AI in Healthcare: How to Assess Safety and Readiness

Healthcare is not ready for AI as a blanket category—and it does not need to be. Some systems may be useful for a clearly defined task when they have been tested on the patients and conditions they are meant to serve, used with appropriate human oversight, and monitored after deployment. A convincing demonstration or a high score on a quiz is not enough to show that a tool is safe in clinical care.

The risks are real: an AI system can produce inaccurate or biased results, expose sensitive information, or lead people to trust a plausible answer too readily. Whether those risks are acceptable depends on what the system does, who it affects, and how a healthcare organization manages it.

What AI is used for in healthcare

“AI in healthcare” covers different technologies and jobs, not one product or level of risk. The World Health Organization (WHO) describes uses in diagnosis and screening, clinical care, research and drug development, public health, and health-system management. A tool that helps record a note is not making the same kind of decision as one that helps assess a patient’s condition.

Use area Possible task Why the context matters
Diagnosis and screening Help identify patterns in information used to assess a condition. A missed finding or an incorrect alert may affect what happens next; performance needs to be evaluated for the intended use.
Clinical care Predict a health trajectory, recommend treatment, guide aspects of surgical care, or monitor a patient. The consequences vary with the decision and the extent to which clinicians rely on the tool.
Research and drug development Support research or parts of pharmaceutical development and delivery. Research support is not the same as proof that a system improves patient outcomes in routine care.
Public health and health-system management Support population-health work, resource planning, or administrative tasks such as note recording and workflow automation. Errors can affect people indirectly through decisions about services, records, or how work is organized.

These examples describe possible applications, not a current measure of how widely each is used. The U.S. Government Accountability Office (GAO) described clinical and administrative tools in a 2020 review, while cautioning that tools were at different stages of maturity and adoption. That dated review should not be read as a present-day adoption estimate.

WHO’s 2024 overview notes that AI may help address workforce gaps and resource constraints. That is a potential benefit, not evidence that a particular deployment improves care. Results depend on the tool, the setting, and the way people use it.

Why a strong demonstration does not prove clinical readiness

A system can perform well on a test and still be unreliable in the situations that matter to patients. Tests may use a narrow set of examples, while clinical decisions involve varying patients, incomplete information, and consequences for which a plausible but wrong answer can be harmful. The relevant question is not simply whether a model can answer correctly, but whether it performs dependably for its defined purpose and population.

A concrete example comes from a study summarized in a July 23, 2024, National Institutes of Health (NIH) news release. Researchers at the National Library of Medicine and Weill Cornell Medicine evaluated GPT-4V on medical quiz questions that included clinical images and short patient summaries. The model answered with high accuracy, but physician graders found that it often misdescribed images and gave flawed reasoning—even when its final answer was correct.

That was a quiz evaluation, not a clinical deployment study. It does not establish how the system would perform in patient care, and it should not be generalized to every healthcare AI tool. It does illustrate why a correct-looking result alone may not reveal whether the system reached it reliably.

The main risks behind AI’s “dark side”

Wrong, incomplete, or misleading output

AI can produce statements that are false, inaccurate, biased, or incomplete. In a health context, a confident-sounding answer may influence a patient’s choices or a professional’s judgment even when it is mistaken. WHO’s 2024 guidance on large multimodal models warns that these failures can affect health decisions.

Unequal performance and bias

AI systems learn from data, and limitations or bias in development data can make a tool less safe or effective for some patient groups than for others. GAO has described how such differences can contribute to treatment disparities. The National Institute of Standards and Technology (NIST) warns that AI can increase the speed and scale of harmful bias; its 2022 Special Publication 1270 was an initial step toward identifying and managing that risk.

This does not mean every healthcare AI system is biased. It means that performance should be evaluated across relevant patient groups, rather than assumed to be equal because an overall result looks good.

Automation bias and misplaced trust

Automation bias occurs when people give an automated recommendation too much weight or overlook signs that it is wrong. WHO warns that both patients and professionals may defer to a model or miss its errors, especially when a decision is difficult. Human involvement only helps if the person can understand the tool’s role, question its output, and remain accountable for the decision.

Privacy and cybersecurity

Healthcare data is sensitive. WHO’s guidance identifies risks involving unethical collection and use of health data, as well as cybersecurity threats to patient information and the trustworthiness of systems. Before a tool is used, an organization needs to consider what information it receives, how that information is protected, and what could happen if the system or data is compromised.

Limited transparency and accountability

If clinicians and patients do not know what a tool is intended to do, what its limitations are, or how its output should be interpreted, they may rely on it inappropriately. In 2024, the U.S. Food and Drug Administration (FDA), Health Canada, and the U.K. Medicines and Healthcare products Regulatory Agency (MHRA) issued transparency principles for machine-learning-enabled medical devices. The principles emphasize sharing information that could affect risk and patient outcomes with the people who interact with a device.

Wider ethical and environmental concerns

WHO’s 2021 report on AI for health also identifies environmental effects among the issues to consider. Its broader ethical principles include protecting human autonomy, promoting safety and the public interest, ensuring transparency and responsibility, supporting inclusiveness and equity, and responding to sustainability concerns.

How to tell whether a healthcare AI use is ready

There is no single readiness score that applies to every healthcare AI application. A practical assessment starts with the specific task and asks whether the evidence, safeguards, and accountability fit the consequences of using the tool.

  1. Define the intended use. What decision or task is the system meant to support, and what is outside its role? A broad claim that a tool “helps with healthcare” is not enough to judge its performance or risk.
  2. Check the evidence for the intended population and setting. Has it been evaluated for the patients, conditions, and environment where it will be used? An overall score or success on a different task does not answer that question.
  3. Understand its limitations and communicate them. Clinicians and patients need information that helps them interpret the output and understand when not to rely on it. Transparency matters most when a limitation could change a decision or its consequences.
  4. Keep a responsible person involved in consequential decisions. Clarify who makes the final decision, how the AI output is considered, and how a person can challenge or override it. Oversight should be meaningful, not a formality.
  5. Protect information and systems. Assess privacy and security risks in the actual deployment, including how patient information is handled and how the organization would respond to a security problem.
  6. Monitor performance after deployment. Track whether the system continues to work as intended, including whether outcomes or performance differ across relevant patient groups. Deployment is not the end of safety evaluation.
  7. Prepare to respond when something goes wrong. The organization should be able to investigate errors, act on them, and reconsider use when the evidence or circumstances change. FDA Commissioner Robert M. Califf has emphasized the need for safety parameters, standards, risk frameworks, and lifecycle safety monitoring.

WHO’s guidance also stresses stakeholder engagement and appropriate laws and oversight. Readiness therefore depends not just on the model, but on whether the people and institutions around it can govern its use.

So, are we ready?

Healthcare is ready for some AI uses only when the particular system has evidence appropriate to its intended task and population, its risks are managed, and people remain able to question and oversee consequential decisions. It is not ready to treat AI output as inherently trustworthy, or to infer clinical safety from a polished demonstration, quiz result, or general promise of efficiency.

The best question is not whether AI is safe or unsafe in the abstract. Ask what this system is being used to do, what evidence supports that use, who might be harmed by an error, and who is responsible for noticing and responding when the tool fails. Without clear answers, readiness has not been established.
