AI Can Outperform Doctors on Some Diagnostic Tests. That’s Not the Scary Part.

AI has beaten physicians on some controlled diagnostic tests, but that does not establish that it is generally smarter than your doctor. A 2025 review found no overall advantage over physicians and lower performance than expert physicians. The bigger concern is what happens when a convincing-sounding AI answer is wrong—and nobody checks it, explains its limits, or takes responsibility for using it.

Is AI really smarter than your doctor?

There is no single test of “smarter.” Getting a diagnosis right in a written case, asking useful questions in a consultation, recommending safe treatment, communicating clearly, and improving patient outcomes are different measures. A system can do well on one without proving it can provide better care overall.

Two studies illustrate why the answer depends on the task and setting:

Evidence What was compared What it found What it does not establish
Tsujimoto et al., systematic review and meta-analysis, npj Digital Medicine, 2025 83 studies validating generative AI for diagnosis, published from June 2018 through June 2024 Pooled diagnostic accuracy was 52.1%. Adjusted comparisons found no statistically significant difference from physicians overall or non-expert physicians; AI performed significantly worse than expert physicians. A universal score for current chatbots or the chance that a chatbot will correctly diagnose an individual patient.
AMIE study, Nature, 2025 A randomized, double-blind crossover evaluation of text consultations with 20 primary care physicians, using 159 case scenarios from providers in Canada, the United Kingdom, and India AMIE had greater diagnostic accuracy in this evaluation. Specialist physicians rated it superior on 30 of 32 evaluated axes, and patient-actors on 25 of 26; it was non-inferior on the remaining axes in each assessment. That AI provides equivalent or better care in routine clinics, or improves patient outcomes.

The review’s pooled figure combines studies with different models, tasks, specialties, test data, and publication status. The authors also note that complete model training data are not disclosed. Its 52.1% figure is therefore a summary of the studies reviewed, not a report card for every AI system available today.

Why a strong test result is not the same as better care

Benchmarks isolate a task

A diagnostic test may ask a system to select a likely answer from a case description. A real appointment can require clarifying an incomplete history, weighing competing explanations, deciding what to investigate, making a safe plan, and communicating uncertainty. Performance on a structured diagnostic question does not by itself measure all those parts of care.

The AMIE result came from simulated text consultations

In the Nature study, validated patient-actors interacted with AMIE or physicians through synchronous text chat, and specialist physicians and patient-actors evaluated the consultations. That is a meaningful controlled comparison, but the authors said the format is unfamiliar in clinical practice and that further research is needed before translating the result to real-world care.

A correct answer does not prove sound reasoning

A 2024 NIH release describes a small image-based quiz study in which an AI model often selected the correct diagnosis, while physician evaluators also found mistakes in its image descriptions and explanations—including cases where its final answer was correct. This is a limited example, not evidence about every multimodal system. It does show why a correct conclusion alone may not reveal whether the system interpreted the evidence reliably.

Where AI is already used in healthcare

An American Medical Association survey in 2026 included nearly 1,700 physicians across specialties, practice settings, and career stages. It found that 81% reported using AI professionally. The reported uses included:

Reported use Physicians reporting it
Summaries of research and standards of care 39%
Discharge instructions, care plans, or progress notes 30%
Billing codes, charts, or visit notes 28%
Chart summaries 28%
Draft replies to patient portal messages 19%
Translation 18%
Assistive diagnosis 17%

These are survey-reported uses, not proof that AI improved care or made independent diagnoses. The figures describe what physicians said they used; they do not show how often a tool was checked or what happened to patients afterward.

The same AMA survey found physicians generally more comfortable with patients using AI for general health and medication questions than for tasks requiring clinical judgment. Nearly half strongly opposed patient use for interpreting radiology or pathology. These are reported professional concerns, not a universal judgment about every tool or situation.

What is the real safety concern?

AI can sound certain even when its answer is incomplete or wrong. The Agency for Healthcare Research and Quality’s 2025 issue brief identifies risks including bias, opaque reasoning, hallucinations, automation bias, and complacency. It notes that AI can appear at multiple points in the diagnostic process, from triage and symptom checking to interpretation of clinical data and follow-up.

Automation bias is the tendency to give an automated recommendation too much weight. If a clinician or patient accepts a plausible answer without checking the evidence, an error can pass through the care process. If the system’s limits are unclear, the patient may also have difficulty knowing who reviewed the output or who is accountable for acting on it.

In June 2026, the AMA announced policies that place AI in an assistive role rather than treating it as an autonomous decision-maker. The policies call for transparency, accountability, physician oversight, evidence attribution, validation, and explainability. This is a professional association’s policy position, not itself a binding regulation. AMA CEO John Whyte, MD, MPH, put the principle plainly: “AI has enormous potential in healthcare, but it cannot replace physician judgment.”

How to ask about AI in your care

If an AI tool is mentioned—or its role is unclear—you can ask practical questions without assuming that its use is automatically unsafe or beneficial:

  • Was AI used in my care, and for what specific task?
  • Did a qualified clinician review its output before it affected a decision?
  • What information or evidence informed the recommendation?
  • What are the important uncertainties, and what would change the plan?
  • Who can explain the decision and address it if the AI output was wrong?

A general-purpose chatbot cannot examine you, verify your full medical record, or take responsibility for your care. Treat its health suggestions as information to discuss with a qualified clinician—not as a substitute for professional evaluation, especially when symptoms are urgent or worsening.
