A Quarter of Male U.S. Army Soldiers May Have Low Testosterone—What the Number Really Means

Yes, a recent estimate suggests that roughly one in four male active-duty Army personnel had a morning testosterone reading below a commonly used screening threshold. That does not mean one in four soldiers has medically confirmed hypogonadism. The estimate comes from a single measurement in a small sample and must be separated from a clinical diagnosis requiring symptoms and repeat testing.

What the Army estimate actually measured

A research letter published in JAMA on October 15, 2026, analyzed morning testosterone measurements from 479 male active-duty soldiers. Of those, 124 had readings below 300 ng/dL, a threshold the American Urological Association considers reasonable when evaluating possible testosterone deficiency. The Washington Post extrapolated that screening result to an estimated 100,691 active-duty Army personnel, or about one-quarter of the force.

That extrapolation applies to active-duty men in the Army. It is not a measurement of every U.S. service member, every military branch, or the prevalence of diagnosed hypogonadism.

Study findings and Pentagon policy at a glance

Question What the evidence says
What was measured? A single morning testosterone concentration in 479 male active-duty soldiers.
Screening result 124 soldiers were below 300 ng/dL; the extrapolated Army-wide estimate was 100,691 personnel.
Reference points 300 ng/dL is the AUA threshold cited for evaluating possible deficiency. A harmonized CDC reference study places the lower limit of normal at 264 ng/dL for healthy, nonobese men aged 19 to 39.
Age and body size About 33% of men aged 30 or older in the sample were below 300 ng/dL, compared with about 20% of younger men. Older age and higher BMI were associated with lower readings.
Clinical diagnosis Experts say diagnosis generally requires symptoms or signs plus low testosterone confirmed by repeat morning testing.
Pentagon screening rule Clinical guidance requires testing for male service members aged 30 and older. Younger members may request testing; the initial policy excludes women.
Treatment and evidence limits Testosterone replacement is voluntary. The guidance warns about sperm-count suppression and says evidence does not support testosterone for improving energy, vitality, physical function, or cognition in men without an established indication.

Why a low reading is not automatically a diagnosis

Testosterone is not a fixed value. JAMA notes that concentrations vary with age, time of day, obesity, illness and physical stress. A morning draw is preferred because levels follow a daily rhythm, but even a properly timed result can be temporarily depressed by acute illness, heavy training, inadequate recovery or other physiological stressors.

The 300 ng/dL figure is therefore a screening and evaluation threshold, not a line that independently proves disease. The lower CDC reference value illustrates why interpretation depends on the population being compared: it was derived for healthy, nonobese men aged 19 to 39, not for an older or physically stressed military population.

A clinician evaluating a soldier would normally ask about symptoms and signs, review medical factors and medications, and arrange another early-morning measurement before labeling the person testosterone deficient. Symptoms can include sexual or reproductive problems, but a number without a compatible clinical picture is insufficient on its own.

Age and BMI help explain the Army-wide estimate

The sample did not have a uniform risk profile. Men aged 30 and older were more likely to fall below the screening threshold than younger men, and higher BMI was associated with lower testosterone. Those relationships mean that an overall percentage can conceal meaningful differences between age groups and body-size categories.

They also help explain why the Pentagon selected an age-based testing requirement. The policy targets men most likely to produce a low screening result while allowing younger service members to request evaluation when symptoms or other concerns warrant it. The age rule is a screening decision, not proof that every older soldier has a hormone disorder or that every younger soldier has normal levels.

What the Pentagon’s policy is designed to do

Associated Press reporting says the Pentagon describes its guidance as an effort to identify and treat “suboptimal hormone levels” to support warfighter health and force readiness. The stated approach is proactive testing followed by voluntary treatment when a clinician finds an appropriate indication.

The same guidance places boundaries around that rationale:

  • It says evidence does not support testosterone as a general way to improve energy, vitality, physical function or cognition.
  • It flags suppression of sperm production and advises against prescribing testosterone to men planning to father a child in the near term.
  • It recognizes practical issues for deployed troops, including controlled storage, supply, custody and continuity of treatment.
  • It initially applies the mandatory testing requirement to men aged 30 and older; younger men can request testing, and women are excluded from the initial policy.

These safeguards matter because replacing a hormone can create risks and logistical obligations. Screening is not an authorization for a service member to self-treat or to use testosterone as a performance enhancer.

How to interpret a low result in practice

  1. Confirm how the sample was collected. Ask whether it was drawn in the morning and whether illness, unusual physical stress or another temporary factor could have affected the result.
  2. Discuss symptoms and medical history. A clinician should consider sexual, reproductive and other relevant symptoms alongside the laboratory value.
  3. Repeat the morning test when appropriate. A second low result is generally needed before diagnosing deficiency, rather than relying on one screening measurement.
  4. Discuss fertility before treatment. Testosterone therapy can suppress sperm production, so men planning near-term fatherhood need that issue addressed before accepting replacement therapy.
  5. Follow military medical guidance. Deployment storage, resupply and treatment continuity can affect whether a prescribed regimen is practical in the field.

What remains uncertain

The estimate has several important limits. It is based on 479 sampled soldiers, one morning measurement per person and an extrapolation to the wider active-duty Army. The study therefore estimates how many readings fell below a threshold; it does not establish how many soldiers have persistent, symptomatic hypogonadism.

The Pentagon’s implementation details were current as of October 15, 2026 and may change. Results from this Army sample should not be transferred automatically to other branches, women, reserve components or civilian populations. The most defensible conclusion is narrower: low screening readings may be common enough in active-duty Army men to justify systematic evaluation, but only clinical assessment and repeat testing can determine who actually has testosterone deficiency.
