The iPhone app can store medication schedules, manual entries, and information from compatible apps and devices. For ongoing heart-rate measurements and automatic overnight sleep tracking, you generally need an Apple Watch or another compatible source. These features are useful for reminders and personal trends, not for diagnosing a condition or replacing medical advice.
What Apple Health does—and what Apple Watch adds
The Health app on iPhone and iPad brings together information from Apple devices, compatible apps and connected devices, supported healthcare organizations, and manual entries. It organizes records into categories such as Heart, Sleep, Medications, Activity, Respiratory, and Vitals, with charts, trends, highlights, and sharing options. Apple Watch and other compatible devices supply measurements; the Health app stores and presents them.

Without a watch, you can still manage medications, set a sleep goal and schedule, enter some information, review supported medical records, and import data. An iPhone by itself does not continuously measure wrist pulse or automatically collect sleep stages and overnight vitals.
Apple Watch health notifications are alerts, not diagnoses. Features can vary by model, software, age eligibility, regulatory authorization, and country. See Apple’s Health app guide and its overview of Health categories and data sources.

Heart-rate tracking: readings, alerts, and limits
What you can see
On Apple Watch, the Heart Rate app shows current heart rate and can display resting, walking, workout, and post-workout measurements, as well as readings associated with Breathe or Reflect sessions. On iPhone, open Health > Browse > Heart to review available history and trends. Compatible external devices and apps may contribute heart-rate information too.
Routine heart-rate monitoring uses optical sensors. It is different from an ECG: on supported models, the ECG app uses electrical sensors to make a single-lead ECG-like recording. That recording is not a full clinical ECG, and neither an ECG result nor an alert is a diagnosis. Apple describes the feature and its limits in its Apple Watch arrhythmia detection information.
Set high- and low-heart-rate notifications
- On Apple Watch, open Settings > Heart. Alternatively, open the Watch app on iPhone and find the Heart settings.
- Choose High Heart Rate Notifications or Low Heart Rate Notifications.
- Select the threshold you want to use.
Apple says the watch can alert you when your heart rate remains above or below the selected threshold after you have been inactive for at least 10 minutes. To view readings directly, open the Heart Rate app on the watch and scroll through the available measurements. Siri may support requests such as “What’s my heart rate?” depending on language, region, device, and software configuration. Other heart features are model- and region-dependent
- Irregular-rhythm notifications can flag patterns that may suggest an irregular rhythm; they do not continuously check for every heart condition.
- ECG is available only on supported models and in supported regions, subject to age and other eligibility requirements.
- AFib History is intended for eligible users with a prior atrial fibrillation diagnosis; it is not a way to diagnose AFib in someone without that history.
- Hypertension notifications are available on supported models and in supported regions. They identify patterns that may warrant follow-up; Apple Watch does not measure blood pressure directly. Apple’s validation paper describes the feature’s scope.
- Cardio-fitness notifications are another trend-oriented alert, not a substitute for clinical assessment.
Reading a result safely
Fit and contact matter. A loose band, movement, sweat, water, cold skin, tattoos, or debris can affect optical readings; Apple advises keeping the watch and wrist clean and dry for better heart-rate recording. If heart-rate data is missing, check Settings > Privacy & Security > Health > Heart Rate on the watch.

A notification is not proof of a condition, and no notification does not rule one out. Do not change medication or treatment based only on watch data. Seek medical help for concerning symptoms such as chest pain, fainting, severe shortness of breath, or a rapid or irregular heartbeat that persists.
Medication tracking: schedules and dose logs
Add a medication on iPhone
- Open Health and tap Search.
- Tap Medications, then Add a Medication.
- Search for the medication or enter its name.
- Select the form, strength, dosage, and schedule, then save it.
The feature can hold prescription and over-the-counter medicines, vitamins, and supplements. You can schedule doses or record an as-needed medication. Check each name, formulation, strength, dose, units, and timing against the label and your clinician’s or pharmacist’s instructions.
Log doses and review history
After setup on iPhone or iPad, Apple Watch can display medication reminders and let you record a dose as taken or skipped. Apple documents Siri logging requests such as “Log my 6AM medications as taken”; supported commands vary by configuration. See the Apple Watch medication guide.
A dose log records what a person reports; it cannot prove a pill was swallowed. A reminder can also be missed or dismissed. Keep the list current after prescription changes, and review it with a pharmacist or clinician after a hospitalization, a change in care provider, or a new or changed prescription.

What the medication feature does not do
Apple Health is not a prescribing service, a medication-reconciliation process, or a drug-interaction checker. Do not assume that entering two medicines confirms they are safe to combine. Similar names or formulations and incorrect units can lead to bad records; shared devices or accounts can also lead to the wrong person logging a dose. Confirm medication questions with a pharmacist or prescribing clinician.
Sleep tracking: schedule, stages, and overnight use
Set a sleep goal and schedule
- On iPhone, open Health and go to Sleep.
- Set a sleep goal and create a schedule with bedtime and wake time.
- Choose whether to use Sleep Focus and Wind Down reminders.
- To track sleep automatically with Apple Watch, wear a supported watch to bed and review the results afterward in Sleep.
A schedule can automatically enable Sleep Focus and provide Wind Down reminders across supported Apple devices. Sleep Focus helps manage interruptions; turning it on is not itself a measurement of sleep. Apple’s sleep tracking guide explains setup and available views.
Understand duration, stages, and score
- Sleep duration is Apple’s estimate of how long you slept; it may not equal time spent in bed.
- Sleep stages are estimates of Awake, REM, Core, and Deep periods based on sensor data and algorithms, not a laboratory polysomnogram.
- Sleep score is available on supported current models and summarizes factors such as duration, bedtime consistency, awakenings, and time in stages.
- Sleep-apnea notifications, where supported, signal a pattern that may merit clinical follow-up. They do not confirm sleep apnea, and absence of an alert does not rule it out.
Apple’s sleep-stage estimation methodology explains the estimates. Use stages and scores to look for patterns rather than treating a single night as a clinical result.
Make overnight tracking practical
The watch must be worn overnight and have enough charge to last. A dead battery means a missing night; charging only at bedtime conflicts with tracking. Charge before bed or after waking, and use fast charging where supported. A loose fit or poor skin contact can reduce data quality. Illness, alcohol, medication, travel, shift work, and unusual schedules can also change sleep or make comparisons less useful.

Persistent snoring, witnessed breathing pauses, gasping, or excessive daytime sleepiness deserve discussion with a clinician regardless of watch results.

On supported Apple Watch models, Vitals can bring together overnight heart rate, respiratory rate, wrist temperature, sleep duration, and blood oxygen where available. After collecting data over multiple nights, the watch establishes a typical range. If multiple metrics fall outside that range, it may send a notification with contextual factors such as illness, elevation changes, or medications. Apple’s Vitals guide describes the metrics and typical ranges.
A shift in several measures is a prompt to consider context, not evidence that one cause is responsible. Poor sleep, infection, altitude, training load, or medication timing can affect multiple metrics. Note symptoms and relevant changes, then discuss persistent or concerning patterns with a clinician rather than treating correlation as proof that a medication caused a change.
Blood oxygen availability has a U.S. model restriction: on Apple Watch models purchased in the United States on or after January 18, 2024, with part numbers ending in LW/A, Blood Oxygen is not available in the Vitals app. Feature availability differs by region and model; consult Apple’s current Apple Watch health-feature guide.

Which current Apple Watch fits these features?
As of August 18, 2026, Apple’s current U.S. lineup is SE 3, Series 11, and Ultra 3. The listed prices below are U.S. starting prices checked on that date; case, connectivity, materials, tax, promotions, and availability can change the price. Current models require iPhone 11 or later running iOS 26 or later; older watches can have different requirements.

SE 3 is the least expensive current Apple Watch for basic alerts and overnight tracking. Choose Series 11 if ECG or hypertension notifications matter and are available where you live. Ultra 3’s premium is chiefly for hardware, battery, and outdoor capabilities; its price does not mean it produces inherently more medically accurate readings. Check Apple’s model comparison, current U.S. lineup, and pricing announcement before buying.
If medication reminders, manual health entries, and organizing records are your main goals, start with the iPhone Health app you already have. If you need a measurement Apple Watch does not supply, ask a clinician about a suitable device or platform rather than assuming a wearable covers it.
Health-data privacy and sharing
Apple says health data on the device is protected by its passcode, Touch ID, or Face ID, while Health data synced to iCloud is encrypted in transit and at rest. Account security, who can access the device, third-party app permissions, and sharing or export choices still matter. Review which apps can read or write Health data, limit access to what an app needs, avoid sharing an Apple ID for separate users, and use a strong passcode and account security. Keep iOS and watchOS updated. Before sharing records with a clinician or exporting them, check what information is included and who will receive it. See Apple’s Health privacy overview.
When to involve a professional
- Ask a pharmacist or prescriber about medication interactions, dose changes, or an uncertain medication list.
- Contact a clinician about persistent heart-rate concerns or recurring alerts; seek urgent care for severe or alarming symptoms.
- Discuss suspected sleep apnea or persistent daytime symptoms with a clinician or sleep specialist even if the watch shows no notification.
- Use a qualified, appropriate medical device when you need measurements Apple Watch does not provide, such as direct blood-pressure measurement.
Apple Health is most useful as a personal record, reminder system, and way to notice trends. Its value depends on correct entries, a compatible data source, and interpreting results alongside symptoms and professional advice.
