoms persist after removal or vision changes. Do not put the lens back in to “test” it, rinse it with water or use someone else’s eye drops. Preserve the lens and case in their current condition if the clinician requests them for examination.

Seek urgent medical attention for severe pain, rapidly worsening redness, a white spot on the cornea, sudden vision loss or marked light sensitivity. These signs can indicate a sight-threatening infection or ulcer and should not be managed with leftover medication.

Prescription and professional supervision are non-negotiable

The American Academy of Ophthalmology and American Academy of Optometry state that all contact lenses, including decorative and costume lenses, are medical devices requiring an eye doctor’s prescription and supervision. Their 2025 statement reported U.S. Food and Drug Administration testing in which 60% of counterfeit cosmetic contact lenses and 27% of unapproved cosmetic lenses showed microbial contamination. Those figures concern counterfeit and unapproved cosmetic products, not a measured complication rate for smart lenses, but they demonstrate why an online purchase without fitting and oversight is unsafe.

A qualified clinician can examine the cornea, measure fit, review tear-film health, set a wear limit and recognize early oxygen deprivation or infection. Tell the clinician about dry eye, allergies, prior corneal disease, diabetes, immune suppression, current medicines and any history of contact-lens complications before trying an advanced lens.

Practical decision rule

  • Consider use only when: the exact product has appropriate authorization or is supplied through an approved clinical study; a licensed professional prescribes and fits it; cleaning, power and replacement instructions are complete; and follow-up is arranged.
  • Decline or postpone when: the seller will not identify the model or regulatory status, the lens is described only with promotional claims, the product lacks human safety data, the required solution is unavailable, or you cannot commit to the wear schedule.
  • Stop wearing and seek advice when: any warning symptom appears, the lens is damaged, the display or electronics behave unpredictably, or the product has been recalled.

Bottom line

Smart contact lenses may eventually provide useful sensing, display or treatment functions, but their safety must be demonstrated for each design and indication. Treat the lens as a regulated medical device, not as an ordinary gadget: verify authorization, obtain a professional prescription and fitting, follow the exact hygiene and wear schedule, keep water away, and act quickly when symptoms occur.
