Added Sugar vs. Natural Sugar: What to Limit for Gut Health

For practical dietary guidance, limit added and free sugars—especially in sweetened drinks and processed foods—rather than treating sugar in whole fruit or plain milk as equivalent. But current guidance does not establish a particular sugar limit as a proven way to improve gut microbiome health.

What counts as added or free sugar?

WHO uses the term free sugars: sugars added to foods or drinks by manufacturers, cooks or consumers, plus sugars naturally present in honey, syrups, fruit juice and fruit-juice concentrates. The category is about where sugar comes from, not a claim that sugar molecules from different sources are chemically different. WHO’s definition excludes sugars naturally present in whole fresh fruits and vegetables and in milk. WHO’s sugar guideline sets out this definition and its recommendation.

That distinction means “natural” is not a reliable shortcut for deciding what counts. Honey and syrup may be natural, but their sugars are free sugars under WHO guidance. Fruit juice can also contain significant free sugars even when no sugar has been added. Whole fruit is treated differently in this guidance because its sugars are intrinsic to the intact fruit.

What sugar limit should you use?

WHO recommends that adults and children keep free sugars below 10% of total daily energy. It conditionally suggests reducing intake below 5% for additional benefits. These are general dietary benchmarks, not gut-health thresholds. WHO’s guideline focuses especially on unhealthy weight gain and dental caries, rather than a quantified effect on the gut microbiome.

WHO’s 2026 healthy-diet fact sheet illustrates the 10% benchmark as 50 g, or about 12 level teaspoons, for someone eating roughly 2,000 calories a day. That is an example tied to that calorie intake, not a universal gram limit. The 5% level is a conditional suggestion, not a gut-specific prescription. WHO’s healthy-diet fact sheet also recommends a broader food pattern centered on fibre-rich foods.

U.S. guidance uses “added sugars”

For the United States, the Dietary Guidelines for Americans 2020–2025 advise people age 2 and older to get less than 10% of daily calories from added sugars, and advise avoiding foods and drinks with added sugars for children younger than 2. This is a U.S. benchmark and uses the term “added sugars”; it should not be treated as an identical legal or labeling definition worldwide. The 2020–2025 U.S. Dietary Guidelines are the edition cited here.

How to compare foods and labels

In the U.S., most packaged foods list added sugars separately on the Nutrition Facts label. Check the serving size first, then compare grams of added sugars and the percent Daily Value. The ingredient list can identify sweeteners, but it does not tell you how many grams are in a serving. NIDDK’s label guidance explains how to read the Nutrition Facts panel.

  • Whole fruit: WHO’s free-sugar recommendation excludes sugars intrinsic to whole fresh fruit.
  • Juice: Fruit juice, including many unsweetened varieties, can contain significant free sugars.
  • Honey and syrups: Their sugars count as free sugars in WHO guidance, even when marketed as natural.
  • Plain milk: Naturally occurring milk sugars are excluded from WHO’s free-sugar definition.
  • Packaged foods: In the U.S., use serving size and added-sugars grams together; the label’s definition and format may differ by country.

Why fibre and food pattern matter too

WHO’s carbohydrate guidance considers carbohydrate quality through food sources, fibre content and how digestible carbohydrates release glucose. WHO recommends that carbohydrates come primarily from whole grains, vegetables, fruits and pulses. Its 2026 fact sheet recommends at least 400 g of fruits and vegetables daily for people older than 10, and at least 25 g per day of naturally occurring dietary fibre for that age group. These are food-pattern recommendations; they do not mean fibre cancels out sugar or that a particular food repairs the microbiome. See WHO’s carbohydrate guideline for its broader framing.

What is—and is not—established about gut health

The official guidance cited here does not establish a quantitative causal effect of added versus naturally occurring sugar on gut microbiome health. It therefore cannot support claims that added sugar “feeds bad bacteria,” that natural sugar “heals the gut,” or that meeting a particular sugar threshold improves microbiome outcomes. This evidence boundary is not proof that no relevant studies exist; it means these recommendations should not be presented as gut-specific evidence.

Use free- or added-sugar guidance for its stated general dietary purposes, and distinguish whole fruit and plain milk from juice, honey and syrups rather than grouping every source under “sugar.” For a gut-health food pattern, WHO’s recommendations emphasize varied carbohydrate sources and naturally occurring fibre, not a promise that changing sugar alone will treat symptoms.
