How Robots Recover When a Task Goes Wrong

Robots recover through a feedback loop: detect a deviation, diagnose its likely cause, correct it, and verify that it is safe to continue.

Robots recover by detecting that an action did not produce its expected result, diagnosing what likely happened, choosing a correction, and checking that the correction worked before continuing. The method depends on the robot and failure: a dropped object, a force or pose error, and a quadrotor losing control authority call for different responses.

How a robot detects that something went wrong

A robot needs evidence that its action diverged from expectation. Rather than treating every sensor reading as equally relevant, a task-aware system monitors signals tied to the current action and checks selected conditions after completing an instruction. Those checks can reveal a missed result before the next action depends on it. NASA’s 1989 testbed description details this approach, while a 2025 manipulation framework organizes fault handling around detecting pose and wrench errors.

How it diagnoses the failure

Detection says that something is wrong; diagnosis tries to determine what. A task plan by itself may not show what the robot actually did or where objects ended up. The NASA system builds an event trace from sensor observations and tracks objects and workspace locations, then uses recent history and task knowledge to reason about the state after an error. That context can help distinguish a failed grasp from a later problem caused by an earlier missed step.

What a robot can do to recover

The right response depends on the failure, the robot’s current state, and whether a safe corrective action remains possible.

  • Retry locally: Repeat an action or make a small correction when the system has reason to believe the same step can succeed safely.
  • Replan: Add corrective steps or return to an earlier task state if the original sequence no longer fits the workspace.
  • Reset: Use a dedicated skill to restore a usable state after a disruption such as a dropped object or collision.
  • Use a learned recovery policy: Switch to a separate policy that returns the robot to a state where its normal controller can continue.
  • Ask for help: Request operator intervention if the system cannot find or verify a safe recovery.

RecoveryChaining is a manipulation example: sensed failure triggers a local learned recovery policy, and the authors report transfer from simulation to a physical robot. A different approach appears in the FAU-indexed 2025 work on fault handling for model predictive interaction control, experimentally validated on a 7-degree-of-freedom Franka-Emika robot.

The CVPR 2026 listing for FLARE describes retry for deviations and a reset pipeline for state-breaking failures such as dropped objects or collisions. That is the listing’s description; it should not be read as an independent assessment of the paper’s results.

How recovery is verified

A corrective movement is not proof that the robot can safely continue. The system needs to check whether it has returned to a state in which the task can proceed. In NASA’s testbed, a successful appended recovery state leads back to the original task; if recovery fails, the system can generate another plan or send a message asking an operator to intervene.

Why recovery must remain physically possible

Recognizing a risk does not guarantee the robot can still avoid it. If the planned action has already used up the time or control authority needed to recover, an alarm can come too late. The RAYA project’s authors describe this problem directly: “A robot can predict failure and still be unable to prevent it.” Their framework incorporates a learned recoverability margin into an optimal controller and adjusts task priorities as that margin declines.

The RAYA authors report 7,200 simulation episodes per controller across quadrotor and autonomous-vehicle benchmarks. They also report deployment on a 35-gram Crazyflie quadrotor and 40 combined hardware flights under wind. In those trials, the project page says RAYA completed 10 of 10 six-cycle missions, while each of three baselines failed every trial. These are author-reported results for the specified experiments, not general reliability figures for robots.

How to compare robot-recovery methods

There is no single recovery rate or best method established across robots. Results concern particular systems, tasks, hardware, and test setups; raw success figures from unrelated experiments are not a common benchmark. Learned policies may also fail to transfer to a different robot or failure type.

  • Failure and task: Is it designed for manipulation, flight, or another domain, and which failures does it address?
  • Evidence: Which sensors or state signals reveal the problem?
  • Diagnosis: Does the system use an execution trace, a learned detector, a task model, or another mechanism?
  • Response: Does it retry, perturb a motion, reset, replan, switch to a learned policy, or hand off to a person?
  • Verification and safety: What confirms that normal execution can safely resume, and what happens when it cannot?
  • Evaluation setting: Is evidence from simulation, laboratory hardware, or real-world deployment?
