Respiratory Viruses in the Workplace: A Practical Prevention Guide

Preventing respiratory viruses at work takes a layered plan tailored to the job and the building—not a single mask, purifier, or cleaning routine. Employers should assess how workers may be exposed, improve indoor air, support people who are sick in staying home, encourage appropriate vaccination and hygiene, and use protective equipment where the task calls for it. Risk varies with duties, interactions, and setting; healthcare, crowded, enclosed, public-facing, and congregate workplaces may need more specific safeguards.

How can respiratory viruses spread at work?

Workplace guidance commonly addresses COVID-19, seasonal influenza, and respiratory syncytial virus (RSV). Exposure is possible in any workplace, but risk is not the same for every worker or site. Duties, the number and proximity of interactions, how long people share space, ventilation, and protective measures all matter. OSHA identifies teachers and school staff, retail and transport workers, cleaning staff in congregate settings, and entertainment workers around crowds as examples of occupations that may face heightened exposure; these examples are not a universal ranking. Consider an enclosed vehicle with prolonged passenger contact and poor ventilation: several exposure factors may coincide. A desk-based office, a crowded customer-service counter, a school, and a clinical care area therefore should not automatically receive the same prevention plan.

How should employers build a prevention plan?

Start with a hazard assessment of the actual tasks and spaces, then choose a combination of controls that fits. OSHA’s hierarchy of controls prioritizes eliminating or substituting a hazard where feasible, followed by engineering controls, administrative controls and safe work practices, and then personal protective equipment (PPE). These measures can complement one another; no single measure guarantees that transmission will not occur.

  1. Assess exposure. Identify where people work close together, share enclosed air, interact with the public, or handle potentially infectious material. Revisit the assessment when duties, occupancy, building conditions, or transmission conditions change.
  2. Reduce avoidable contact. Where practical, substitute a lower-exposure method—for example, videoconferencing instead of an in-person meeting.
  3. Improve the space. Maintain and optimize ventilation and filtration for the building and its use. Consider a qualified HVAC professional when evaluating how to improve workplace ventilation.
  4. Set workable practices. Establish illness procedures, support hygiene and routine cleaning, provide training, and consider flexible or remote work when community or workplace transmission is elevated.
  5. Add appropriate PPE. Select it for the task and hazard, and ensure that any applicable OSHA requirements are met.
  6. Involve workers. Ask workers to help develop and implement sustainable practices. OSHA says: “Employers are encouraged to engage workers to create long-term, sustainable, routine practices that protect workers on the job.”

Why indoor air matters—and what a purifier can and cannot do

Airborne viral particles can become more concentrated indoors than outdoors. Protective ventilation can reduce airborne concentrations and overall exposure. CDC/NIOSH describes ventilation broadly: it can involve bringing in outdoor air and diluting indoor air, filtering air through central HVAC or in-room equipment, and using germicidal ultraviolet air treatment. A building’s appropriate mix depends on its type, occupancy, activities, season, and constraints. Multiple measures may work together. A portable HEPA air cleaner can provide additional filtration in a suitable room, but device choice and room suitability depend on the space and its existing HVAC system. It is not a standalone prevention plan, does not guarantee that workplace infections will be prevented, and does not replace minimum outdoor-air ventilation required by building codes. Evaluate filtration alongside the building’s ventilation needs rather than treating an air cleaner as a substitute for them.

What should employers do when workers are sick?

Encourage workers with respiratory illness to stay home while sick, and make that feasible through clear policies, paid leave where available, and flexible telework. CDC’s general community guidance says people with symptoms should wait until symptoms are improving overall and they have been fever-free for at least 24 hours without fever-reducing medicine before resuming normal activities, then take added precautions for five days. This is general guidance, not a universal workplace rule: employers should check current CDC advice and any applicable workplace, local, or setting-specific requirements before setting return-to-work criteria. Employers can also encourage recommended vaccination against influenza and COVID-19, and RSV vaccination when appropriate, and support hand and respiratory hygiene. Cleaning remains a routine practice, but should sit alongside controls aimed at shared air and close interactions.

When are masks or respirators appropriate?

Use PPE when the hazard assessment indicates it is appropriate for the exposure and task. A NIOSH-approved N95 filtering facepiece respirator is a respirator, not simply a generic face covering. If an employer requires respirator use under an OSHA standard or its own policy, applicable respiratory-protection requirements include a written program, medical evaluations, fit testing, and training. Voluntary use has separate requirements, including providing Appendix D information when applicable. Merely handing out respirators does not by itself satisfy the requirements. Consult OSHA’s workplace recommendations and applicable respiratory-protection rules when developing a program.

Why healthcare settings need a separate approach

General workplace advice is not a substitute for healthcare infection-prevention guidance. Healthcare employers may need transmission-based precautions, suitable PPE, worker training, and formal respiratory-protection programs. Clinical care, aerosol-generating procedures, specimen handling, and congregate-care work may call for more specific guidance. Employers should consult OSHA’s healthcare recommendations and the relevant setting-specific requirements rather than applying a general office plan unchanged.
