AI in Breast Cancer Care: Better Screening, Not Yet a Treatment Plan

AI-supported breast screening has found more cancers in some real-world studies, but those gains do not yet show that AI prevents deaths, eliminates disparities or carries a patient from a mammogram to a personalized treatment plan. Today, its clearest role is at specific points in care—especially reading or triaging screening images—while clinicians combine biopsy, pathology and patient information to make treatment decisions.

Can AI detect breast cancer on a mammogram?

AI can help clinicians identify suspicious findings, but its performance depends on the software, the imaging workflow and the population in which it is used. In a large US deployment, an AI-supported workflow detected more cancers per 1,000 screening exams than the practices’ prior standard workflow. That is evidence of improved detection in that setting, not proof that AI alone caused the difference or that more cancers found will translate into longer lives.

The 2025 ASSURE study covered digital breast tomosynthesis at five practices and 109 sites. It compared 208,891 exams in the AI-supported workflow with 370,692 earlier exams under standard care. The workflow used AI detection and diagnosis alongside a radiologist safeguard review of selected cases; it was a prospective deployment compared with historical care, not a randomized trial.

Measure AI-supported workflow Prior standard care What the result means
Cancers detected per 1,000 exams 5.6 4.6 21.6% higher detection rate in the AI-supported period
Recall rate 11.1% 10.6% A modestly larger share of people were called back for further assessment
Positive predictive value among recalled exams 5.0% 4.4% A recalled exam was more likely to be associated with cancer in the AI-supported workflow

These figures are from the ASSURE investigators’ 2025 US deployment study; because the comparison was before-and-after, changes cannot be attributed to AI alone. The detection increase also came with a small rise in recalls, so it should not be described as a gain with no additional follow-up. Read the ASSURE study in Nature Health.

A separate German implementation study, PRAIM, found 6.7 cancers per 1,000 women screened in the AI-supported group and 5.7 per 1,000 in the control group. It studied organized screening for women aged 50–69, where radiologists voluntarily used AI-supported double reading at some sites. Because this was observational and use was voluntary, it does not establish that AI caused the difference; its double-reading setting also differs from US single-reading practice. Read the PRAIM study in Nature Medicine.

Does AI work better for dense breasts?

Dense breast tissue can make mammograms harder to interpret. In ASSURE, investigators found no measured differences in cancer detection, recall or positive predictive value across the racial and breast-density subgroups they studied. That is a useful result about subgroup performance in this particular deployment. It does not show that AI performs equally in every health system or that it removes barriers to screening, diagnosis or treatment.

Using AI to select people for supplemental MRI

AI is also being studied as a way to identify people who might benefit from additional imaging after a negative mammogram. In Sweden’s randomized ScreenTrustMRI study, an AI score selected a small, high-risk group for supplemental MRI. The study reported 64 cancers detected per 1,000 MRI examinations, compared with 16.5 per 1,000 in an earlier trial that used breast-density measures to select participants. That is a comparison across trials, not a randomized head-to-head comparison of the two selection methods. The reported MRI cancer-detection result does not establish a survival benefit; the study’s primary outcome concerned advanced cancers after follow-up. Can AI miss breast cancer?

Yes. An AI result is not a guarantee that cancer is present or absent. A 2026 UK evaluation assessed Google mammography AI version 1.2 retrospectively on 115,973 mammograms and also included a prospective, noninterventional feasibility deployment. In the retrospective comparison with first-reader results, reported sensitivity was 0.541 for the AI system and 0.437 for the first reader; specificity was 0.943 and 0.952, respectively. Sensitivity reflects how often cancers are identified, while specificity reflects how often people without cancer are correctly classified. These results describe that system and evaluation dataset, not a universal performance level for mammography AI. The feasibility deployment was not a randomized demonstration of patient outcomes, although the study reported that the system detected some interval cancers. Because screening tools can miss disease, an AI output should be interpreted within the clinical workflow rather than treated as a diagnosis. If a person has a concerning symptom or finding, screening AI results alone are not a substitute for clinical evaluation.

Does AI help doctors decide treatment?

AI research may help extract information from pathology images or integrate patient data, but that is different from a proven system that independently selects a treatment. A screening mammogram is only one step: a suspicious finding may lead to diagnostic imaging and biopsy. Once cancer is diagnosed, treatment decisions can depend on stage, histology, grade, hormone-receptor and HER2 status, gene-profile tests and other clinical factors. The National Cancer Institute describes AI applications in cancer diagnosis and research, while its breast-cancer treatment guidance lays out the broader factors involved in care. NCI: AI and Cancer; NCI: Breast Cancer Treatment.

The FDA classifies certain pathology software that analyzes digitized images and provides prognostic risk estimates to assist physicians as a medical device; the listed intended use is not to determine a clinical diagnosis. Regulatory status and permitted use depend on the specific software and its intended purpose. The FDA’s clinical decision-support framework also makes clear that functions analyzing medical images or providing a specific patient treatment plan may be subject to device oversight. This is why a general claim that a product “uses AI” does not establish that it is validated or authorized for a particular clinical task. Can AI reduce differences in breast cancer outcomes?

It may contribute to better detection in a workflow, but the evidence summarized here does not establish that AI has closed breast cancer’s outcome gaps. ASSURE found no measured differences in selected screening-performance measures across the racial and density subgroups studied. It did not test whether AI changed who received screening, how quickly people completed diagnostic follow-up, whether they could access treatment, or whether mortality disparities narrowed.

AnTester 2 Test Colorectal Cancer Screening (FOB) Test Kit - Easy-to-Use at-Home Fecal Occult Blood Test for Early Detection - Convenient and Reliable Colorectal Cancer Screening Tool
  • Early Detection: Colon Cancer Screening Test Kit is designed to detect bowel cancer in its early stages through a simple stool sample (feces) test. The test is easy to perform and provides accurate results within minutes, allowing for quick detection and treatment.
  • Preferred Screening Method - Colon test kit for cancer is a FIT test, the preferred annual stool screening method for colon cancer according to the Colorectal Cancer Alliance and Cleveland Clinic.
  • No Preparation Required: Unlike other testing methods, FOB test Kit requires no special preparation or diet changes, making it a convenient and hassle-free option for at-home testing.
  • Fast and Easy-to-Use: Colon cancer Test at home and get results in just 10 minutes, with no mail-in lab testing required.
  • High Accuracy - The colorectal cancer screening test is over 99% accurate.

Those distinctions matter: finding more cancers is a screening outcome, while access to timely diagnosis and effective treatment—and ultimately mortality—are later outcomes. The cited deployment and implementation studies do not demonstrate that improvements at the image-reading stage automatically carry through the rest of care.

What the evidence supports—and what it does not

  • Supported: AI-assisted workflows have been associated with higher cancer detection in several defined screening settings, including US tomosynthesis and organized German screening.
  • Supported: AI scoring is being tested to select people for supplemental MRI, and AI pathology or data-integration research may contribute prognostic information.
  • Not established: That any single AI system reliably connects screening to a personalized, validated treatment plan.
  • Not established: That higher detection rates alone improve survival, eliminate disparities or prove that AI has closed access and treatment gaps.
  • Important to assess: The specific clinical step, population, workflow, recall trade-offs, evidence type, human review and authorized intended use for the system in question.
