Nitrogen dioxide (NO2) and ozone (O3) also show smaller associations. The available evidence does not establish that hot or cold weather independently causes type 2 diabetes; temperature appears mainly to track season and alter the pollution mixture.
What the strongest evidence shows
A 2025 global systematic review and meta-analysis by Azizi, Dehghani and Nabizadeh assessed 46 studies and found a significant association between long-term exposure to PM2.5 or PM10 and type 2 diabetes. These studies address the chance of developing diabetes over time, not whether one polluted day produces diabetes.

An umbrella review published in Environmental Research in 2025 also found positive associations for several pollutants. Its pooled estimates are odds ratios (ORs), and the exposure increment was not standardized in the summary across all underlying studies, so the figures should be read as evidence of relative association rather than a personal risk calculator.
| Pollutant | Pooled association with type 2 diabetes | What it means |
|---|---|---|
| PM2.5 | OR 1.12 (95% CI 1.09–1.15) | The most consistent signal in the evidence base; linked with long-term diabetes risk. |
| PM10 | OR 1.12 (95% CI 1.06–1.18) | Associated with long-term risk; this was the only pollutant with a statistically significant dose-response relationship in that umbrella analysis. |
| NO2 | OR 1.09 (95% CI 1.07–1.12) | A modest positive association, subject to differences among locations and study methods. |
| O3 | OR 1.05 (95% CI 1.03–1.08) | A smaller pooled association than for particulate matter. |
| CO | Not stated in the pooled estimates summarized here | Lagged associations were reported in a separate short-term hospitalization study. |
Because these are observational results, an OR above 1 does not prove that pollution caused diabetes in a particular person. It indicates that higher measured exposure and diabetes occurred together more often than lower exposure and diabetes in the analyzed populations.

Why PM2.5 and PM10 are biologically plausible risks
The leading explanation is that inhaled particles can contribute to systemic inflammation and oxidative stress. The 2025 systematic review also identifies endoplasmic-reticulum stress as a proposed pathway. These processes may interfere with insulin signaling and glucose regulation, creating conditions associated with type 2 diabetes.
Those mechanisms are hypotheses supported across the literature, not proof that every exposed person will develop diabetes. Individual risk still depends on factors such as genetics, body composition, activity, diet, medications and other environmental or social conditions that the studies may measure differently.

Short-term pollution and diabetes hospitalizations
Long-term incidence is not the only outcome reported. Hou and colleagues analyzed 13,791 patients with type 2 diabetes in a time-series study in Xinxiang, China, published in Frontiers in Endocrinology in 2024. They examined daily pollution, weather and diabetes hospitalizations.

The estimate concerns hospitalization among people with diabetes during the study period; it does not mean that a 3.1% increase is expected for every person or every location. The study also found lagged associations for NO2, carbon monoxide and O3.

A hospitalization signal and a new-diagnosis signal answer different questions. Short-term analyses can detect changes in acute healthcare use after exposure, while prospective or long-term studies examine whether sustained exposure is associated with developing type 2 diabetes.
What temperature has to do with the association
Temperature is important context, but the evidence summarized here does not provide an independently estimated causal effect of temperature on developing type 2 diabetes. In the Xinxiang analysis, mean temperature was 16.23 °C, with a range from −8 °C to 34.6 °C. Temperature was negatively correlated with most measured pollutants, meaning colder and warmer periods tended to have different pollution mixtures.

Several pollutant associations were larger during the cold season. That pattern can arise because temperature changes pollutant concentrations, human behavior, atmospheric conditions and seasonal illness at the same time. It can also reflect confounding if a study cannot fully separate those factors. Therefore, “hot weather causes diabetes” and “cold weather causes diabetes” are not conclusions supported by these findings.
How to interpret the numbers without overclaiming
- Relative, not absolute, risk: An OR or RR describes a change relative to another exposure level. It does not give an individual’s probability of developing diabetes.
- Different pollutants are not interchangeable: PM2.5, PM10, NO2, O3 and CO come from different sources and behave differently in the atmosphere and body.
- Exposure windows matter: A seven-day lagged exposure and years of average exposure cannot be compared as if they were the same measurement.
- Location matters: Results from Xinxiang cannot automatically be applied to every city, climate or pollution source profile.
- Statistical association is not proof of causation: Exposure measurement errors, socioeconomic differences, geography, season, humidity and other health determinants can influence the estimates.
The umbrella review authors called for better prospective research. More consistent personal-exposure measurements and clearer adjustment for weather, season and socioeconomic factors would help determine how much of the observed association is attributable to pollution itself.

What readers can reasonably conclude
- Fine-particle pollution has the most consistent relationship with type 2 diabetes across the cited evidence.
- NO2 and O3 are associated with diabetes in pooled analyses, but their estimated effects are smaller and vary by study.
- Short-term pollution spikes may coincide with more diabetes-related hospitalizations, particularly when exposure is accumulated over several days.
- Temperature should be treated as a meteorological modifier or confounder of pollution studies, not as an independently proven cause of type 2 diabetes.
- These findings describe population-level risk patterns and cannot diagnose diabetes or predict an individual’s outcome.
