A secure AI agent is constrained before it acts, checked at the tool boundary, and observable afterward. Give it only the files, tools, identities, and network destinations required for its task; run code inside an isolated environment; keep valuable credentials outside that environment; and require a human decision before high-impact side effects.
These controls solve different problems. Least-privilege permissions reduce what the agent can attempt, sandboxing limits where its code can run, credential isolation limits which secrets it can use, and approval workflows stop risky actions. None is sufficient alone, and model behavior is not a security boundary.

What AI-agent security has to control
An AI agent combines a language model with tools, files, credentials, network access and the ability to carry out multi-step plans. That combination creates a larger attack surface than a chatbot that only returns text. A malicious prompt, a poisoned document or a mistaken plan can cause the agent to call tools in an unsafe sequence.
What’s actually slowing this PC down?
Pick the symptom - the matching free tool is one click away.
Security therefore has to govern the entire action path: what the agent is allowed to see, what it can call, which identity performs the call, where generated code executes, whether a person must approve the operation, and what evidence remains afterward.

Model behavior is not an access-control system
Anthropic’s security guidance says model behavior is inherently not a security control. A model may follow a malicious instruction, misunderstand a boundary or change its behavior after seeing adversarial content. Enforce limits with credentials, network policy, sandboxing and approvals at the infrastructure and execution layers instead of relying on a system prompt to “be careful.”
How the layers fit together
OpenAI describes the relationship succinctly: “Approvals and sandboxing work together.” The sandbox defines where the agent can operate; the approval policy determines when it must ask before crossing a boundary. Permissions determine which tools and resources are available in the first place, while logging makes each decision reconstructable.
Threats a design should address
Start with concrete failure modes rather than a generic “secure the agent” requirement.
- Prompt or goal hijacking: untrusted text in a web page, file, email or ticket attempts to replace the user’s objective.
- Excessive agency: the agent has write, administrative or financial capabilities that the task does not require.
- Tool misuse: a legitimate tool is called with an unsafe target, argument or sequence.
- Identity and privilege abuse: a shared or production identity lets a low-trust task perform high-trust operations.
- Credential exposure: a token, environment variable or configuration file becomes readable to generated code or an attacker.
- Data exfiltration: sensitive content leaves through an unapproved host, tool or output channel.
- Memory poisoning: durable agent memory stores false instructions or malicious data that influences later runs.
- Supply-chain compromise: an extension, package, connector or remote tool introduces unsafe code or permissions.
- Approval manipulation: a request hides the real target, changes parameters after approval or pressures a reviewer into rubber-stamping it.
- Cascading failure: one agent’s output triggers another agent or automation, multiplying an initial mistake.
OWASP specifically warns against wildcard permissions and arbitrary code execution without sandboxing. Treat every tool and extension as a potential path to these outcomes.
Design permissions with least privilege
OWASP recommends least privilege, per-tool scoping, separate tool sets for different trust levels and explicit authorization for sensitive operations. Build the policy from a default-deny position: every file, command, host, identity and resource is unavailable until the task demonstrates a need for it.

1. Define the task boundary
Write down the requested outcome and the resources needed to achieve it. A report-writing agent might need read access to a project folder and a document-generation tool, but not shell access, customer records or production credentials. A deployment assistant may need a staging API, while production release remains a separately approved operation.
2. Scope each tool separately
Do not treat “the agent” as one permission. Give each tool its own policy covering allowed operations, resources, argument formats and calling identities. A database tool can expose parameterized queries against a named reporting schema without exposing arbitrary SQL or administrative commands. A file tool can allow reads in one directory and writes only to a newly created output directory.
3. Separate read and write identities
Use distinct identities for read-only activity and changes. Keep production, root or organization-wide administrative identities out of the agent environment. OWASP recommends executing extensions in the user’s context with minimum privileges; that prevents a connector from silently borrowing a more powerful service account than the user is authorized to use.
4. Constrain resources and arguments
Allowlist exact repositories, buckets, tables, API routes and hostnames. Normalize paths and URLs before matching them so equivalent spellings cannot bypass a rule. Reject wildcard paths, unrestricted command interpreters and arguments that enable a second interpreter. If a task needs a bounded operation, expose a purpose-built function rather than a general shell.
Permission checklist
- Start from deny and allow only required files, tools, commands, hosts and resources.
- Use separate tool sets for low-trust and high-trust work.
- Distinguish read, create, modify and delete operations.
- Keep administrative and production identities outside the agent environment.
- Authorize actions in the user’s context where possible, rather than granting a broader service identity.
- Review permissions when the task, connector or data classification changes.
What sandboxing protects—and what it does not
Sandboxing is a technical execution boundary. OpenAI’s sandbox documentation notes: “Agent-generated code can access the files, credentials, and network available to its environment.” The practical implication is to provide an isolated compute environment with only the filesystem, processes, network routes and identity needed for that run.

Bound the execution environment
- Filesystem: mount a task-specific workspace; make source data read-only when possible; keep host files, SSH keys and unrelated projects inaccessible.
- Process: remove unnecessary interpreters, device access and privileged capabilities; prevent escape to the host.
- Network: deny outbound traffic by default and allow only approved endpoints, methods and ports. Log every request.
- Project: isolate users, tenants and workloads so one run cannot inspect another run’s artifacts.
- Identity: use a short-lived, narrowly scoped runtime identity rather than a developer’s or administrator’s credentials.
Use independent boundaries. A filesystem rule should not be the only protection if network access can copy the same data elsewhere, and a network rule should not compensate for an overprivileged identity.
Why a sandbox is not authorization
A sandbox limits blast radius; it does not decide whether an action is legitimate. If a credential or powerful tool is exposed inside the sandbox, the process can misuse it while remaining perfectly contained. OpenAI’s guidance warns that injecting a stored secret into the environment still exposes that secret to agent-generated code. Keep application keys outside the sandbox and broker approved operations through a vault or trusted proxy.
Keep credentials out of the agent
Long-lived keys should reside in a vault or secrets manager, not in prompts, source files, images, environment dumps or the sandbox filesystem. Prefer short-lived, scoped tokens that can be revoked and rotated independently of the agent process.
Use a broker or proxy for sensitive operations
A trusted service can receive a narrowly defined request, authenticate the user and agent, enforce policy, call the upstream system and return only the required result. The agent never receives the upstream secret. The proxy should validate resource, operation and parameters, apply rate limits, record the decision and reject requests outside the approved scope.
Reduce exposure during normal operation
- Issue credentials just before use and expire them quickly.
- Scope tokens to one tenant, project, API route or object where the provider supports it.
- Keep separate credentials for staging and production.
- Rotate and revoke tokens after incidents or policy changes.
- Redact secrets from logs, traces, error messages and model-visible tool results.
- Never echo a secret back to the model merely to prove that authentication succeeded.
When an agent should ask for human approval
Use automatic guardrails for routine validation and human review for approval decisions. OpenAI’s guidance is explicit: “Use guardrails for automatic checks and human review for approval decisions.” A guardrail can reject malformed arguments or a disallowed host; it should not silently decide that a deletion, payment or public post is acceptable when a person is required to make that judgment.

Require approval for high-impact side effects
Pause before cancellations, edits to shared or production data, shell commands with destructive potential, financial transactions, administrative changes, externally visible communications, irreversible operations, sensitive MCP actions and anything ambiguous or outside the stated engagement. OWASP’s risk guidance likewise calls for human-in-the-loop control before high-impact actions.
Choose an approval mode by risk
| Action class | Typical policy | Examples |
|---|---|---|
| Read-only, reversible and in scope | Automatic execution after input and tool validation | Search an approved repository; generate a draft in a private workspace |
| Write operation with limited impact | Risk-based escalation or approval on first use | Create a ticket; update a non-production record |
| Destructive, external or financially consequential | Always ask a human before execution | Delete data; cancel a service; send a public message; initiate a payment |
| Administrative or production change | Step-up authentication plus explicit approval | Change access policy; deploy to production; rotate a critical secret |
| Ambiguous or out of scope | Deny or pause until the user clarifies | Unrecognized host, missing target, conflicting instructions |
Bind approval to the exact action
An approval is not a general “yes, continue.” OWASP’s high-impact integrity controls require binding it to the actor, tool, target resource, normalized parameters, timestamp and expiry. Show the reviewer the proposed action in a canonical form, including the identity that will execute it and the data it will affect.
Use short-lived authorization artifacts, replay protection and step-up authentication for critical operations. Invalidate an approval if any material parameter changes. Where possible, make the operation idempotent so a retry cannot duplicate a charge, deletion or external message.
Always-ask versus automatic server evaluation
Anthropic’s managed-agent documentation describes policies that can allow a tool call, wait for approval or let the server evaluate it. Automatic evaluation is not a human checkpoint. If a real person must authorize the operation, configure an always-ask policy for that tool and make the approval interface show the exact target and arguments.
A practical implementation sequence
- Classify the task and data. Identify users, tenants, sensitive records, external recipients and irreversible outcomes.
- Inventory tools and identities. Record each tool’s operations, resources, credentials, network destinations and failure behavior.
- Create a deny-by-default policy. Add only the minimum files, commands, hosts and resources needed for the defined task.
- Build the isolated runtime. Separate filesystem, process, network, project and identity boundaries; provide a disposable workspace.
- Move secrets behind a vault or proxy. Issue short-lived, scoped access and keep application keys outside the runtime.
- Add tool-boundary validation. Normalize paths and parameters, check the caller’s identity, enforce allowlists and reject unexpected arguments before execution.
- Set approval thresholds. Mark destructive, financial, administrative, external, irreversible, ambiguous and out-of-scope actions as approval-required.
- Make approvals specific and expiring. Bind them to actor, tool, target, normalized parameters, time and expiry; prevent replay.
- Instrument every decision. Capture request context, effective permissions, proposed action, approval result, execution result and failure state.
- Test denial and recovery paths. Verify that policy outages, approval timeouts and logging failures stop execution safely, and test rollback or idempotent retry behavior.
Guardrails, observability and fail-closed behavior
Guardrails can validate inputs, outputs and tool calls. Examples include schema checks, data-loss filters, destination allowlists, command restrictions, rate limits and checks that a tool result contains no credential material. They are automatic controls, not substitutes for a reviewer when the consequence is high.
Best Value
Maintain an audit trail that links the original prompt or request context to the permissions granted, tool calls proposed, approval decision, execution identity, outputs and errors. Store enough normalized parameters to reconstruct what happened without retaining unnecessary sensitive data. Independent logs are valuable because an agent that can modify its own workspace should not be able to rewrite the authoritative record of its actions.
Fail closed when policy lookup, approval validation, identity verification or audit logging is unavailable. A timeout should produce a denial or a paused run, not an automatic allow. If an upstream system returns an ambiguous result, stop before retrying a non-idempotent action and require reconciliation.
How to compare agent-security implementations
When evaluating a platform, framework or internal design, compare capabilities rather than marketing labels.

Testing and operating the controls
Test adversarial inputs
Place hostile instructions in documents, web pages, issue descriptions and tool results. Confirm that the agent keeps the user’s objective, cannot expand its permissions and cannot send extracted data to an unapproved endpoint. Test path traversal, alternate URL forms, encoded arguments and attempts to invoke a second interpreter.
Test approval integrity
Change one parameter after approval and verify that the authorization is rejected. Replay an expired approval, alter the target identity and submit a duplicate request. Each case should fail without executing the side effect.
Test containment and recovery
Attempt to read host files, credentials from unrelated projects, device interfaces and blocked network destinations. Simulate a vault outage, policy-service timeout and logging failure. Confirm that the run pauses or denies, that operators can identify the last safe state and that retries cannot duplicate an irreversible action.
Review permissions continuously
Access needs change as tools, models, data and workflows evolve. Revoke unused connectors, rotate credentials after personnel or vendor changes, review high-impact approvals and investigate unusual destinations or volumes. Treat every new extension as a new trust boundary rather than inheriting the agent’s existing access automatically.

Secure agent design is a layered control system: least privilege limits what can be attempted, sandboxing limits where code can operate, credential isolation limits what secrets can be used, human approval stops high-impact side effects, and independent logging plus fail-closed behavior makes the result auditable. If any layer is missing, a prompt mistake or compromised tool can turn ordinary automation into an incident.
