Secure Active Directory with three coordinated controls: patch domain controllers and every system that can influence them, continuously monitor directory and privileged activity, and separate everyday identities from tightly controlled administrative access. Use the AD tier model to define trust boundaries, privileged access workstations (PAWs) to protect the administrator path, and just-in-time (JIT) approval to reduce standing privilege. The sections below turn Microsoft and CISA guidance into an operating baseline for on-premises and hybrid environments.
How often should Active Directory be patched?
There is no single interval that is safe for every organization. Patching is a preventive control, so set a risk-based cadence and shorten it when a vulnerability is being actively exploited or affects identity infrastructure. A practical policy gives domain controllers, identity connectors, administrative hosts, and management systems that can control Tier 0 the highest priority. Apply the same discipline to applications, operating systems, network devices, hypervisors, backup systems, endpoint tools, and other platforms that could provide an attacker a path to the directory.

Microsoft’s security guidance explicitly includes patching applications, operating systems, and devices. Treat that as a complete attack-path requirement rather than a server-only task: an unpatched management console or virtualization host may be as dangerous as an unpatched domain controller.
Build a risk-based patch cadence
- Emergency changes: handle actively exploited or identity-critical vulnerabilities as soon as testing and a safe rollback plan permit. Use an emergency change record rather than waiting for a routine window.
- Routine changes: maintain a recurring cycle for operating-system, application, firmware, and agent updates. The interval should reflect exposure, exploitability, vendor severity, compensating controls, and the time required to test.
- Exception handling: document every deferral, its owner, compensating controls, expiry date, and next review. An exception without an end date becomes permanent exposure.
Patch Active Directory safely
- Inventory the control path. List domain controllers, read-only domain controllers, sites, trusts, DNS and certificate dependencies, identity synchronization, backup and recovery tools, hypervisors, monitoring platforms, and administrative workstations. Mark any system that can modify, impersonate, or recover Tier 0 as Tier 0.
- Test in a representative environment. Include the operating-system versions, authentication protocols, applications, scripts, agents, and network paths used in production. Test logon, Group Policy, DNS, replication, certificate use, service accounts, and recovery procedures.
- Verify recovery before maintenance. Confirm recent, usable system-state and directory-aware backups, test that recovery media and credentials are available, and record the rollback decision point. A backup that has never been restored is an assumption, not a recovery plan.
- Schedule and communicate. Use change control, maintenance windows, dependency owners, and an outage communication path. Sequence domain controllers so authentication and replication remain available; confirm replication health before moving to the next server.
- Apply and validate. After each change, check replication, DNS registration, time synchronization, Group Policy processing, authentication from representative clients, and the health of connected identity services. Review event logs for new errors.
- Close the change or roll back. Record versions, test results, exceptions, and observed issues. If validation fails, follow the preapproved rollback or recovery procedure instead of improvising on a live controller.
What not to assume
Do not equate a vendor release schedule with a safe delay, and do not promise that patching alone prevents compromise. Patching lowers the chance that a known flaw becomes an entry point; monitoring, segmentation, strong authentication, and recovery limit the damage when prevention fails.

What should be monitored in Active Directory?
Monitoring is the detective half of the control set. Collect events centrally, protect the logs from tampering, synchronize time, and define who investigates alerts. The minimum useful scope covers directory-object changes, Windows security events, privileged-account activity, and security-control tampering.
Directory and group changes
- Creation, deletion, or modification of users, computers, service accounts, groups, organizational units, trusts, and Group Policy objects.
- Additions to or removals from privileged groups, including Domain Admins, Enterprise Admins, Administrators, and delegated groups that can alter identity policy.
- Changes to account-control flags, password settings, delegation, authentication methods, SPNs, certificates, and replication permissions.
- Modification of access-control entries on the domain, organizational units, and sensitive objects.
Authentication and Windows security events
- Unusual logon types, failed-logon bursts, impossible travel patterns, new source hosts, and authentication from a lower trust tier into a higher one.
- Creation or use of new services, scheduled tasks, remote-management sessions, and credential-dumping indicators on administrative hosts.
- Attempts to disable or impair endpoint protection, auditing, event forwarding, backup agents, or other security tooling.
- Replication, DNS, certificate, and time-service errors that could hide or enable identity attacks.
Privileged activity and role activation
Alert on privileged-account use outside approved windows, from an unmanaged workstation, or against an unexpected tier. CISA’s SCuBA baseline states that user activation of the Global Administrator role shall trigger an alert. In a hybrid environment, route that cloud-role alert into the same incident process used for on-premises privileged events, while keeping the cloud requirement clearly distinct from an on-premises AD rule.

A monitoring checklist
- Enable advanced auditing appropriate to your domain and operating-system versions.
- Forward security, directory-service, and relevant operational logs to a protected SIEM or equivalent platform.
- Normalize identities, source devices, target objects, and administrative tiers so correlation can detect tier violations.
- Write high-confidence detections for privileged-group changes, unusual administrator logons, security-tool disabling, and role activation.
- Assign an owner, severity, response time, and escalation path to every alert.
- Review privileged activity and access exceptions on a defined schedule; retain evidence long enough for incident investigation and compliance needs.
Should domain administrators use separate accounts?
Yes. Use a standard account for email, web browsing, collaboration, and other everyday work, and a separate, dedicated administrative identity only for approved administrative tasks. CISA recommends separating administrator and user accounts, and Microsoft advises against using highly privileged identities for routine activity.
The reason is exposure surface. A daily account encounters links, attachments, browser sessions, third-party applications, and local tokens. If that identity also has domain-level authority, one phishing event or malware infection can immediately become an identity-control incident. A dedicated administrator identity is used less often, from a more controlled host, and can be monitored and disabled without interrupting ordinary work.

Operating rules for separate identities
- Use distinct names and credentials; never reuse the daily password, recovery factors, or browser profile for the administrative identity.
- Use the administrator account only from an approved administrative host or PAW, with phishing-resistant MFA where supported.
- Do not read email, browse the public web, or install unapproved software in the administrative session.
- Assign each identity only the rights required for its tier and task. Do not use a Domain Admin identity for routine server or workstation support.
- Record ownership, purpose, manager approval, expiration or review date, and emergency recovery contacts.
- Monitor interactive and remote use, failed authentication, privilege elevation, and access from an unexpected tier.
What is the Active Directory tier model?
The AD DS tier model is a security architecture that separates administrative identities, workstations, and managed assets into trust tiers. It limits credential exposure and prevents a compromise in a lower tier from becoming a shortcut into the identity control plane.
| Tier | What it contains | Administrative boundary |
|---|---|---|
| Tier 0 | Domain controllers, identity services, and systems that can directly or indirectly control them | Only Tier 0 identities and management paths may administer these assets |
| Tier 1 | Member servers and server-management solutions | Server administrators manage servers but do not use those credentials on Tier 0 |
| Tier 2 | End-user devices and account-support functions | Help-desk and workstation administration remain isolated from server and identity control |
Classify by effective control, not by product label. A backup, monitoring, patching, hypervisor, antivirus, or EDR system that can alter, restore, execute on, or obtain credentials from Tier 0 must itself be protected and administered as Tier 0. The same rule applies to service accounts, automation servers, remote-management gateways, and privileged scripts.

Enforce tier boundaries
- Issue separate administrative identities for each tier; do not reuse a Tier 0 credential in Tier 1 or Tier 2.
- Restrict network paths so lower-tier hosts cannot initiate management sessions to higher-tier assets.
- Use tier-appropriate PAWs or hardened jump hosts and deny ordinary browsing and mail on them.
- Review Group Policy, delegation, service accounts, scheduled tasks, and management tooling for hidden cross-tier paths.
- Alert whenever an identity, device, or tool violates the expected tier relationship.
Do I need a privileged access workstation?
A PAW is a hardened workstation dedicated to administration. It protects the administrator path itself rather than only the server being managed. Use one whenever an identity can affect Tier 0; organizations with smaller environments can begin with a hardened administrative host or jump server, provided it receives equivalent controls.

PAW baseline
- Use a supported, fully patched operating system with application allow-listing and endpoint protection.
- Require MFA and restrict sign-in to approved administrative identities.
- Disable or tightly control email, public browsing, removable media, local administrator rights, and unapproved software.
- Allow management connections only to the tiers the operator is authorized to administer.
- Forward workstation, authentication, and security-tool logs to protected central storage.
- Maintain a separate recovery process; do not make the PAW the only place where emergency credentials or backup keys exist.
How do PAM and just-in-time access remove standing privilege?
Privileged access management (PAM) replaces permanent group membership with an approval workflow. Just-in-time access provisions membership when an approved task begins, limits its duration, and removes it automatically when the task ends. Just-enough administration narrows the permission set to the commands or objects needed for that task.
Best Value
- The operator submits a ticket describing the target, action, business reason, and requested duration.
- An authorized approver confirms scope and checks for separation-of-duties conflicts.
- The PAM workflow grants the required group membership or delegated role for the approved window, with MFA and session controls.
- The operator performs the task from the correct PAW or jump host. Record commands, target objects, and session identity where technically possible.
- The workflow removes access automatically, verifies removal, and stores the approval and activity trail.
- The owner reviews exceptions, failed removals, and recurring requests to decide whether delegation or automation should replace ad hoc elevation.
JIT does not make a weak design safe by itself. Protect the approval system, its service accounts, APIs, emergency-access process, and logs as part of the relevant tier. Test automatic removal and define a break-glass procedure that is monitored and reviewed after use.
How do I enforce separation of duties in Active Directory?
Separate directory administration, server administration, security monitoring, backup and recovery, and approval functions where staffing permits. No single person should be able to request, approve, execute, and conceal a high-impact identity change without independent review.
Design incompatible combinations
- Directory administration versus approval of directory privilege.
- Security monitoring versus the ability to disable or delete the monitoring controls.
- Backup or recovery administration versus unsupervised restoration of Tier 0 systems.
- Help-desk support versus membership in domain-level administrator groups.
- Change implementation versus final production approval for the same identity-control change.
Document the combinations that are incompatible, map them to groups and workflows, and alert when a user receives conflicting access. Microsoft Entra governance guidance uses incompatible-access packages, alerts, and periodic reviews; the same governance principle can structure on-premises AD processes, while cloud role requirements remain specific to the hybrid service.
Review and recover
- Maintain an authoritative list of privileged identities, groups, service accounts, PAWs, management tools, and their owners.
- Perform periodic access reviews with evidence of approval, removal, and unresolved exceptions.
- Reconcile actual group membership and delegated permissions against the approved design; investigate drift.
- Test a scenario in which an administrator account, PAW, or PAM service is compromised. Confirm that another authorized person can revoke access, restore service, and investigate the event.
- After incidents or major architecture changes, update tiers, incompatible-access rules, monitoring detections, and recovery documentation.
What should an on-premises or hybrid implementation prioritize?
Start with the identity control plane and work outward. Inventory every direct and indirect path to domain controllers, then protect the hosts, credentials, tools, and people that use those paths. In hybrid environments, include synchronization servers, cloud identity roles, federation, conditional-access administration, and emergency accounts in the same threat model, but label which control is enforced on-premises and which is cloud-specific.
| Implementation choice | Lower operational burden | Stronger control |
|---|---|---|
| Privilege model | Standing membership with manual review | JIT approval, time limits, and just-enough permissions |
| Administrator workstation | Standard desktop with policy restrictions | Dedicated PAW or hardened jump host |
| Monitoring depth | Basic authentication and server logs | Object changes, Windows events, role activation, tier violations, and security-tool tampering |
| Governance | Annual or ad hoc access checks | Defined reviews, incompatible-access rules, alerts, and documented exceptions |
| Recovery | Unverified backups | Tested directory-aware backups, rollback plans, and break-glass procedures |
Choose the stronger option where compromise would affect the identity control plane. If staffing or tooling prevents full implementation, document the gap, add compensating controls, assign an owner, and set a date to reduce the remaining standing access.
