Achilles Tendon Surgery Recovery: From Early Protection to Return to Sport

Recovery after Achilles tendon surgery is a gradual progression from protecting the repair to restoring walking, calf strength, running, and sport-specific function. Everyday activity may return in 4 to 6 months, but return to sport can take 6 to 12 months or longer; returning to a former activity level may take 18 to 24 months. These are different milestones, not competing estimates, and your surgeon and physical therapist should set your individual plan.

What happens during Achilles tendon recovery?

Rehabilitation usually moves through four broad stages: protection, gradually increased loading and motion, strength and control work, then a gradual return to running and sport. The timing and restrictions depend on the repair, clinical findings, symptoms, and the treating team’s protocol. A published protocol is not a personal treatment plan.

Early protection

Immediately after surgery, the tendon is protected in a splint and/or boot. Crutches may be used to take weight off the ankle. The surgeon determines when and how much weight you may put through the leg; follow those instructions rather than changing your boot, heel wedges, or weight-bearing yourself. Mass General Brigham describes weeks 0–3 as the immediate postoperative phase and says care should be individualized to the repair and clinical findings (Mass General Brigham rehabilitation protocol). MedlinePlus also describes boot use and crutches for offloading (MedlinePlus aftercare).

Progressive loading and ankle movement

As healing progresses, the care team may advance weight-bearing and ankle movement according to the plan. A 2022 GAIT Study Group expert consensus reported an average non-weight-bearing period of 2.3 weeks and recommended keeping the foot pointed downward (plantarflexed) during the first four weeks, with ankle movement not going beyond neutral during that period (GAIT Study Group recommendations). These are consensus recommendations for the studied context, not universal instructions. Do not adjust wedges or start ankle exercises without clearance.

Strength, balance, and walking control

Rehabilitation builds toward better walking mechanics, calf strength, balance, and single-leg function. In the GAIT recommendations, concentric two-leg heel raises begin after six weeks, while stretching and eccentric exercise are not started before 12 weeks. Your clinician may use a different schedule based on your repair and progress.

Running and sport

Running and sport come later because they demand more strength, control, and confidence than ordinary walking. A 2022 Ohio State Wexner Medical Center guideline describes return to sport at 9–12 months, depending on injury severity and sport, with progression based on functional criteria rather than time alone (Ohio State guideline). Other sources give earlier estimates for starting athletic activity, but starting participation is not the same as regaining pre-injury performance.

How long does Achilles tendon surgery recovery take?

The answer depends on what “recovered” means. Estimates below describe different endpoints and come from different sources, populations, and protocols; they should not be read as a single universal schedule.

Milestone or estimate What the source says
Normal activity with physical therapy 4 to 6 months, according to the undated MedlinePlus patient aftercare page, accessed 2026 (MedlinePlus).
Boot or brace protection; athletic activity 6 to 8 weeks of boot or brace protection, with athletic activity typically after six months, according to undated American Orthopaedic Society for Sports Medicine patient guidance, accessed 2026 (AOSSM guidance).
Overall recovery; return to sport 6 to 9 months for recovery and 9 to 12 months for return to sport depending on injury and sport, according to Ohio State Wexner Medical Center’s 2022 guideline. Its phases are approximate and function-based (Ohio State guideline).
Return to a former activity level 18 to 24 months, according to Mayo Clinic’s undated treatment overview, accessed 2026. This broader endpoint can extend well beyond beginning sport (Mayo Clinic overview).
Consensus rehabilitation milestones Average 2.3 weeks non-weight-bearing; plantarflexion during the first four weeks; two-leg concentric heel raises after six weeks; stretching and eccentric exercise no earlier than 12 weeks; and average sports initiation at 24.4 weeks. These are GAIT Study Group consensus outputs from 2022, not guarantees for an individual (GAIT recommendations).

An older archived American Academy of Orthopaedic Surgeons guideline summary listed return to sport within 3–6 months after operative treatment as an option for people who participate in sports. It is an older option, not a universal current expectation; later guidance differs and functional readiness matters (AAOS archived guideline summary).

What should be in place before returning to running?

A 2026 modified Delphi consensus study proposed seven criteria for return to running. The authors noted that evidence-based criteria remain limited, so treat these as expert-consensus proposals, not a validated clearance checklist (2026 modified Delphi study):

  • No pain during daily life.
  • No pain during or after rehabilitation sessions.
  • Walking without a limp.
  • Ability to walk on tiptoes.
  • Ability to perform ten single-leg heel rises.
  • Good single-leg balance.
  • Psychological readiness to run.

Ohio State’s guideline likewise describes phase progression as readiness-based rather than determined solely by elapsed time (Ohio State guideline). Your treating team should assess your symptoms and function before you advance.
