When a retrieval-augmented assistant answers a question it cannot ground, the apology it gives can end up inside the next search query. In one engineering postmortem, an avatar repeated nearly the same apology across four consecutive user turns, because its previous apology was being fed back into retrieval. The author’s fix was to stop reusing an assistant answer unless the system had actually grounded it, rather than filtering apology wording.
What the failure looked like
The case is a first-person postmortem of a voice avatar built on retrieval-augmented generation. It was published on DEV Community on September 16, 2026, and states that it was originally written in Japanese at forge.workstyle.tech. The full post could not be retrieved for this article, so the details below come from its accessible text and are attributed to its author, Orca Forge. They describe one system and have not been independently verified.

The log shows four consecutive user turns. They began with the user asking whether the avatar could hear them, continued with a second question of the same kind, and ended with the user saying “I can hear you.” Each turn received nearly the same reply, which apologized and said the corresponding page description could not be found. The user was not being asked for anything the system needed; the system was failing to find content, and it kept failing in the same way.

The loop started with an earlier improvement. Underspecified follow-ups such as “Tell me more about that” carry almost no searchable topic on their own. To help with them, the system began appending the previous user utterance and the previous assistant answer to the next retrieval context. That change was meant to give vague follow-ups a subject.

In the example log, the previous answer was “Can you hear me? I apologize, but the corresponding description was not found.” The author’s diagnosis is that this text contained no topic of its own. Phrases such as “corresponding description” and “not found” then entered the next query and pulled retrieval toward generic material instead of the intended subject. The author summarizes the design problem in one line: “Designs that return output to input amplify when they fail.”
The reported sequence works as a loop:
- A question cannot be grounded in page content, so the assistant produces an apology.
- The user’s question and that apology are stored as the prior turn.
- The next query combines the user’s words with generic apology language.
- Retrieval misses the intended content, so the next answer is also ungrounded and also apologetic.
Each failed turn therefore supplied the material for the next one. The loop did not need a new fault to continue; it only needed the failed output to stay in context.
The fix: gate reuse on grounding, not on wording
The corrective rule is factual rather than phrase-based. An assistant answer is passed into later retrieval context only if that turn was grounded. The author chose this over a list of apology phrases for a practical reason: deployments use different templates, a phrase filter misses new variants, and it can also discard a useful answer that happens to contain an apology.
The implementation exposed a second problem. The grounding state had initially been updated during best-effort conversation-log recording. That meant an exception in logging could prevent the grounding state from being updated at all, leaving the system with a stale view of whether the previous turn was grounded. The author moved the update to the end of the turn, so it no longer depends on the logging path. The apology path was also marked, so that an ungrounded apology is never reused as context.

The lesson is to keep the grounding decision as an explicit piece of state, set by the turn logic, and make it independent of any logging step that can fail quietly.
Two “not found” states that look the same to users
The post separates two situations that a user would experience identically but that mean different things internally. Before the fix, the system could report that content was not found without having recorded whether the page text had been loaded at all.
| Condition | What it means | What the case required |
|---|---|---|
| Page text not available | Retrieval never obtained the page content, so there was nothing to match against. | Report this state separately and record that the load did not succeed. |
| Page text loaded, no matching content | The content was read, but nothing in it matched the question. | Record that the load succeeded, then respond with a no-match message. |
Keeping the two states apart matters for two reasons. It gives operators an accurate log of what happened, and it prevents a loading failure from being presented as evidence that the content does not exist.

Five checks for a follow-up pipeline
The author’s account suggests a set of checks for any system that reuses conversation history in retrieval. The table records how each check related to the failure in the case. The first four are issues the author encountered; the fifth is a recommendation that the post does not report testing.

| Check | Status in the case |
|---|---|
| Was the prior assistant answer grounded? | Not checked before reuse; ungrounded apologies were carried forward. |
| Does the follow-up query still contain the user’s topic? | Generic apology wording displaced the topic in the next search. |
| Was the page text actually loaded? | Not logged, so a “not found” reply could not be distinguished from a failed load. |
| Does grounding state survive a logging failure? | No at first; a logging exception could block the state update. |
| Is retrieval quality measured on representative follow-up turns? | Not stated in the accessible text. |
What the reported metric does and does not show
The post reports that an earlier retrieval improvement moved an internal measurement from 0.602 to 0.741. The accessible text does not name the metric, the dataset, the sample size, or the evaluation protocol, so these numbers cannot be read as a standardized benchmark or as a general measure of performance. They are one team’s internal figures for one system, and they should be read that way.

A 2025 review of AI apology research treats apology as one possible trust-repair step. Its findings are mixed across apology components and contexts. It reports that optimistic language about reform can raise trust at first but frustrates users when the system does not improve. A more realistic admission of limitation was described as less frustrating and more believable in the study it discusses. The review also notes that long-term research on repeated AI apologies is limited.
A separate 2021 exploratory study of conversational assistants found that “cannot help” responses do not guide users toward a next question. In that study, moving on without an explicit acknowledgment of the misunderstanding was the most successful recovery strategy.
Both studies concern how users respond to apologies and failures. Neither examines whether apology text contaminates retrieval queries, so they provide context for the user-facing side of the problem but do not validate the retrieval fix.

Where the same pattern can appear
The author’s principle applies whenever generated output becomes input to a later step: a conversation summary that is summarized again, generated examples that re-enter a training set, or a search result used as the next query. The post raises these only as analogies, and the sources available for this article do not evaluate them. The practical advice that follows from the case is to check provenance and evidence status before generated content is reused.
What this case does and does not establish
The case establishes one mechanism in one system: an ungrounded assistant answer, once carried into the next retrieval context, can push the following search away from the user’s topic and produce another ungrounded answer. The grounding-gated fix addresses that mechanism as the author describes it.
It does not establish that all apologies or all retrieval-augmented systems behave this way. It is a single author’s account, without independent replication or a larger benchmark, and the reported gain is not a validated effect size. Readers adapting the approach should test it against their own follow-up turns before relying on it.
