Track requirements, high-risk failure scenarios, modeled behavior, input combinations, security threats, and test sensitivity as separate measures. Each answers a different question; none turns into a reliable overall quality score simply by being averaged with the others.
Start by defining what “covered” means
A coverage percentage is meaningful only when its coverage items and scope are explicit. ISO/IEC/IEEE 29119-1:2022 defines test coverage in terms of specified items exercised by test cases; examples include equivalence partitions, state transitions, and executable statements. See the ISO overview of ISO/IEC/IEEE 29119-1:2022.
For any measure, record the numerator, denominator, exclusions, test level, and reporting window. A useful calculation is covered in-scope items / total in-scope items. Define the evidence required to count an item as covered—for example, a linked test has run and passed, or a scenario was exercised and its result recorded. Coverage alone does not say whether a test passed.
- Test basis: the requirements, workflows, risk register, state model, interfaces, or quality attributes that tests address.
- Scope: which items and product versions or environments are included, and what is excluded.
- Evidence: the test or activity linked to each item, plus its latest status: passed, failed, blocked, or not run.
- Ownership and review: who updates the item set when behavior, requirements, or risks change.
Keep different bases in separate rows rather than summing their numerators and denominators. Requirements, states, threats, and code elements are not interchangeable units.

Measure coverage against requirements and acceptance criteria
Build a traceable list of requirements and acceptance criteria, then link each to one or more tests. Report how many in-scope items have relevant tests, and show the latest result for those tests separately. A requirement with a linked but failing test is not evidence of successful behavior; a requirement with no linked test is an uncovered gap.
Review the requirements themselves, too. A vague, contradictory, or incomplete requirement can be fully linked to tests while still failing to describe what users need. NASA’s report on coverage metrics for requirements-based testing discusses criteria applied to formal requirements, including antecedent coverage and Unique First Cause coverage for Linear Temporal Logic properties. These formal methods are most relevant where requirements are expressed in a suitable formal structure; they are not a substitute for checking ordinary acceptance criteria with stakeholders.
Track risk scenarios separately from requirements
List consequential ways the system could fail, connect each scenario to preventive or detecting tests, and report uncovered high-risk scenarios distinctly. Risk-based testing uses analyzed risk to guide test selection and resources. The application team must define its scoring scale and acceptable residual risk; there is no universal scale implied by a coverage percentage.
A single aggregate can conceal a serious gap: many low-impact scenarios may be covered while one severe failure path is not. A risk view should therefore show the scenario, impact or priority, linked tests, latest outcome, and any accepted residual risk. Revisit priorities as the product and its operating context change.
Cover behavior, states, and input space
States and transitions
For systems with meaningful states, define the states and transitions in scope, then link tests to transitions or transition sequences. Report which modeled items were exercised and which were not. State-transition coverage only describes the model you wrote: omitted states, invalid transitions, or incorrect assumptions remain outside its denominator.

Input partitions, boundaries, and combinations
Divide inputs into meaningful equivalence partitions, identify boundary values, and consider combinations that could change behavior. For a form, for example, the model might distinguish valid and invalid formats, empty and maximum-length values, and combinations of optional fields. Use pairwise combinations where appropriate, but state the combination criterion; a pairwise set does not claim to cover every possible combination.
ISO/IEC/IEEE 29119-1:2022 describes specification-based testing through external inputs and outputs and includes state-transition and pairwise testing concepts. The coverage denominator should match a documented model of the product’s expected behavior, not an arbitrary count of test cases.
Use mutation testing to probe test sensitivity
Mutation testing makes deliberate small changes to code or specifications and checks whether the suite distinguishes the changed version from the original. NIST gives changing < to >= as an example. A change detected by a test is typically called “killed”; one that survives needs investigation.

Report the mutation operators and code or specification scope, along with the number of mutants assessed, detected, surviving, excluded, or unresolved. A surviving mutant can point to a missing assertion or test, but some changes may be equivalent in behavior and impossible for the suite to distinguish. Mutation results indicate sensitivity to the selected changes; they do not predict what proportion of real defects the tests will find.
NIST’s IR 8397, Guidelines on Minimum Standards for Developer Verification of Software (published October 6, 2021) discusses verification practices including mutation testing. Treat results as one diagnostic view, not a universal adequacy score.

Include security, fuzzing, and exploratory coverage
Threat scenarios
Derive testable scenarios from the threat model—for example, unauthorized access, malformed input, or misuse of a sensitive workflow—and link each to tests or other verification evidence. Track uncovered high-priority threats separately. NIST recommends threat modeling, black-box cases, fuzzing, and attention to included libraries, packages, and services.
Fuzzing scope
Describe what the fuzzer exercised: the target or interface, harness, input scope, configuration, and duration or resource budget. Record crashes and other findings, and whether they were triaged. “Fuzzing performed” is not a countable coverage claim without this scope. NIST notes that fuzzing generally needs a harness, can be computationally intensive, and often produces better results at scale.

Exploratory work
Track exploratory charters or scenarios completed, areas visited, and findings—not just session count. ISO describes exploratory testing as seeking hidden properties or behaviors that could create failure risk. The results depend on the charter and tester’s exploration; they should complement repeatable tests rather than be presented as an interchangeable percentage.
Build a dashboard without collapsing the measures
A useful report puts the dimensions side by side and makes each denominator inspectable. For every row, identify the test level and the measure’s principal limitation.

Do not average these unlike measures into one “quality” number unless you have a defensible, context-specific method and explain it. Publish exclusions and gaps, especially high-risk ones, rather than letting a headline percentage hide them.

Interpret code coverage as one signal
Code coverage remains useful for finding structural elements that tests did not execute and for supporting traceability among code, requirements, and tests. It is not a proxy for correctness or total test adequacy. NASA’s Software Engineering Handbook entry SWE-066 states that “Merely achieving 100% code coverage isn’t enough.” It explains that full code coverage alone does not establish complete requirements testing or correctness; even 100% function coverage does not show that every statement in each function ran.
The handbook also reproduces the maxim “testing shows the presence of bugs, not their absence.” Read any structural percentage alongside behavioral evidence and known gaps, not as proof that no defects remain.
Set completion criteria around risk, not a universal percentage
The standards and guidance cited here do not establish a universal percentage for overall test adequacy. Set completion criteria for the product’s context: required coverage dimensions, acceptable uncovered items, treatment of high-risk gaps, test outcomes, and approved exclusions. Make the denominator and decision owner clear so a reported number can be interpreted and challenged.
Coverage criteria should evolve when requirements, threats, models, or product behavior change. Validate the test basis with stakeholders; neither a high percentage nor a detailed model can reveal expectations that were never captured.
