The best AI medical scribe is not the one with the highest headline accuracy. It is the one that produces clinically faithful drafts, fits the way clinicians actually see patients, and moves approved information into the correct fields of the target EHR with acceptable governance and correction effort. Compare candidates with the same representative encounters, require clinician review before chart submission, and validate every integration claim in your own EHR configuration.
What an AI medical scribe does
An ambient medical scribe listens to a clinical conversation and generates a transcript, summary, or draft clinical note. The clinician then reviews, edits, and approves that draft before it becomes part of the legal medical record. It is a documentation aid, not an autonomous author of the chart or a substitute for clinical judgment.

The distinction matters because speech recognition can fail in clinically important ways. NHS England guidance published April 27, 2025 and updated July 29, 2026 warns that accents, regional dialects, speech disorders, and other impairments can affect transcription. A fluent-looking note can still omit a symptom, reverse a negation, misidentify a speaker, or attach a medication instruction to the wrong person.
Products also differ in what happens after drafting. Some leave clinicians to copy or paste a narrative note into the EHR. Others provide an interface that sends approved text through an integration, and some map selected structured content to fields such as problems, medications, or orders. Treat those as different workflows, not interchangeable versions of “integration.”

Compare clinical accuracy as fidelity and correction burden
There is no independently validated, common accuracy leaderboard in the available evidence. A vendor percentage therefore cannot rank products by itself. Compare the errors that could change care and the time clinicians spend finding and fixing them.
Test the errors that matter clinically
- Omissions: missing symptoms, allergies, examination findings, follow-up instructions, or safety-netting language.
- Negation: “no chest pain” becoming “chest pain,” or a family history being recorded as the patient’s history.
- Speaker attribution: confusing a caregiver’s statement with the patient’s statement, or attributing a clinician’s plan to the patient.
- Medication detail: wrong drug, dose, route, frequency, duration, or status such as “stopped” versus “started.”
- Terminology: misspelled diagnoses, procedures, anatomy, abbreviations, or specialty-specific terms.
- Context: failing to distinguish historical information from today’s assessment or a planned test from a completed test.
Use clinicians from the specialties and roles that will use the system. Have them score each draft against the conversation or an agreed reference note, and classify every substantive correction by clinical risk. A shorter note is not better if it removes details needed for continuity, coding, or informed consent.
Build a representative test set
Do not test only a quiet, single-speaker office visit. Include routine and difficult encounters: preventive care, chronic disease follow-up, medication reconciliation, urgent complaints, multiple speakers, interpreter-assisted visits, telehealth audio, and visits with background noise. Include the accents, dialects, languages, and speech patterns present in your patient population. If a candidate advertises English and Spanish support, test the actual language combinations and switching behavior your clinicians use rather than accepting the label as proof of performance.
Measure correction effort
Record the time from draft arrival to clinician approval, the number of substantive edits, and the number of safety-critical corrections. Separate cosmetic edits from changes to diagnoses, medications, history, examination, assessment, and plan. A useful local metric is correction minutes per encounter, reported alongside critical-error rate and omission rate. Have reviewers work from the same instructions and, where possible, blind them to the product name.
Interpret vendor accuracy claims correctly
Ambient Scribe’s product page states “95%+ medical terminology accuracy.” That is a vendor claim, not an independently validated comparison with other tools. Ask what dataset, specialty mix, language, definition of accuracy, and review process produced the figure. The help center and product materials describe transcript and documentation generation for clinician editing and approval; they do not establish a universal performance level across practices.
Evaluate the complete clinical workflow
Workflow fit is the entire path from encounter capture to a signed chart. A tool that creates an excellent draft but adds several screens, manual downloads, or duplicate data entry may increase rather than reduce documentation burden.

- Wireless voice recording Microphone
- You can easily move up to 5 meters or 16 feet away from your workstation and your recordings are safely transmitted to your computer in highest quality, without any interruptions.
Observe capture in real conditions
- Confirm whether recording starts in the examination room, from a mobile device, through a browser, or inside the EHR.
- Test in-person and virtual visits, including reconnects, pauses, and late starts.
- Check how the system handles multiple speakers and whether participants receive the required notice or consent prompt.
- Measure draft latency: when the conversation ends, when the draft appears, and whether clinicians can begin review before leaving the encounter.
Inspect templates and editing
Templates should match the note types your clinicians actually use, such as problem-oriented, preventive, specialty, or telephone notes. Check whether headings, required fields, macros, and organization-specific phrasing are configurable without vendor intervention. Test editing in the same sequence clinicians follow: skim for omissions, correct facts, adjust assessment and plan, add orders or instructions, and approve.

Those capabilities can vary by account, release, and EHR configuration, so require a demonstration in the intended deployment rather than relying on a generic tour.
Count friction, not just clicks
During a pilot, count extra screens, copy-and-paste actions, re-authentication events, duplicate entry, and interruptions to patient interaction. Ask clinicians whether they can keep eye contact and conversation flow while capture is active. Include the time required to correct a poor draft; that often dominates the apparent time saved by automatic transcription.
Determine what “EHR integration” really means
Ask vendors to describe the exact destination of every output. “Works with your EHR” can mean a copied narrative, a document attachment, a one-way note transfer, or structured write-back into mapped fields. These options have different safety, coding, search, and downstream-workflow consequences.
Questions to ask about the connection
- Which EHR product, edition, release, and environment are supported?
- Does the tool launch in the correct patient and encounter context, or must staff search for the chart again?
- Is data transferred one way or bidirectionally?
- Which fields are supported: note body, history, examination, assessment, plan, medications, problems, diagnoses, orders, or charges?
- Are outputs narrative only, or are coded elements mapped to the EHR’s structured terminology?
- What authentication, single sign-on, browser, mobile, and network requirements apply?
- What configuration limits, unsupported fields, or regional restrictions should be expected?
- What happens when the connection fails: can the clinician safely retrieve, edit, and submit the draft later?
Test the target environment
Run the demonstration and pilot in a non-production or controlled setting that mirrors the live build. Verify patient matching, encounter selection, note location, author attribution, timestamps, amendment behavior, and audit trail. Confirm whether approval in the scribe interface is equivalent to signing in the EHR or whether a separate EHR signature remains mandatory.
Vendor documentation describes mappings and workflows for TouchWorks and Veradigm and notes that supported fields and actions vary by EHR and configuration. That is useful evidence that integrations exist, but it is not proof that your organization’s version, modules, permissions, or custom fields will behave the same way.

Use interoperability standards as a governance question
NHS England guidance recommends attention to EHR interoperability and standardized terminologies, including FHIR and SNOMED CT. Ask whether a product uses those standards for your deployment, what resources and codes are exchanged, and how local terminology maps are maintained. A standards label alone does not guarantee that the fields clinicians need are exposed or writable.
A repeatable comparison method
Use one protocol for every candidate. Keep the scoring visible to clinical, operational, privacy, and IT reviewers.
- Define the decision. List the specialties, visit types, languages, locations, patient ages, virtual-care requirements, and note types in scope.
- Freeze the test set. Select representative recorded or simulated encounters, including difficult speech, multiple speakers, medication changes, negation, and specialty terminology. Obtain the permissions and notices required by your organization.
- Run identical encounters. Use the same audio, instructions, template expectations, and reviewer roles for each candidate. Do not let one vendor optimize the test after seeing another vendor’s results.
- Score clinical fidelity. Track omissions, invented content, negation errors, speaker errors, medication errors, terminology errors, and clinically significant corrections.
- Time the workflow. Measure capture setup, draft latency, review time, editing time, approval, and final chart submission. Count extra screens and re-entry.
- Validate integration. Demonstrate the exact EHR version and configuration. Confirm patient context, supported fields, structured versus narrative write-back, authentication, and failure recovery.
- Review governance. Obtain current contracts and technical documentation for privacy roles, consent, retention, access, auditability, subprocessors, data use, and incident response.
- Run a controlled pilot. Start with a defined group, monitor safety events and correction burden, collect clinician and patient feedback, and set exit criteria before expanding.
Governance, privacy, and security checks
Accuracy does not make an ungoverned recording system acceptable. Identify who acts as data controller and processor, where recordings and transcripts are stored, how long each is retained, and whether audio is deleted after drafting or retained for another purpose. Require a clear account of whether customer data is used to train models and whether that use can be disabled.
Do these 3 things before closing this tab:
1Fix the driver behind crashes, sound loss and screen glitches2Repair Windows errors before they cause bigger problems3Scan for outdated or missing drivers - takes under a minuteReview consent and notice for the jurisdictions and care settings in which the tool will operate. Decide how staff will handle a patient who declines recording, and provide a documented fallback workflow. Check role-based access, encryption, administrator controls, audit logs, export and deletion procedures, subprocessor lists, breach notification commitments, and business-continuity arrangements.
Clinical governance should define who reviews drafts, when a note may be rejected, how corrections are documented, and how incidents are escalated. Include a process for reporting systematic errors affecting a specialty, language, or patient group. Vendor security or compliance language is not a substitute for your organization’s legal, privacy, and clinical risk assessment.

Prepare clinicians and staff
Training should cover microphone placement, patient notice, starting and stopping capture, handling interruptions, reviewing high-risk content, correcting structured fields, and completing the EHR signature step. Provide specialty-specific examples instead of a single generic demonstration. Make the fallback process as easy as the new process, or staff will improvise when the system is unavailable.

Set measurable pilot gates
Define acceptable correction minutes, critical-error thresholds, draft latency, availability, and integration success before the pilot starts. Review results by clinician, specialty, language, visit type, and location; averages can hide failures for smaller groups. Include patient experience and clinician trust, but do not allow satisfaction scores to override unsafe documentation errors.

How to interpret the available comparative evidence
A 2026 comparative-effectiveness study analyzed 163 primary care providers and 59,130 provider-days from January 2024 through June 2025. Those figures describe the study cohort and observation period; they are not a product accuracy score or proof that one tool is best for every organization. The study’s central practical value is the reminder that integrated and external-interface workflows can differ in documentation burden and productivity over time.
A 2025 policy brief on ambient AI scribes and coding describes a contrast between native EHR integration and workflows in which notes are transferred into the EHR later. Use that distinction when questioning vendors, but do not treat examples in a policy brief as a complete market census. A 2026 narrative review frames ambient scribes as a technology, impact, and implementation issue rather than a single performance number.
Procurement checklist
- Clinical: Have clinicians tested representative encounters and documented critical-error and correction rates?
- Workflow: Is capture acceptable in every intended setting, with manageable latency and review time?
- EHR: Are the exact product version, patient context, fields, mappings, signature steps, and failure paths demonstrated?
- Interoperability: Are FHIR, SNOMED CT, or other standards used in a way that supports your required structured data?
- Governance: Are consent, retention, data use, access, audit, subprocessors, and incident terms documented for your deployment?
- Equity: Have accents, dialects, speech impairments, languages, and accessibility needs been tested?
- Operations: Are training, support, configuration ownership, version control, and rollback procedures assigned?
- Commercial: Have you obtained current pricing, implementation fees, minimum commitments, renewal terms, and data-exit provisions directly from the vendor?
Bottom line
Shortlist AI medical scribes by clinical fidelity, correction burden, workflow friction, and verified EHR behavior—not by a single accuracy percentage. Require clinician review and approval, test the voices and visits your organization serves, and validate structured write-back in the exact EHR configuration you will deploy. Because independent cross-vendor accuracy results and current contract terms are not established consistently, a local, controlled evaluation is the only defensible way to choose a product.
