AI Medical Scribes vs. Human Medical Transcriptionists: Privacy and Accuracy Compared

Neither AI medical scribes nor human medical transcriptionists can be called categorically more accurate or private on the available evidence. AI scribes may do more than transcribe: they can summarize and structure a draft clinical note, which creates additional ways to omit or invent information. Human transcriptionists turn recordings or dictation into text, but the sources reviewed here do not provide a matched accuracy or privacy comparison. In either workflow, the organization must assess the service, protect patient information, and ensure the clinical record is appropriately reviewed.

What is being compared?

An ambient AI scribe listens to a patient-clinician encounter and typically produces a draft note. A human medical transcriptionist turns a recording or clinician dictation into text. These are related documentation workflows, but they are not identical: a transcript aims to represent speech, while a clinical note may select, summarize, organize, and interpret information.

That distinction matters. Speech recognition can mishear words; summarization can omit details; note generation can introduce findings or present uncertain information too confidently. A transcript’s accuracy alone therefore does not establish whether the final clinical note is accurate or safe.

Comparison point Ambient AI scribe Human medical transcriptionist
Typical task Listens to an encounter and creates a draft note, potentially through transcription, summarization, and structuring. Turns a recording or dictation into text.
Potential error types Misheard speech, omitted information, misplaced content, invented findings, or loss of clinical uncertainty. Transcription errors are possible; the sources cited here do not establish a comparable error profile or rate.
Head-to-head accuracy result Not established. The cited studies evaluate AI tools, not matched AI and professional-transcriptionist outputs. Not established. The cited studies do not compare transcriptionists and AI on the same recordings using shared definitions.
Privacy status Depends on what data the service handles, its uses and safeguards, and the applicable agreement and risk assessment. Also depends on what data the service handles, its uses and safeguards, and the applicable agreement and risk assessment.

The practical comparison is between specific services and workflows—not “AI” versus “human” in the abstract.

How accurate are AI medical scribes?

Published evaluations show that quality can vary by platform, task, specialty, and study setting. They do not support one universal accuracy rate for all AI scribes. They also show why a clinician should treat generated text as a draft requiring review, not as an automatically final record.

Transcript errors can flow into the note

A 2025 simulated ambulatory study of four ambient-scribe platforms reported an average of 13.9 transcript errors across the tested platforms; 19.5% of transcript errors reached the clinical note. Those are results from that study’s simulated encounters, not a universal rate or a comparison with human transcriptionists. The study record also reported variation among platforms and in documentation quality.

Note quality is not the same as word recognition

A 2026 study evaluated AI-generated notes from simulated encounters in cardiology, gastroenterology, hematology-oncology, and neurology. Sixteen clinicians—four per specialty—rated the notes at a Veterans Health Administration simulation center in February 2025. The mean overall PDQI-9 note-quality score was 36.2 out of 50 (SD 10.9); ratings were highest for succinctness and lowest for thoroughness. Specialty scores differed descriptively, but the difference was not statistically significant (ANOVA P=.09). The authors identify simulation, the limited specialty set, and lack of EHR integration as limitations. These results are a controlled stress test, not an estimate of routine performance across products and practices. (JAMA Network Open study)

Clinicians in that study described concerns including fabricated examination findings, missing pertinent negatives, and failure to convey diagnostic uncertainty. One hematology-oncology participant said: “Or [for] allergies—it said ‘none,’ and it wasn’t asked. Like, those are big safety issues.” This is a qualitative comment from a simulated evaluation, not a measured error rate. Participants also saw possible benefits such as less typing and better eye contact, while noting that access to prior notes, tests, and EHR workflows could affect usefulness. Evaluate the whole pipeline

A 2025 instrument-validation study raised concerns about AI-enabled scribe output, including misplaced or irrelevant material and content that may be technically correct but inappropriate in context. A separate 2025 framework paper evaluates ambient scribing across transcription, diarization (identifying who spoke), and note generation, and reports shortcomings in factual accuracy and capturing new medications. Together, these sources support assessing each stage rather than relying on a single blended “accuracy” label; they do not establish that every product has the same failure profile. (JMIR study; npj Digital Medicine framework)

Are AI scribes more accurate than human transcriptionists?

The available studies do not answer that question. None of the cited primary sources directly compares AI-scribe output with professional human medical transcriptionist output from the same encounters using shared error definitions. The 2026 specialty study assesses AI-generated notes rated by clinicians; it is not a comparison against transcriptionists.

Nor should a transcriptionist’s text be treated as equivalent to a clinician-authored or clinician-edited note. The roles and outputs differ. To compare services at a particular organization, use the same representative recordings, specialty mix, error taxonomy, and review standard. Measure transcription mistakes separately from summarization omissions, unsupported additions, formatting problems, and clinical interpretation. Also record correction time and who approves the final note. Without that kind of matched audit, a broad accuracy winner is not established.

Are AI scribes and transcription services HIPAA compliant?

In the United States, privacy obligations follow the data flow and the service relationship, not whether the vendor uses AI. HHS identifies independent medical transcriptionists and app vendors providing transcription services to physicians as examples of business associates when their work involves protected health information (PHI). A cloud service processing or storing electronic PHI for a covered entity or business associate is also generally a business associate. The relevant parties generally need a compliant business associate agreement (BAA), and the covered organization must understand the service and conduct its own risk analysis. See HHS’s guidance on business associates and cloud computing.

A vendor’s “HIPAA compliant” marketing statement is not an HHS product endorsement and does not, by itself, show that a service suits a particular organization. HHS’s Office for Civil Rights states: “OCR does not endorse, certify, or recommend specific technology or products.” HHS also explains that encrypting data without giving a cloud vendor the key does not by itself remove business-associate status when the vendor maintains electronic PHI. A BAA is part of the arrangement, not a substitute for assessing the risks and actual service terms. (HHS cloud guidance)

Questions to ask about data and contracts

  • Lifecycle: Is audio retained? Are transcripts and generated notes stored separately? How long is each kept, and what happens to the data at termination?
  • Permitted use: Do the BAA and service terms allow model training, product improvement, analytics, or other secondary uses?
  • Access and subcontractors: Which vendor staff, cloud providers, or subcontractors may handle the information, under what contracts, and with what controls?
  • Security and incidents: What access controls, safeguards, audit evidence, breach-notification commitments, and backup and recovery arrangements apply?
  • Return and deletion: How are data returned or deleted, and what retention limits apply?

HHS says BAAs and related service terms can address permitted uses and disclosures, safeguards, availability, backup and recovery, security responsibilities, data return, retention, and disclosure limits. The covered organization remains responsible for assessing its own risks. (HHS cloud guidance)

What should a clinic check before choosing a workflow?

Privacy paperwork and a product demo do not show whether a tool produces usable notes in a clinic’s real conditions. Evaluate the service with representative encounters, define who reviews and approves every draft, and check whether it fits the existing recordkeeping workflow.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.
  • Accuracy: Test for omissions, unsupported additions, misattributed speech, lost uncertainty, and errors involving medications, allergies, or pertinent negatives.
  • Review and correction: Confirm that a clinician reviews the draft before it enters the record. Track correction time as well as errors; a tool may shift effort from typing to checking.
  • Workflow fit: Determine whether the service can use prior notes, medications, test results, templates, and the relevant EHR fields. A note based only on the conversation may not suit a follow-up that depends on substantial chart context.
  • Patient communication: Establish what notice or consent process applies in the setting and jurisdiction. The sources here do not settle every recording-consent rule.
  • Equity and accessibility: Test with the accents, regional dialects, speech impairments, specialties, and encounter types the service will actually face.

NHS England’s guidance, published April 27, 2025, advises UK health and care organizations to assess product evidence and accuracy—including errors, omissions, and dysfunctions—and account for accents, regional dialects, and speech impairments. It recommends clear governance, accountability, human oversight, auditing, and record-keeping. This is UK guidance, not a U.S. legal rule. (NHS England guidance)
