Apple Watch can do much more than count steps, but its health tools are easy to confuse with medical tests. The nine functions below cover heart alerts and ECG readings, sleep and breathing patterns, overnight vital trends, reproductive-cycle estimates, medication logging, and mood check-ins. Each has a different purpose, and availability depends on the watch model, watchOS version, iPhone setup, and country or region.
No official Apple source cited here verifies that a particular doctor at Apple revealed these functions. The product descriptions and limitations come from Apple’s published support and technical materials, including its health-features guide.

The nine features at a glance
| Feature | What the watch produces | Primary use | Important limit |
|---|---|---|---|
| High- and low-heart-rate alerts | Notifications when heart rate crosses a chosen high or low condition | Prompting you to pay attention to an unusual reading | An alert is not a diagnosis |
| Irregular-rhythm notifications | A notification about a rhythm that may be suggestive of atrial fibrillation | Encouraging medical follow-up | It does not confirm AFib |
| ECG app | A recording similar to a single-lead electrocardiogram, with a shareable PDF | Capturing a rhythm reading for discussion with a clinician | It is not a multi-lead clinical ECG |
| Sleep tracking and stages | Sleep duration, schedule/history and estimated time in stages | Understanding sleep patterns | Stages are estimates |
| Possible sleep-apnea notifications | A notice based on repeated breathing-disturbance patterns during sleep | Prompting evaluation for possible sleep apnea | It is not a diagnosis |
| Vitals | Overnight metrics compared with your personal typical range | Spotting several measurements that changed together | Displayed metrics and availability vary |
| Cycle Tracking | Period and fertile-window predictions; supported watches can estimate ovulation retrospectively | Recording cycle information and recognizing patterns | Predictions are not contraception or diagnostic certainty |
| Medications | Medication, vitamin and supplement schedules and logs | Recording whether you took an item | It does not prescribe or adjust treatment |
| State of Mind | Logs of momentary emotions and daily moods | Reflection and trend tracking | It is not a mental-health diagnosis |
Apple describes these as separate health, notification and logging functions rather than nine hidden diagnostic tests. Some require a compatible watch and paired iPhone; others also depend on regional approval.

Where to find it
On the paired iPhone, open the Watch app, go to the Heart settings, and review the High Heart Rate and Low Heart Rate options. You can also review heart-rate data in the Health app. The exact controls and defaults can differ by watchOS release.
How to interpret an alert
A single notification can reflect circumstances such as recent activity, illness, medication, stress or a measurement problem. Sit down, repeat the measurement if appropriate, and seek medical advice for persistent, severe or worrying symptoms. Apple’s health guide explains the notification feature but does not present it as a diagnosis.
2. Irregular-rhythm notifications
With supported hardware and software, Apple Watch periodically analyzes heart-rhythm data and can notify you when the pattern may be suggestive of atrial fibrillation (AFib). Apple’s wording matters: “may be suggestive” is a prompt for follow-up, not confirmation that you have AFib.
What it can and cannot do
The notification is intended for people who are not already diagnosed with AFib and is not a continuous clinical monitor. It can miss episodes and can also produce a notice for reasons that require professional interpretation. If you receive one, save the relevant health information and arrange a clinician’s assessment rather than changing medication yourself.
3. The ECG app
The ECG app uses the watch to create a tracing similar to a single-lead electrocardiogram. Apple says the resulting PDF can be shared with a doctor, which makes the feature useful when a symptom occurs away from a clinic.
Using the reading responsibly
Open the ECG app on a compatible watch and follow its on-screen contact instructions. Treat the result as one data point. A single-lead recording cannot replace a multi-lead ECG, physical examination or clinician’s interpretation, and the app is not available in every country or on every model.

4. Sleep schedules, history and estimated stages
Apple Watch can record sleep duration, show recent sleep history and support a sleep-duration goal and schedule. Apple also presents estimated time in sleep stages, so you can see broad patterns across nights rather than just a bedtime and wake time.
What the estimates mean
Sleep-stage labels are algorithmic estimates based on watch measurements; they are not a laboratory sleep study. Wear the watch consistently, keep the fit comfortable, and look for trends over multiple nights. A missed night or an unusual value is not evidence of a disorder by itself.
Where to review it
Use the Sleep app on the watch for recent information and the Health app on iPhone for history, schedules and longer-term views. Sleep tracking requires a compatible watch, sufficient charge and an enabled sleep schedule or sleep-tracking setting.
5. Possible sleep-apnea notifications
Apple’s September 2024 technical overview says Apple Watch can measure breathing disturbances during sleep. If the disturbance pattern reaches a level associated with moderate to severe sleep apnea, the watch can provide a notification of possible sleep apnea.
The Tool Desk
Outbyte Driver Updater FREEScan for outdated or missing drivers - takes under a minuteDriver Scan →Outbyte PC Repair FREEClear out junk files and repair common Windows errorsFree Scan →Why the wording is deliberate
An alert does not diagnose sleep apnea, establish severity or identify the cause of a breathing disturbance. Snoring, congestion, alcohol, altitude and a poor watch fit can affect sleep measurements. An alert is a reason to discuss symptoms and testing with a healthcare professional, not a reason to self-treat or stop prescribed care.

Apple’s technical explanation is available in Sleep Apnea Notifications on Apple Watch. Model, watchOS and regional eligibility must be checked because the feature is not universal.
6. Vitals: a personal baseline, not a universal “normal”
The Vitals app compares overnight measurements with your own typical range. Depending on the watch and region, it can display heart rate, respiratory rate, wrist temperature, sleep duration and blood oxygen. When multiple measurements move outside your typical range, it may notify you and show possible contextual factors.
Why multiple metrics matter
Vitals is designed to highlight a cluster of changes rather than declare that one number is abnormal for everyone. Travel, illness, training load, alcohol, elevation and a disrupted sleep schedule can all influence a night’s readings. Use the context shown in the app to decide whether to rest, repeat measurements or contact a clinician.

Blood-oxygen availability caveat
Apple’s guide says Blood Oxygen is unavailable in the Vitals app on certain Apple Watch models purchased in the United States on or after January 18, 2024. Apple identifies affected devices by specified part-number endings; the restriction should not be generalized to every watch or every country.
7. Cycle Tracking and retrospective ovulation estimates
Cycle Tracking records menstrual information and uses it to provide period and fertile-window predictions. Heart-rate data can improve those predictions. On supported watches, wrist-temperature data can also contribute to period predictions and retrospective ovulation estimates.
Hardware and regional limits
Temperature-based insights require compatible hardware and consistent overnight wear. Apple also notes that retrospective ovulation estimates are unavailable in some countries or regions. Check the feature list for your exact model and location before assuming it will appear.

Use predictions appropriately
Predicted fertile windows are estimates, not contraception and not proof of ovulation. Record symptoms and cycle changes accurately, and consult a healthcare professional about unusual bleeding, pain, pregnancy concerns or fertility questions.
Do these 3 things before closing this tab:
1Repair Windows errors before they cause bigger problems2Scan for outdated or missing drivers - takes under a minute3Clear out junk files and repair common Windows errors8. Medication logging
The Medications feature lets you keep a list of medicines, vitamins and supplements and log them from the watch. You first add the items in the Health app on iPhone; after setup, the watch can provide the logging interaction at the time you choose.
What it is useful for
A consistent log can help you and your clinician see whether doses were recorded and can reduce reliance on memory. It does not verify that a dose was swallowed, check interactions, recommend a dose or replace instructions from a pharmacist or prescriber.
9. State of Mind logging
In the Mindfulness app, State of Mind lets you record a momentary emotion and a daily mood. Over time, those entries can help you notice relationships between mood, sleep, activity and life events.
Keep the data in perspective
This is a reflection and journaling tool, not a test for depression, anxiety or another condition. If mood changes are intense, persistent or associated with thoughts of self-harm, seek qualified help immediately rather than relying on watch trends.

How to check whether your watch supports a feature
Before buying, upgrading or promising that a function will work, check five separate variables. Apple’s support documentation is the appropriate final authority because compatibility and regional approvals change.

- Identify the exact model. On iPhone, open the Watch app and choose General > About. Record the model name and case generation, not just “Apple Watch.”
- Check watchOS. In the Watch app, open General > Software Update. Install an eligible update only after confirming that your iPhone also meets its requirements.
- Check the paired iPhone and Health setup. Some functions require data to be added or reviewed in the iPhone Health app, including Medications and portions of Cycle Tracking.
- Check your country or region. ECG, irregular-rhythm features, sleep-apnea notifications, temperature-based cycle insights and blood-oxygen displays can have geographic restrictions.
- Check the output type. Decide whether you need a log, an estimate, an alert or an ECG-style recording. A newer watch may add one category without changing the others.

What these health features do not replace
Apple Watch health functions are best understood as an observation and record-keeping layer. Alerts can prompt a conversation; estimates can reveal a pattern; logs can make a consultation more informative. None of the nine functions, alone or together, is a comprehensive diagnosis, emergency monitor or substitute for prescribed care.
Seek urgent help for severe chest pain, fainting, serious breathing difficulty, sudden weakness or other emergency symptoms instead of waiting for a watch notification. For non-urgent changes, bring the relevant readings or exported ECG PDF to a qualified clinician and explain when symptoms occurred.
A separate note on hypertension notifications
In a September 2025 announcement, Apple said hypertension notifications would not detect every instance of hypertension and forecast that the feature could notify more than 1 million people with undiagnosed hypertension in its first year. That figure is Apple’s company estimate, not an independently measured outcome. Availability and rollout should be checked separately from the nine functions above.
Apple’s announcement is documented in its September 2025 Newsroom release.
Bottom line
The most useful “hidden” capability is not a single sensor: it is the combination of alerts, trend views and records. Match the feature to your goal, verify the exact model, software and region, and treat every result as information to interpret—not a diagnosis delivered by your wrist.
