What is the difference between sanity testing and regression testing?

The sanity-testing column describes common industry usage, not a universal ISTQB category or prescribed process. Teams should state what their “sanity check” covers rather than assume everyone uses the term the same way.

When should I use sanity testing?
Use a sanity check when a changed build or feature needs a fast initial assessment before spending time on deeper testing. Keep it focused on the relevant change and the basic conditions needed to proceed. For example, after a small update to a sign-in page, a team might check that the page loads and that the changed sign-in interaction works before running a broader test plan.
A passing sanity check is not evidence that unrelated features still work. It is a gate for deciding whether to continue, not a substitute for regression coverage. Because scope is team-defined, record which checks are included and what a pass means in your process.

When should I use regression testing?
Run regression tests after a software, configuration, or data change that could affect existing processes. The goal is to detect defects introduced or uncovered in unchanged areas. ISTQB defines regression testing as change-related testing aimed at finding those defects; Microsoft Learn recommends considering it after relevant changes and before a production change.
Regression testing can be manual or automated. It is not limited to the changed feature: choose existing tests that provide evidence about the behavior at risk. A test that checks only the new or fixed behavior may be useful, but that alone does not establish that neighboring or dependent behavior remains intact.

How do confirmation testing and regression testing differ?
Confirmation testing checks whether a specific reported defect was fixed, typically by repeating the steps that reproduced it. Regression testing checks for collateral defects in unchanged areas. A fix can pass confirmation testing and still introduce a side effect elsewhere, so both purposes may matter when resolving a bug.

Is sanity testing the same as smoke testing?
There is no universal distinction established by the cited standards sources. Organizations use “sanity” and “smoke” inconsistently, and the consulted sources do not define a formal, universal boundary between them. In practice, teams may use either label for an initial check, but the reliable way to understand a process is to ask what it tests, how broad it is, and what decision follows a pass or failure.
Does regression testing retest everything?
No. Regression testing does not inherently require rerunning every test. Its scope can be broad, risk-prioritized, focused on affected areas, or a combination. Microsoft Learn describes four practical approaches:

- Broad coverage: test nearly all processes. This offers wider coverage but costs more to run and maintain.
- Business-impact prioritization: protect the processes whose failure would matter most.
- Change-focused testing: target areas affected by the change. This takes less effort but cannot establish that unrelated areas are unaffected.
- Combined strategy: protect critical processes while increasing testing around changed features.
Choose according to risk and available test time. A change-impact assessment can help identify affected areas, but a narrowly targeted suite should not be presented as proof that every unrelated behavior is safe.
How should teams automate regression testing?
Automate progressively, beginning with high-value, frequently repeated business processes and expanding coverage as the suite and maintenance capacity grow. Microsoft’s guidance says automation can improve speed, coverage, and repeatability while reducing human-error risk. Automation is not automatically cheaper: teams still need to design, maintain, and run the tests.
