Start with the decision you want customer feedback to inform, then define who should answer, what experience they should rate, and which measure fits the question. A useful questionnaire asks clear, focused questions, gives respondents answer options that fit their experience, and is tested before launch. Its results are only as representative and comparable as the survey’s sampling, wording, and delivery allow.
Start with the decision, audience, and experience
Write one sentence describing what you need to learn and what action a response could help you take. Then specify the customer group, product or service, touchpoint, and period respondents should consider. For example, a survey about a recent support interaction should go to customers who contacted support, not an undefined group of all customers.
Before creating a survey, consider whether existing operational data, interviews, or another research method would answer the question better. If you do use a questionnaire, choose a sampling frame and recruitment method that give the intended population a reasonable chance to be represented. A survey of people who chose to respond does not automatically represent every customer.
Choose the measure that matches what you need to know
Customer satisfaction, recommendation, and effort are related, but they measure different things. Select a core question based on the decision at hand rather than combining the answers into an undefined customer-happiness score.

For a satisfaction question, a direct example is “How satisfied are you with [organization]?” Qualtrics shows a five-point example scale anchored from highly unsatisfied to highly satisfied, but that is an example format—not a universal requirement. Tailor the wording and choices to the actual experience and respondents. See Qualtrics’ CSAT survey examples.
A rating tells you what someone selected, not necessarily why. If an explanation will guide a decision, add one focused follow-up, such as “What most influenced your rating?” or “What one change would improve this experience?”

Write clear, answerable questions
- Ask about one thing at a time. “How satisfied were you with the speed and friendliness of support?” combines two attributes; a single answer cannot show which one the respondent meant. Ask separate questions if both matter.
- Name the experience and timeframe. Avoid vague references such as “the service” when respondents may have used several services or interacted at different times.
- Use familiar, neutral language. Do not imply that a particular answer is expected, or assume the respondent had an experience they may not have had.
- Make the response options fit. Use a single-choice scale, multiple-choice list, or open response according to the question. Keep categories clear and appropriate, and provide “not applicable,” “don’t know,” or a skip option when relevant.
- Do not treat a blank as “none.” A missing answer may mean the respondent did not know, did not want to answer, or never saw the question.
- Use open responses selectively. They can reveal reasons or issues you did not anticipate, but take more effort to answer and analyze. Include them only when the answers could help explain a rating or identify an improvement.
There is no universal question-count rule established by the sources cited here. Keep the questionnaire focused on what you need to learn, and remove questions that do not serve that purpose.
Order and deliver the questionnaire thoughtfully
Put easy, relevant questions first and arrange the rest in a natural sequence. Question order and surrounding context can influence answers, so consider what respondents have just been asked when interpreting later results. Pew Research Center illustrates this response-format effect with a 2008 post-election poll: 58% selected “the economy” when it appeared among closed-ended choices, compared with 35% who volunteered it in an open-ended version. Those figures are not customer-satisfaction findings; they show that answer format can affect what people report. Read Pew Research Center’s guidance on writing survey questions.

Choose a channel and timing that suit the experience and your customers’ access. Email, a website, an app, SMS, chat, mail, and in-person collection are all possible channels; none is best in every situation. Keep respondent burden low, explain the purpose where appropriate, and collect only information needed for the stated use. Follow organizational policies and applicable privacy laws, and consider whether demographic details combined with responses could identify an individual. AAPOR advises allowing respondents to skip sensitive questions.
Pretest the questions and pilot the whole process
Before launch, ask people similar to your intended respondents to explain what each question means to them and how they chose an answer. Cognitive interviews and other qualitative pretests can uncover confusing terms, missing response choices, and assumptions that do not hold. Pew Research Center calls pretesting “an essential step in the questionnaire design process” for evaluating responses to the questionnaire and individual questions, especially when questions are new. Then pilot the complete process—not just the wording—including recruitment, survey programming, delivery, response handling, data cleaning, and analysis. Revise and test again if changes affect the instrument or process. AAPOR’s best-practice page quotes the American Statistical Association’s What Is a Survey?: “The quality of a survey is best judged not by its size, scope, or prominence, but by how much attention is given to [preventing, measuring and] dealing with the many important problems that can arise.” Read AAPOR’s best practices for survey research.

Define data-cleaning rules before interpreting scores. Check for incomplete or inattentive responses, summarize findings in a way that connects to the original decision, and examine differences across customer groups or touchpoints only when the sample supports those comparisons. Low response can leave some types of customers underrepresented, so do not assume respondents stand in for all customers.

When reporting results, include the population, sample construction and recruitment, response count, survey mode, exact question wording and answer options, and any weighting or uncertainty measures used. Compare results over time without confusing measurement changes for customer changes
For a trend, keep question wording, framing, context, mode, timing, and recruitment method as consistent as possible. A changed scale, channel, question order, or sample can alter the result even if customer sentiment has not changed. If you need to change a measure, test old and new versions side by side where feasible, and disclose the change when reporting the trend.
