How AI Is Supporting Breast-Cancer Screening in Mexico—and What It Has Yet to Prove

In Mexico, AI is being introduced to support mammogram interpretation and is also being studied for breast-cancer risk prediction. Public announcements describe new reading capacity and thousands of studies, while a Mexican test-set study reports how one system classified mammograms against radiologists’ judgments. None of these findings yet shows that AI has caused more cancers to be diagnosed at an earlier stage or improved survival in Mexico.

What AI is doing in Mexico’s mammography pathway

The initiatives described by Mexican public institutions use mammography images as their input. In an interpretation workflow, AI can assist with classifying or flagging studies for human readers; the announced services are intended to support the process of reading mammograms, not to make treatment decisions. Separately, researchers are studying whether AI can estimate an individual woman’s future breast-cancer risk.

These are different uses of AI. Mammogram interpretation concerns the images from an exam. Risk prediction concerns estimating risk, and does not itself read a current mammogram or diagnose cancer. The available announcements and studies do not establish that these tools operate independently of clinicians or replace radiologists.

Three Mexican initiatives, with different evidence behind them

Quintana Roo and Roche México (2025) The state government reported a collaboration using Lunit Insight MMG on 9,000 studies donated under an agreement. It described the initiative as the first of its kind in Latin America. The government’s report of the collaboration and study count; “first in Latin America” is the state government’s characterization. Independent evidence of clinical performance or improved detection and outcomes from this initiative.
Breast-SlimView evaluation (2026) A test dataset of 9,560 mammographic images from 2,390 Mexican women; evaluated for low- versus high-risk categorization. Reported classification performance compared with the consensus of two radiologists: mean sensitivity 0.81, specificity 0.70 and accuracy 0.71. Proof of population-level earlier diagnosis, reduced mortality, or performance in every clinical setting.
MIA research protocol An INSP-listed project to study AI-based breast-cancer risk prediction in Mexican women, with a study period of October 31, 2025 to December 31, 2026. That a study protocol and planned period are listed by INSP. Validated clinical use or reported study results; none are given on the project listing.

The figures in the first two rows describe announced service capacity or reported activity; the Breast-SlimView figures describe a defined evaluation dataset and its classification results. They are not interchangeable measures, and the systems should not be ranked as if they had been tested head to head.

What the Breast-SlimView numbers mean

In the 2026 study by Murillo-Ortiz and colleagues, Breast-SlimView’s low- versus high-risk classifications were compared with a reference formed by two radiologists’ consensus. The reported mean sensitivity of 0.81, specificity of 0.70 and accuracy of 0.71 describe agreement or classification performance for that evaluation—not how many additional cancers were found early in routine screening.

  • Sensitivity (0.81): a measure of how often the system classified cases in the study’s positive category as positive under the study’s definitions and reference standard.
  • Specificity (0.70): a measure of how often it classified cases in the negative category as negative under those same definitions.
  • Accuracy (0.71): the share of classifications that matched the reference overall in the tested data.

Because the study reports classification metrics rather than a population outcome, the results do not show that using the model changes when patients are diagnosed, what stage their cancer is at diagnosis, or their survival. Nor do the reported values make the tool infallible. The study’s comparison was with radiologist consensus, not a trial of an AI-supported screening service against usual care.

Why expanding reads matters—and what remains unproven

Access and diagnostic delays are important context. In an October 2024 communication, Mexico’s Health Secretariat reported that 66.4% of cases were diagnosed at late stages, citing the 2023 Colima national consensus. In the same communication, it said five IMSS-Bienestar teleradiology centers for screening mammogram reads had been launched in Chihuahua, Querétaro and Mexico City.

More capacity to interpret mammograms could support access to reads, especially where specialist interpretation is constrained. That is a plausible workflow benefit, but it is not the same as demonstrating earlier diagnosis. To establish an effect on early detection, evidence would need to show changes in outcomes such as the stage at diagnosis among screened patients, rather than only the number of studies processed or a model’s classification performance. The announcements and study results described here do not report that kind of outcome for these AI initiatives.

Mammography remains central to the screening guidance cited in Mexico

AI assistance does not replace the screening exam or clinical follow-up. Mexico’s National Cancer Institute (INCan) describes a breast-detection program that uses mammography to identify changes before they are palpable or visibly symptomatic and targets women over 40. In an October 2025 communication, the Health Secretariat recommended annual mammography starting at age 40. That is the government guidance stated in that release, not individualized medical advice; a person’s screening plan should be discussed with a qualified health professional.

The same 2025 communication attributed to Claudia Arce Salinas, head of medical oncology at INCan, the statement: “Detecting breast cancer in time raises the chances of cure to more than 90%.” This is an attributed public-health claim, not a universal prognosis for an individual patient; outcomes vary with the cancer and the person’s circumstances.

Can AI read a mammogram?

AI systems can analyze mammography images and may help classify or flag studies for interpretation. In Mexico, ISSSTE has announced AI-supported interpretation rooms, Quintana Roo has reported a project using Lunit Insight MMG, and Breast-SlimView has been evaluated for classification against radiologists’ consensus. These examples support describing AI as a developing aid to mammogram interpretation—not as a standalone diagnosis or a proven way to improve population outcomes.
