Her attorneys said she remained alive and was taken to a hospital. The state said it followed its execution protocol, but as of October 3 it had not publicly explained why the attempt failed.
What happened during the execution attempt?
Pike’s execution was scheduled for 10 a.m. local time at Riverbend Maximum Security Institution in Nashville. About an hour beforehand, a federal appeals court stayed the execution. The U.S. Supreme Court later overturned that stay, allowing Tennessee to proceed.
The Associated Press reported that witnesses saw Pike strapped to a gurney and heard her last words. Court filings cited by the AP say officials administered two doses of pentobarbital. Witnesses later heard snoring, and Pike’s attorneys said she was alive and transported by ambulance to a hospital.
Those accounts come from different sources: the dose information was reported from court filings, the snoring from witnesses, and the account of Pike’s survival and transport from her lawyers. They do not, on their own, establish the medical or technical cause of the failure.

What is known about Pike’s condition?
The latest condition update identified in reporting available by October 3 was published October 2. Pike’s attorneys said she was unconscious and on a ventilator while hospital staff worked to save her life. That is an attributed update from that date, not a current clinical record.
Why did the execution attempt fail?
The cause had not been publicly explained by Tennessee in the reporting available as of October 3. The state’s protocol calls for a secondary set of syringes if an inmate is not deceased after the first set, according to the AP. The protocol reportedly does not specify what happens if the person remains alive after the second set.

The Tennessee Department of Correction said it followed the approved protocol and that it did not allow additional procedures beyond those carried out. That is the agency’s position, not an independent finding about what happened medically.
Before the execution, Pike’s lawyers had argued that thrombocytosis and difficulty accessing her veins raised the risk of a painful or problematic injection. Afterward, they alleged that IV problems caused pentobarbital to enter tissue rather than her bloodstream. That explanation remains an allegation by her legal team; the state had not confirmed it, and the ordered review may address it.

What happens next in Tennessee?
Governor Bill Lee ordered a “comprehensive, third-party review” and canceled the other execution scheduled in Tennessee for 2026. As quoted by the AP, Lee said: “Carrying out a lawfully imposed sentence is among the State’s most serious responsibilities, and the people of Tennessee expect it to be done in a manner that is not only legal and constitutional, but is effective.”
The review’s findings had not yet been reported as of October 3. Until evidence from that review or a court finding clarifies the cause, accounts of the IV placement and drug delivery should be treated as unresolved.

What was the legal backdrop?
Pike had challenged Tennessee’s lethal-injection protocol. A September 25, 2026 application filed on her behalf with the U.S. Supreme Court recounts her challenge and procedural history, including the Tennessee Supreme Court’s setting of her execution date for September 30. The application presents Pike’s side of the case; its arguments are not themselves court findings.
The federal appeals court’s last-minute stay and the Supreme Court’s decision to overturn it were separate steps in the proceedings. The latter allowed the execution to go ahead, but it did not resolve the later question of why the lethal-injection attempt failed.

Who is Christa Pike?
Pike was convicted of the 1995 killing of Colleen Slemmer, 19, a fellow student at a Knoxville Job Corps facility. The AP reported that prosecutors said Pike and her boyfriend attacked Slemmer. Pike received a death sentence; her boyfriend received a life sentence with the possibility of parole.
How unusual was the incident?
The Death Penalty Information Center reported that seven people had previously survived execution attempts after difficulties establishing IV access, with no execution drugs administered in those cases. That is the organization’s historical count; it does not establish that those cases are medically or procedurally comparable to Pike’s, in which two doses were reportedly administered. Robin M. Maher, executive director of the Death Penalty Information Center, described Pike’s case as “singular and unparalleled,” as reported by the AP.
