An Apple Watch hypertension notification is a prompt to check your blood pressure with a cuff and follow up with a healthcare professional—not a blood pressure reading or a diagnosis. In Apple’s pivotal study submitted to the FDA, the feature had 41.2% sensitivity, meaning it missed many participants who met the study’s definition of hypertension. No alert does not mean your blood pressure is normal.
Can Apple Watch detect high blood pressure?
Apple says the feature analyzes optical heart sensor data over 30-day evaluation periods for patterns associated with chronic high blood pressure. It does not measure blood pressure in mm Hg. A notification is a screening prompt, not a substitute for a cuff measurement or a clinician’s assessment. Apple’s support page also warns that not all people with hypertension receive a notification.

The feature is intended for people aged 22 or older who have not been diagnosed with hypertension and are not pregnant. Compatible devices, software, and regional availability are also required; Apple’s availability page lists current regional support. Apple says the feature is not intended to diagnose, treat, or manage hypertension. Apple’s watchOS guide separately notes that Apple Watch cannot detect a heart attack.
How accurate are Apple Watch hypertension alerts?
In the pivotal study described in Apple Inc.’s 2025 FDA 510(k) submission K250507, 2,229 adults without a prior hypertension diagnosis enrolled. Participants wore an Apple Watch for 30 days and used an FDA-cleared home blood pressure monitor as the reference. A total of 1,863 participants provided at least 15 days of usable data for the primary analysis. The FDA 510(k) summary reports these study-level performance estimates:

| Measure in the pivotal study | Result | What it indicates |
|---|---|---|
| Overall sensitivity | 41.2% (95% CI 37.2–45.3) | Of participants who met the study definition of hypertension, this share received an alert. |
| Overall specificity | 92.3% (95% CI 90.6–93.7) | Of participants who did not meet the study definition, this share did not receive an alert. |
| Sensitivity for stage 2 hypertension | 53.7% (95% CI 47.7–59.7) | The feature detected a larger share of stage 2 cases than of hypertension overall, but still missed many. |
| Specificity among normotensive participants | 95.3% (95% CI 93.7–96.5) | Most normotensive participants did not receive an alert. |
Put plainly, the overall sensitivity means 58.8% of participants with undiagnosed hypertension in the study did not receive an alert; 7.7% of participants without hypertension had an inappropriate alert, according to a 2026 JAMA Network Open commentary. These rates describe the study population, not an individual’s personal odds of having hypertension. Those odds also depend on prevalence and clinical context.
Can you trust a normal result if the watch did not alert?
No. Because the study’s overall sensitivity was 41.2%, absence of an alert cannot rule out hypertension or reassure you that your blood pressure is normal. Keep up with routine blood pressure checks using a suitable cuff, even if the watch has never notified you.

What should you do after a notification?
- Measure with a cuff. Use a home blood pressure monitor authorized for your market and suitable for you. The FDA warns that unauthorized devices can give inaccurate measurements. See the FDA’s guidance on blood pressure monitoring devices.
- Follow Apple’s measurement schedule. Apple directs users to take and record cuff readings twice daily for seven days after an alert. Apple’s instructions for hypertension notifications explain the follow-up.
- Discuss the results. Share your readings and the notification with a healthcare professional, who can interpret them in context and advise on next steps.
- Do not change medication based on the watch. The notification is not a diagnosis or a treatment guide.
What the evidence does—and does not—establish
The performance figures come from Apple’s pivotal validation study submitted to the FDA, rather than a broad independent trial of real-world health outcomes. A peer-reviewed 2026 commentary discusses limitations of the validation approach and the need for evidence about clinical impact and practical follow-up pathways. Read the commentary.
The FDA cleared the software feature through a 510(k) pathway; that is not the same as FDA approval of a cuffless blood pressure monitor. The FDA’s January 2026 draft guidance on cuffless non-invasive blood pressure devices is nonbinding and not for implementation. See the FDA draft guidance. The sources cited here do not establish that the notification feature improves diagnosis rates or health outcomes.
