AI drug-response models predict how a biological system may react; cell-based screening measures what happens when a specific cell system is exposed to a compound under defined conditions. Neither output answers every drug-development question. Models can rank compounds, identify promising cell contexts and suggest follow-up experiments, while cell assays provide the observations needed to test those predictions.
The practical choice is therefore rarely “AI or screening.” A reliable program uses models to focus experimental work, then feeds well-designed experimental results back into model development. The limits of transfer, assay context and biological complexity determine how much confidence to place in either result.

What each approach actually answers
AI drug-response models produce predictions
An AI model learns relationships from previously collected drug-response measurements and other features. Depending on its design, those features can describe compounds, cell lines, molecular states or experimental conditions. The model then estimates a response for a drug and context that may be partly or entirely new to it.
That estimate is a prediction, not a measured response. It inherits the coverage, quality and biases of its training data. A model trained mostly on particular cancer cell lines, drug classes or assay protocols may perform well within those boundaries and fail when applied to a new drug, cell type, laboratory or measurement method. Its most defensible use is to prioritize compounds or experiments and to make testable hypotheses.

Cell-based screening produces an observation
In a traditional cell assay, researchers expose a defined biological system to a compound and record an outcome. The result belongs to the exact cell type or cell line, drug identity, dose, exposure time, culture conditions and assay used. A viability assay, for example, reports a signal produced by that assay at the tested conditions; it does not automatically describe every possible dose, tissue or patient.
Because the result is experimental, it can reveal effects that were absent from the model’s training data. It can also expose assay artifacts, unexpected toxicity or biology that a computational representation omitted. The trade-off is that each new condition requires laboratory work, reagents, quality control and interpretation.
Why prediction and measurement should be combined
AI is most useful when the number of possible drug–cell–condition combinations is too large to test immediately. A model can rank a library, identify cell contexts where a compound may be active and suggest which combinations deserve scarce experimental capacity. Screening then checks whether those priorities survive contact with biological measurements.

The loop should be explicit:
- Define the decision. Specify whether the goal is to find active compounds, distinguish sensitive from resistant cell contexts, estimate a dose response or select a follow-up experiment.
- Audit the training data. Record the drugs, cell systems, endpoints, doses, time points, laboratories and assay formats represented in the data.
- Generate predictions with uncertainty. Preserve confidence intervals, prediction distributions or other uncertainty estimates rather than ranking by a single number alone.
- Select experiments that test the boundary. Include high-ranked candidates, negative controls and cases where the model is uncertain or extrapolating.
- Run the cell assay with documented conditions. Keep the measured endpoint tied to the exact protocol and biological system.
- Compare prediction with observation. Evaluate calibration, ranking quality and errors on held-out experiments, not only on the data used during development.
- Refine the model. Feed validated measurements and informative failures back into the training set, then repeat independent testing.
A 2026 review of AI-driven virtual-cell models states that virtual-cell use should be closely integrated with biological experiments. In practice, that means a plausible prediction is evidence for what to test, not permission to treat the prediction as a laboratory result.
What the cross-study evidence says about transfer
Xia and colleagues’ 2021 cross-study analysis examined drug-response prediction across the NCI60, Cancer Therapeutics Response Portal (CTRP), Genomics of Drug Sensitivity in Cancer (GDSC), Cancer Cell Line Encyclopedia (CCLE) and Genentech Cell-line Screening Initiative (gCSI) datasets. The work is especially relevant because it tests whether relationships learned in one biological dataset transfer to another.
They caution that cross-validation inside one dataset can overestimate performance on independent data. A random split may place very similar drugs, cell lines or assay conditions in both training and test sets. That can make a model appear reliable even though it has not faced a genuinely new laboratory context.
The study also examined agreement between experimental datasets after linear mapping of response measurements. Two reported values are useful, but they are not AI accuracy scores:

| Mapped comparison | Reported R² | What the number represents | Important qualification |
|---|---|---|---|
| CTRP to CCLE | 0.68 | Agreement between response measurements after linear mapping | Both datasets used CellTiter-Glo in the study’s dataset table |
| CTRP to GDSC | 0.41 | Agreement between response measurements after linear mapping | GDSC used Syto60 while CTRP used CellTiter-Glo in the study’s dataset table |
The lower CTRP-to-GDSC agreement illustrates why assay context matters. The R² values describe correspondence between experimental datasets under that analysis; they do not state that an AI model is 68% or 41% accurate. Differences in viability assays, protocols and data processing can change the apparent relationship even when the underlying biology is related.

The same analysis used simulations based on CTRP to study training-data composition. In that setting, increasing drug diversity contributed more to generalizability than increasing cell-line diversity. This is a result of that simulation and dataset design, not a universal rule for every therapeutic area or model architecture.
How the approaches compare by decision criterion
| Criterion | AI drug-response model | Cell-based screening |
|---|---|---|
| Primary output | A predicted response, ranking or probability for a specified context | An observed response from the tested cell system, compound, dose, time and assay |
| Conditions represented | Only conditions and features covered well enough by the training data, plus whatever extrapolation the model attempts | The exact biological and assay conditions performed in the experiment |
| Untested conditions | Can prioritize untested drugs or contexts, but confidence declines with distribution shift | Cannot establish an untested condition without running an experiment |
| Throughput | Can score many candidate combinations computationally once data and infrastructure are available | Limited by plates, reagents, cell growth, controls, readouts and laboratory capacity |
| Transfer to new drugs or cell types | Requires evidence that the new inputs resemble the training distribution or a validated adaptation strategy | Directly measures the new drug or cell system, although assay quality and biological relevance still require evaluation |
| Transfer between laboratories or assay formats | Can be disrupted by protocol and measurement differences present in the data | Can be disrupted by protocol, reagent, instrument and operator differences; controls are essential |
| Mechanistic value | May reveal predictive features or associations, but a strong prediction is not automatically a mechanism | Shows a phenotype under the tested conditions; additional experiments are needed to establish mechanism |
| Uncertainty | Should be reported from model uncertainty, data coverage and applicability-domain checks | Comes from biological variability, technical error, controls and assay reproducibility |
| Complex tissue behavior | Requires training data and representations that capture multicellular interactions; many models do not | Simple monolayers also omit tissue behavior, while more complex systems add reliability and scalability challenges |
There is no evidence here for a universal winner across therapeutic areas, drug classes or assay formats. The right balance depends on whether the immediate question is computational prioritization, direct intracellular response, or behavior that requires a multicellular environment.
Throughput is not the same as biological coverage
Once trained, a model can evaluate combinations that would be expensive or slow to test one by one. That makes it valuable for triage: a team can reserve laboratory capacity for a smaller set of candidates, deliberately include uncertain cases and explore combinations that would otherwise be overlooked.
High computational throughput does not guarantee broad biological coverage. If a model has seen few examples of a chemical scaffold, a rare cell state or a different assay endpoint, scoring thousands of such cases may simply produce thousands of extrapolations. A large ranked list should therefore be accompanied by an applicability statement: which drugs, cell systems and protocols are similar to the training data, and which are not.
Cell screening has the opposite profile. It is slower to scale, but each result can add information about a condition the model did not previously observe. Carefully designed plates can measure dose, time course, controls and replicate variability at once. The resulting data are useful for retraining only when metadata and quality checks are preserved.
Interpreting assay context and dataset differences
A response value is not independent of its measurement method. In the Xia et al. analysis, CTRP and CCLE used CellTiter-Glo, whereas GDSC used Syto60 in the reported dataset table. Those assays measure related viability phenomena through different signals and protocols. Linear mapping can improve comparability, but it cannot erase every biological and technical difference.
When combining datasets, document at least:
- the assay name and readout principle;
- cell identity, passage or culture information when available;
- compound identity, formulation and concentration range;
- exposure duration and time of measurement;
- replicate structure, controls and quality exclusions;
- laboratory, instrument and batch metadata; and
- the response transformation and normalization used for modeling.
Without that context, a model may learn laboratory signatures rather than drug biology. A performance number from a familiar assay can also look impressive while failing on a different readout.

Start with a prospective test set
Hold out compounds, cell systems or laboratories before model fitting when the goal is to estimate transfer. A random split within one dataset can answer an in-distribution question, but it is insufficient for a claim about new drugs or independent experiments. Where possible, design the test around the deployment scenario: new compounds, new cell lines, a new site or a new assay.

Match the assay to the prediction
If the model predicts viability at a particular time and dose, the validation assay must measure that endpoint at the same time and dose. A prediction of one response definition should not be declared validated by an unrelated endpoint merely because both are described as “drug response.”
Use controls and replication
Include untreated controls, known active or inactive controls where appropriate, plate-level quality checks and biological or technical replicates. Record failures rather than silently removing inconvenient results. Replication distinguishes a model error from a noisy measurement.
Assess more than correlation
Correlation or R² can miss systematic bias and poor ranking at the decision threshold. Examine calibration, sensitivity to dose, false-positive and false-negative rates, rank enrichment and performance on the specific subgroup that matters for deployment. Report confidence intervals and the number of independently tested conditions.
Update cautiously
New experimental data can improve a model, but repeatedly tuning against the same validation set turns that set into training data. Keep a final untouched evaluation set or conduct a later prospective test after the model and selection rules are frozen.
Where cell assays remain indispensable
Cell experiments are needed whenever the central question is what a living biological system actually does under a defined exposure. They can reveal unanticipated phenotypes, toxicity, adaptation and assay-specific effects that were not encoded in historical training data.
They are also the reference point for validating computational hypotheses. A model can propose that a compound is selective for one cell context, that a combination may be synergistic or that a dose range deserves attention. Only an experiment can establish whether that prediction is observed under the chosen protocol.
However, a cell-based result is not automatically a patient outcome. Two-dimensional cancer cell-line assays usually omit immune, stromal, vascular and pharmacokinetic factors. A positive result can justify further work without proving clinical efficacy.

The 2026 virtual-cell review identifies tissue morphology and complex multicellular behavior as areas where experimental approaches or tissue-level models remain necessary. A model trained on isolated-cell measurements should not be described as predicting a tumor, organ or patient unless it has been validated for that level of biology.
More complex experimental systems are not automatic solutions. A 2026 review of human pluripotent-stem-cell-derived organoid screening reports that reliability and scalability remain under evaluation, with robustness, throughput and protocol standardization still challenging. Organoids can add architecture and cell interactions, but their value depends on reproducible protocols, appropriate controls and evidence that the chosen model represents the question being asked.

A separate 2026 review on organoids in drug development reports limited training and validation datasets, use of conventional two-dimensional or animal-derived data in some machine-learning models, and a lack of transparent benchmarks and standardized metrics. These are review-reported limitations, and they mean that an organoid model may have less established transfer evidence than a model built on larger, more standardized cell-line collections.

A practical decision framework for research teams
Use an AI model first when prioritization is the bottleneck
- The candidate library or combination space is too large for an initial screen.
- Relevant historical measurements and metadata are available.
- You can define an independent experimental test and preserve uncertainty estimates.
- The deployment setting resembles the training data closely enough for a meaningful applicability check.
Use cell screening first when direct evidence is missing
- The drug, cell type, assay or dose range is poorly represented in available data.
- The endpoint is sensitive to protocol details that the model does not capture.
- You need to discover an unexpected phenotype rather than rank known patterns.
- The decision depends on reproducibility under a specific laboratory protocol.
Use both when the cost of a wrong priority is high
For consequential decisions, use the model to choose a diverse, uncertainty-aware experimental set rather than only its top-ranked predictions. Let the assay test both expected successes and likely failure modes. This design produces evidence about model reliability while generating biological results.
Common failure modes and how to correct them
Calling a prediction a measured response
Problem: A report describes a model score as though cells were exposed to the compound.
Correction: Label the output as predicted and state the input data, endpoint and context. Reserve “measured” or “observed” for results generated by the assay.

Best Value
Correction: Add independent splits by drug, cell system, laboratory or dataset that reflect the intended deployment.
Ignoring assay incompatibility
Problem: Measurements from different readouts are pooled as if they were interchangeable.
Correction: Preserve assay metadata, assess cross-study agreement and model protocol effects explicitly. The CTRP-to-CCLE and CTRP-to-GDSC R² values show why one mapping cannot be generalized to every pair of studies.

Assuming more data automatically solve transfer
Problem: A larger dataset adds many near-duplicate conditions but little diversity in drugs, biology or protocols.
Correction: Evaluate coverage and diversity, not just record count. The CTRP simulation found drug diversity more influential than cell-line diversity in that setting, but teams should test the relevant diversity for their own use case.
Equating cell-line activity with patient benefit
Problem: An in-vitro response is presented as clinical efficacy.
Correction: Treat it as evidence at the tested biological level and require additional models, pharmacology and clinical studies for claims about patients.
Assuming organoids settle the complexity problem
Problem: A three-dimensional or stem-cell-derived system is treated as automatically predictive.
Correction: Check robustness, throughput, protocol standardization, benchmark transparency and whether the organoid captures the tissue feature relevant to the decision.
Bottom line
AI drug-response models and traditional cell-based screening answer related but different questions. Models extend existing measurements into predictions that can make experimental programs more selective; cell assays establish observations under defined biological and technical conditions. Transfer can fail when drugs, cell systems, laboratories or readouts change, and the published cross-study R² values demonstrate measurement disagreement rather than universal AI accuracy. The strongest workflow keeps the distinction intact: use models to prioritize and formulate hypotheses, validate them with independent experiments, and treat each result as evidence only at the biological level it actually tested.
