Then correct the form using that state’s current instructions and resubmit through a route accepted for that specific form. There is no single nationwide correction form or resubmission method.
First identify the rejection and the form
A portal validation error, a workers’ compensation board rejection, an insurer’s objection, and a document that arrived but was not matched to a case are different problems. Keep the notice or error message and note any receipt or reference number. Check which organization issued it and whether it explains how to correct the problem.
Before editing, confirm the state, form number and revision, who is responsible for filing, the intended recipient, and the form’s current submission instructions. Requirements vary by jurisdiction and by form; an example from another state is not a substitute for the instructions that apply to your filing.
Correct the data and the document
- Work from the notice. Make a list of every field, signature, page, or attachment identified as missing, inaccurate, unreadable, or unacceptable. If the notice is unclear, ask the issuing board or filing support unit how to address the specific item.
- Verify identifiers and details. Check the claim or case number, injury date, claimant and employer details, and insurer or claims-office information where the form requests them. Do not guess when information is unknown or conflicting; ask the responsible agency or claim administrator how to proceed.
- Check required sections and signatures. Make sure the correct person completed the sections assigned to them, and that required signature and date fields are complete. Include every required page and supporting attachment.
- Inspect the saved file. Reopen the final PDF and verify that typed entries appear, pages are present and legible, and attachments are included. Follow the form’s rules about permitted formats and alterations.
- Use an accepted route and keep evidence. Submit through a channel listed for that exact form and filer. Retain the complete copy and any portal confirmation, tracking information, or delivery record. Send copies to other parties if the form’s directions require them.
Common problems—and what official examples show
Missing fields, identifiers, or pages
Georgia’s State Board lists defects including a missing Board Claim Number; incomplete sections on WC-1; a missing or incorrect five-digit SBWC ID; missing insurer or claims-office details; illegible or altered forms; multiple injury dates on one form; missing claimant or employer addresses; and outdated board forms. Georgia also says WC-1 and WC-14 must be typed or printed in black ink. These are Georgia-specific requirements, not a nationwide checklist. See the Board’s Reasons Forms Are Rejected.

Identifiers must remain associated with the pages they identify. In a March 30, 2026 bulletin, New York’s Workers’ Compensation Board advised placing the WCB Case ID and accident date in the upper-right area of every page. If no WCB Case ID is available, the bulletin says to use the claimant’s name and accident date. The Board warns that missing identifiers can leave a submission unmatched to the case file. Blank or incomplete PDFs
New York’s forms pages warn that entered data may not be transmitted if a completed form is not printed to PDF. The Board’s instructions also describe saving the completed PDF and opening downloaded forms in Adobe Reader if needed. Reopen the file you plan to submit rather than assuming that what appeared on screen was saved. The Board’s claimant forms page and employer forms page provide form-specific directions; claimant instructions also address multi-page or two-sided forms and say not to send instruction-only pages to the Board.

Outdated forms or an unacceptable filing channel
Check the current page for the exact form before resubmitting. New York may announce when older versions are no longer accepted and marks forms for which only the current version is accepted. Georgia also lists outdated board forms as a reason for rejection.
Submission methods can change, and paper is not always an acceptable fallback. New York announced that, effective April 1, 2026, it no longer accepts faxed claim-related forms and documents; its bulletin lists mail, email, electronic upload, web filing, and XML, with exceptions for certain non-claims units. Indiana requires forms marked with an asterisk to be submitted electronically through an approved EDI 3.1 process and says hard-copy forms sent before electronic submission will be rejected. If the rejection reason is unclear or the problem happens again
Contact the board or support unit responsible for the form, or the claim administrator if the notice came from the insurer. Have the form name and revision, rejection wording, submission date, and receipt or reference number ready. Ask which field or attachment must change and which resubmission route is currently accepted. A call or inquiry does not necessarily extend a filing deadline, so do not assume it does; follow the deadline instructions that apply to your claim.
