AI hallucinations are plausible but false statements produced by a language model. A chatbot can present a wrong date, invented quotation, nonexistent citation or incorrect number in polished, confident prose because it generates likely sequences of words rather than independently verifying every claim. The practical rule is simple: treat fluency as presentation, not proof. For anything important, inspect the evidence behind the answer.
This guide explains what causes these errors, what published evaluations actually measure, and a repeatable way to check an answer before relying on it.

What an AI hallucination means
OpenAI defines a hallucination as “a plausible but false statement generated by a language model.” The term describes an output; it does not mean the software has human perception, beliefs or an intention to deceive. A false answer can concern an obscure fact, but it can also concern a familiar question.

Hallucination, mistake and uncertainty
In ordinary conversation, people often call any wrong answer a mistake. In AI discussions, hallucination emphasizes that the wording may be coherent and detailed despite being false. It is also useful to distinguish a wrong answer from an appropriate admission of uncertainty. A system that says it cannot establish a fact has not supplied a correct answer, but it has avoided turning a gap in knowledge into a fabricated assertion.
Why chatbots make things up
They generate likely text, not a truth-labeled statement
During pretraining, a language model learns statistical patterns from large collections of text and is optimized to predict likely next words. The objective teaches it which wording tends to follow other wording; it does not attach a verified truth label to every sentence in the training material. The model can therefore produce a fluent continuation without performing a source check for each factual proposition.
This explains why an answer can be grammatically excellent and still wrong. Language regularities are learnable even when the underlying fact is missing, disputed or represented inaccurately in the material used for training. The generation process is optimized for producing a plausible sequence, not for proving each statement.
Rare details are difficult to recover reliably
OpenAI uses a pet’s birthday as an analogy for the kind of detail that is hard to infer from surrounding patterns. A rare fact may have little repeated context from which to reconstruct it. If the model does not have dependable evidence for that detail, it may complete the pattern with a likely-sounding date, name or explanation instead of stopping.
That limitation applies to specific details in otherwise familiar subjects. A chatbot may know the general history of an event but still invent an exact quotation, page number or anniversary date. Familiarity with the topic does not guarantee that every requested detail is grounded.

Evaluation can reward guessing
Training and testing systems involve a scoring problem. If an evaluation awards points for an exact answer but gives no credit for a justified “I don’t know,” guessing can improve the score even though it also creates more wrong answers. OpenAI argues that confident errors should be penalized more heavily and that appropriate uncertainty should receive credit. That is an argument about better evaluation design, not proof that every hallucination has one single cause.
Later training can reduce some failures, but it does not remove the underlying limitation. A model can still be pushed toward answering when the evidence is weak, especially when a prompt demands a definite response.

What published numbers do—and do not—tell you
There is no universal hallucination rate for “chatbots.” Results depend on the model, questions, scoring rules and domain. OpenAI’s September 5, 2025 explainer reports this bounded example from its GPT-5 system-card SimpleQA evaluation:
| Model | Abstention | Accuracy | Errors | What the figures represent |
|---|---|---|---|---|
| gpt-5-thinking-mini | 52% | 22% | 26% | Organization-reported results on the cited SimpleQA example |
| o4-mini | 1% | 24% | 75% | Organization-reported results on the same named evaluation example |
These percentages are not a cross-vendor ranking, a forecast for every use of either model or a general rate for chatbot conversations. They illustrate why accuracy alone is incomplete: two systems can have similar accuracy while differing dramatically in how often they abstain and how many answers are wrong. Always attach a statistic to its model, benchmark and date.
Where hallucinations commonly appear
Names, dates and numbers
Check exact dates, statistics, versions, prices, measurements and names first. These details are easy to copy into a report and difficult for a reader to notice when they are only slightly wrong. Ask what document or record establishes the number, then verify it in that document.

Quotes and attributions
A chatbot may combine words associated with a public figure and present them as a quotation. Search for the complete sentence in an authoritative transcript, book, recording or institutional archive. If the wording cannot be located in a primary record, do not use quotation marks or attribute it as a direct quote.
Citations and links
References can be fabricated, misdated or attached to a source that does not support the stated conclusion. A working link is not enough: read the cited passage, check its publication date and confirm that the scope matches the chatbot’s claim. A source about a related topic cannot validate a more specific claim that it never makes.
Legal, medical and other high-consequence advice
An answer that sounds reasonable can still omit a jurisdiction, contraindication or recent change. For a consequential decision, use the chatbot to help formulate questions, then consult the responsible professional, authoritative record or current official guidance. Do not treat confidence as a safety signal.
How can I tell if ChatGPT made something up?
Use the same process for ChatGPT or any other chatbot. The interface and model name do not change the need to verify a material claim.
- Flag claims that matter. Mark every statement whose error would change a decision: names, dates, quotations, legal or medical assertions, calculations, statistics, product specifications and citations. You do not need to fact-check harmless conversational wording with the same effort.
- Ask for the basis of the claim. Request the exact source, passage, dataset or calculation used. Also ask the system to identify what it cannot establish. This can expose uncertainty, but the response remains a lead for checking rather than evidence by itself.
- Open each cited source. Confirm that the page, paper, law, dataset or record exists. Then read enough surrounding context to see whether it supports the precise sentence, not merely a nearby subject.
- Prefer direct, independent support. For a law, consult the responsible government publication; for a company policy, use that organization’s current documentation; for a scientific result, consult the paper or official study record. For an important claim, seek a second independent authoritative source rather than repeating the same chatbot output.
- Separate confidence from evidence. Specific wording, a long explanation and an assured tone do not demonstrate that the model verified anything. Record the evidence you found, not how persuasive the prose sounded.
- Resolve disagreements explicitly. If two authoritative records conflict, note the dates, jurisdictions or definitions that differ. Do not ask the chatbot to choose merely because it sounds certain.
A quick verification worksheet
- What exactly is being asserted?
- Which part is factual, and which part is interpretation or advice?
- What primary record could prove or disprove it?
- Does the cited source exist and contain the claimed information?
- Is the source current for the relevant country, edition, version or date?
- Would an error cause financial, legal, health or safety harm?
Can you trust a chatbot’s sources?
Trust the source only after checking it, not because a chatbot supplied it. A citation has at least three separate questions: does the source exist, is it authoritative for this subject, and does it support the exact proposition? A failure at any one of those stages makes the citation unsafe to repeat.

Look for scope clues that are easy to lose when a model summarizes: a statistic may apply to one sample; a rule may apply only in one jurisdiction; a technical result may concern one version; and a recommendation may depend on a date that has passed. Preserve those qualifications next to the claim in your notes or publication.
When a source cannot be located, rewrite the sentence as an unverified possibility or remove it. Do not fill the gap with a second chatbot asking whether the first chatbot was correct; that is not independent evidence.
Independent reader supportYour contribution helps us test, update, and keep practical guides available for everyone.Do AI detectors catch hallucinations?
Usually, they answer a different question. An AI-authorship detector estimates whether text resembles machine-generated writing. Hallucination checking asks whether a particular claim is true and supported. Human writing can contain factual errors, and an AI-generated sentence can be accurate, so authorship classification cannot substitute for verification.
| Approach | Primary task | Evidence examined | Main failure trade-off | How far the result generalizes |
|---|---|---|---|---|
| Fact verification | Determine whether a claim is supported | Primary documents, authoritative records and independent sources | Missed errors when evidence is incomplete; false confidence when a source is misread | Depends on the claim, source quality, date and jurisdiction |
| Repeated sampling or consistency checks | Look for unstable answers or disagreement | Multiple generated responses to the same prompt | Consistent wording can still be consistently wrong; variation does not identify the true answer | Depends on the model, prompt and sampling setup |
| AI-authorship discrimination | Estimate whether text was generated by a machine | Stylistic or statistical features of the text | False positives and missed machine text; no direct truth test | Varies by generator, discriminator, text type and dataset |
| Model-internal research methods | Study representations associated with truthfulness or errors | Internal model states and error patterns | Research findings may not transfer across skills or models | Not established as a consumer reading technique |
What the NIST pilot found
NIST’s 2024 GenAI pilot study evaluated text-to-text generation and discrimination using groups of human- and machine-generated article summaries. Its publication dated June 25, 2025 reports substantial variation: some generators could deceive most discriminators, while some discriminators detected generated content from almost all generators. Those findings describe performance on that task and set of systems. They do not turn a detector score into proof that a factual statement is true or false.
What model-internal research means
A 2025 Google Research paper reported that model states contain information related to truthfulness and error patterns, while challenging the idea of one universal truthfulness feature. Its abstract describes skill-specific generalization. This is evidence about model behavior and possible future detection research, not a method for reading truth directly from a chatbot’s prose.

How to use chatbots more safely
For low-stakes brainstorming
Use the output for ideas, outlines or alternative wording. Verify only the factual elements you plan to publish or act on. Label unverified suggestions in your notes so they are not mistaken for established facts later.
For research and reporting
Ask for a claim-by-claim list of sources, then retrieve the primary documents yourself. Keep quotations, numbers and dates tied to the passages that establish them. If the model cannot provide a source you can inspect, treat the statement as a lead, not a finding.
For high-consequence decisions
Use the chatbot to organize questions or explain terminology, never as the final authority. Check current official records and consult the qualified person responsible for the decision. Preserve the relevant version, date and jurisdiction because a generally correct explanation can still be wrong for your specific case.

Prompts that encourage appropriate uncertainty
OpenAI’s Model Spec guidance, quoted in its explainer, says: “It is better to indicate uncertainty or ask for clarification than provide confident information that may be incorrect.” You can request that behavior directly:
- “List the claims in your answer that you cannot verify.”
- “If the question is ambiguous, ask me a clarifying question before answering.”
- “For each number and quotation, provide the primary source and the exact passage.”
- “Separate established facts from inference and suggestions.”
- “If no reliable source is available, say so instead of guessing.”
These prompts can make uncertainty visible and reduce pressure to guess. They do not prove that the resulting answer is correct; the verification steps still apply.
Common checking mistakes
- Relying on tone: Confidence is a stylistic output, not a measurement of accuracy.
- Checking only whether a link opens: A real page can be irrelevant or fail to support the sentence attached to it.
- Using one generated answer to validate another: Agreement between outputs from the same system is not independent confirmation.
- Assuming a detector score is a fact-check: Machine-written text can be true, and human-written text can be false.
- Removing qualifications: A result tied to a particular benchmark, date or model cannot be presented as a universal rate.
- Expecting a perfect detector: NIST’s observed variation shows that detector performance depends on the systems and task being tested.
Bottom line
Chatbots make things up because their core language-generation objective produces likely text without verifying every claim, while some evaluation setups reward answering instead of abstaining. The reliable defense is not a special visual tell. Identify the claims that matter, inspect the cited evidence, prefer direct and independent records, preserve date and scope qualifications, and treat uncertainty as safer than polished invention.
