Neither adult diapers (absorbent briefs) nor incontinence pads are best for everyone overnight. A well-fitting brief is often the more suitable starting point for someone with moderate-to-heavy leakage, limited mobility, or combined urinary and fecal incontinence. An overnight pad held snugly in close-fitting underwear may work for an ambulatory wearer who prefers pads and can keep one in place. The right choice depends on leakage, mobility, fit, skin health, and personal preference—not the product label alone.
What specialist guidance recommends
The Wound, Ostomy and Continence Nurses Society (WOCN) issued recommendations based on user circumstances, rather than declaring briefs or pads universally superior. Its 2018 consensus recommends nighttime disposable briefs for nonambulatory women with moderate-to-heavy urinary, fecal, or dual incontinence, and for men with moderate-to-heavy nighttime urinary, fecal, or dual incontinence. For ambulatory women in the moderate-to-heavy nighttime group, it considers a disposable pad worn with close-fitting underwear an acceptable alternative. Read the WOCN consensus statement.

These are recommendations for specified groups, not a head-to-head trial showing that one category prevents leaks better for every person. A product that suits one wearer may shift, feel uncomfortable, or fail to contain another wearer’s leakage.
How to choose for overnight use
Start with leakage type and amount
Consider whether the leakage is urinary, fecal, or both, and how much typically occurs overnight. WOCN guidance supports briefs for several moderate-to-heavy nighttime groups, including people with fecal or dual incontinence. A pad may be appropriate for an ambulatory wearer when it can contain the leakage and stay correctly positioned. No universal absorbency number or product category guarantees an uninterrupted night.

Consider mobility and who applies the product
A brief may be more practical when the wearer has difficulty standing, pulling underwear up and down, or positioning a pad, or when a caregiver assists with changes. Tabbed briefs can be applied without pulling them up over the legs. For someone who can position a pad and keep it held close to the body, a pad may be easier or more comfortable. Individual ability and preference matter.
Check fit and containment
For a brief, use the size guidance for waist and hip measurements, and check that the absorbent area, leg gathers, and elastics sit correctly without being constrictive. For a pad, check that its length and coverage suit the wearer and that close-fitting underwear holds it securely. A pad that shifts, or underwear that lets it sit loosely, can create gaps and leaks. If leakage continues, reassess fit and product type rather than assuming a higher marketing absorbency claim will solve the problem.

Account for skin health and comfort
Either product type needs to be comfortable and compatible with the wearer’s skin. Use gentle cleansing and suitable skin protection. WOCN cautions that some ointments can interfere with absorbency, so more barrier cream is not automatically better. If irritation persists, seek advice from a healthcare professional or continence clinician.

For high-volume urine output, WOCN says to consider a booster pad as an addition to an absorbent brief or pull-up. It is not a replacement for the primary worn product. A washable bed pad can protect the mattress by catching leaks, but it also does not replace a brief or incontinence pad. The NHS overview of incontinence products and leak management describes absorbent pads worn inside underwear and washable bed pads as separate options.

When to change overnight
There is no one change interval that suits everyone, and the guidance does not say to leave a product in place all night regardless of saturation or soiling. WOCN advises that change timing should be patient-centered, taking account of skin health, odor, sleep, elimination patterns, dignity, and the product’s properties—not simply routine or caregiver convenience. Follow the product instructions and the wearer’s care plan, and adjust if the product is soiled, saturated, uncomfortable, or affecting skin.

Persistent leakage, recurring skin irritation, or difficulty finding a product that fits are reasons to ask a healthcare professional or continence service for help. The NHS notes that in the UK, access to supplied products may depend on the local integrated care board and professional assessment; arrangements vary by location.
