A single dose reduces Infection by 25%. You can loot them from Medicine Cabinets, Pop-N-Pills pill cases and Supply Crates, or make them yourself. The Chemistry Station is the efficient crafting route: one Old Sham Sandwich, one Nitrate Powder and one Water produce one Antibiotics. A Campfire can also make them, but it consumes four Old Sham Sandwiches and three Nitrate Powder and requires a Beaker.

Quick answer: the fastest way to cure Infection
If you already have an Antibiotics item, use it immediately from your inventory. Each dose reduces the Infection percentage by 25 points of the effect described by the item documentation. When no dose is available, search medical loot first, then check your stored ingredients and choose the Chemistry Station recipe if you have access to one. That recipe uses one Old Sham Sandwich, one Nitrate Powder and one Water per Antibiotics, making it far more economical than the Campfire recipe.

For minor Infection, Herbal Antibiotics or Honey may be enough. Their reductions are smaller, so regular Antibiotics are the better reserve for a higher Infection reading or a situation in which you cannot afford several weaker treatments.
How each treatment changes Infection
| Item | Infection reduction per dose | Best use |
|---|---|---|
| Antibiotics | 25% | Fastest standard reduction; save for serious or urgent cases when supplies are limited. |
| Herbal Antibiotics | 10% | A lower-strength alternative when regular Antibiotics are unavailable. |
| Honey | 5% | Small reductions when the Infection level is already low or stronger items must be conserved. |
The percentages are the item effects per dose, not a guarantee that one use removes every Infection state. Read the current Infection value in your character interface, use the item, and check the value again before deciding whether another treatment is necessary. The game may present several treatment choices at once, but their strengths are not interchangeable: choosing Honey when you need a large reduction can leave you with a persistent status effect and fewer resources for the next emergency.

Where to loot Antibiotics
Antibiotics are medical loot, so prioritize containers associated with healthcare instead of spending your first search on general household storage.
Medicine Cabinets
Medicine Cabinets are one of the named sources for Antibiotics. Check them in houses, bathrooms and other interiors as you clear a point of interest. Searching every cabinet before moving on is worthwhile when you need a dose immediately because the container category is directly associated with medical items.
Pop-N-Pills pill cases
Pop-N-Pills locations contain pill cases that can roll medical items, including Antibiotics. When you enter a Pop-N-Pills building, search the pill cases systematically rather than opening only the obvious front-room containers. Clear adjoining rooms and upper floors before leaving; a single missed case can be the difference between finding a dose and having to craft one.
Supply Crates
Supply Crates are another documented source. They are less predictable than a dedicated medical container because a crate can contain a broader set of supplies, but they should always be opened when your party is short on treatment.
Medical Supplies containers in points of interest
Dungeon-style point-of-interest loot rooms and other POIs can contain Medical Supplies containers. The container type and its placement are described in the Medical Supplies reference. Treat these rooms as high-value stops when planning a medical run, especially after you have identified a POI with a locked or final loot area.

A practical medical-loot route
- Start with the nearest Medicine Cabinets so you can obtain a treatment item without committing to a long trip.
- Move to a Pop-N-Pills location and search every pill case and medical container on the route.
- Open Supply Crates encountered during the same run; do not assume they contain only building or ammunition supplies.
- When a POI has a dungeon-style end room, search its Medical Supplies containers before exiting.
- Return to storage and sort all treatment items separately from food. This prevents accidentally consuming a weaker option when you intended to use regular Antibiotics.
Loot is probabilistic, so a trip can produce no Antibiotics. Keep the crafting ingredients below in a dedicated box; that way a failed medical run does not leave you starting from zero.
Crafting recipes
There are two documented workstations for Antibiotics. Both produce one item, but their ingredient costs differ substantially.
| Workstation | Ingredients for one Antibiotics | Extra requirement | Efficiency |
|---|---|---|---|
| Campfire | 4 Old Sham Sandwiches, 3 Nitrate Powder, 1 Water | A Beaker must be in the Campfire. | Consumes four times as many Old Sham Sandwiches and three times as much Nitrate Powder as the Chemistry Station recipe. |
| Chemistry Station | 1 Old Sham Sandwich, 1 Nitrate Powder, 1 Water | No Beaker is listed for this recipe. | More ingredient-efficient route according to the game’s recipe documentation. |
Making Antibiotics at a Campfire
- Place or use a Campfire.
- Verify that a Beaker is installed in the Campfire’s tool slot. Without it, the Antibiotics recipe will not be available as documented.
- Open the Campfire crafting interface and select Antibiotics.
- Load four Old Sham Sandwiches, three Nitrate Powder and one Water.
- Start the craft and wait for the item to finish before removing it from the output queue.
- Move the completed Antibiotics to your inventory or medical storage, then check the remaining ingredient counts.
The Campfire route is useful when you have a Beaker but no Chemistry Station, or when you need to turn a small collection of ingredients into a treatment without waiting to establish a chemistry setup. Its drawback is the Old Sham Sandwich cost: four sandwiches are consumed for one dose.
Making Antibiotics at a Chemistry Station
- Open a Chemistry Station that you can use.
- Select the Antibiotics recipe in the medical or consumables category.
- Add one Old Sham Sandwich, one Nitrate Powder and one Water.
- Start the craft and collect the completed dose from the output slot.
- Repeat only as needed, keeping enough Old Sham Sandwiches for future crafts if your supply is uncertain.
This is the preferred production method when both stations are available. It uses one quarter of the Campfire recipe’s sandwich requirement and one third of its Nitrate Powder requirement for each dose. Because Old Sham Sandwiches are the hardest ingredient to acquire, the savings become significant after the first craft.
Finding the ingredients
Old Sham Sandwiches
Old Sham Sandwiches are found in ordinary food containers, including refrigerators, microwaves and kitchen cupboards. The game’s documentation identifies them as the hardest Antibiotics ingredient to acquire. Plan searches around kitchens rather than medical buildings: homes, apartments, restaurants and similar interiors can contain the food-storage containers you need.

- Cooperate or Compete - Includes two player split screen mode, with support for up to 4 players online, in Player versus Player, co-op survival, or co-op creative modes
- Refrigerators: Search every refrigerator in a building before leaving. Kitchens often contain more than one container.
- Microwaves: Check countertop microwaves and appliances in compact kitchens; they are easy to overlook when moving quickly.
- Kitchen cupboards: Open cupboards systematically, including those in side rooms and small utility kitchens.
Do not discard Old Sham Sandwiches as useless food when your goal is medical crafting. Reserve them in a clearly labeled storage box until you have decided whether to make a dose. If your only option is the Campfire, count the sandwiches first: one craft requires four, while the Chemistry Station needs only one.
Nitrate Powder
Nitrate Powder comes from nitrate deposits in the Pine Forest, Burnt Forest, Desert and Wasteland biomes. Underground cave formations also provide it through stalactites and stalagmites. Mine deposits during normal resource trips and store the powder rather than waiting until an Infection emergency forces a dedicated expedition.
Biome access affects how quickly you can gather it. A player established in the Pine Forest can collect nitrate locally, while a base in another biome may need a longer surface run or an underground cave search. Mark productive deposits on your map and revisit them after the resource has regenerated according to your world’s rules.
Water
Both recipes require one Water per Antibiotics. Keep a supply available before starting a craft; otherwise the workstation may show the recipe but refuse to begin because the final ingredient is missing. The current Chemistry Station recipe uses Water, not Murky Water.
Beaker
A Beaker is specifically required for the Campfire recipe. It is not listed as a requirement for the Chemistry Station recipe. If you have all three ingredients but no Beaker, use a Chemistry Station instead of delaying treatment while searching for the tool.

Choosing between loot and crafting
| Situation | Best choice | Reason |
|---|---|---|
| You need treatment immediately and are near a medical POI. | Loot Medicine Cabinets, Pop-N-Pills pill cases and Medical Supplies containers. | Loot can provide a finished dose without consuming stored ingredients. |
| You have one Old Sham Sandwich, one Nitrate Powder and one Water plus a usable Chemistry Station. | Craft at the Chemistry Station. | This combination makes one dose with the lowest documented ingredient cost. |
| You have four Old Sham Sandwiches, three Nitrate Powder, one Water and a Beaker, but no Chemistry Station. | Craft at the Campfire. | The Campfire recipe works without waiting for the more efficient workstation. |
| You have only a small Infection level and no regular Antibiotics. | Consider Herbal Antibiotics or Honey. | The weaker items may be sufficient and preserve a stronger dose for a later emergency. |
| You have neither loot nor ingredients. | Run a combined medical-and-kitchen search. | Medical containers can provide a finished item while kitchens supply Old Sham Sandwiches for the next craft. |
A fastest-cure decision process
- Read the Infection status. Confirm that the status effect is still active and note its current percentage before consuming anything.
- Check your medical slot and inventory. Use regular Antibiotics if one is available and the Infection reduction you need is substantial.
- If none are available, check Herbal Antibiotics and Honey. Use a weaker option when the remaining Infection is small or when conserving regular Antibiotics is more important than speed.
- Check workstations. A Chemistry Station lets you craft with one of each listed ingredient. A Campfire requires the larger recipe and a Beaker.
- Search the closest medical containers. Prioritize Medicine Cabinets, Pop-N-Pills pill cases, Supply Crates and Medical Supplies rooms instead of random containers.
- Search kitchens for Old Sham Sandwiches. Refrigerators, microwaves and kitchen cupboards are the relevant targets.
- Gather Nitrate Powder during the same trip. Mine deposits in the Pine Forest, Burnt Forest, Desert or Wasteland, or harvest cave stalactites and stalagmites.
- Recheck the status after treatment. If Infection remains, choose the next item based on the remaining level and what you can replace safely.
Managing supplies for repeated infections
Repeated exposure makes inventory planning more important than a single emergency craft. Keep finished Antibiotics in a dedicated medical container, separate from cooking ingredients. Store Old Sham Sandwiches separately from ordinary food so they are not consumed accidentally. Label a second box for Nitrate Powder and Water, and record whether a Beaker is available for the Campfire fallback.
When you return from a scavenging trip, process ingredients in this order:
- Count finished Antibiotics, Herbal Antibiotics and Honey.
- Count Old Sham Sandwiches and reserve enough for at least one Chemistry Station craft if possible.
- Check Nitrate Powder and Water totals.
- Choose the Chemistry Station for routine production and keep the Campfire as an emergency alternative only when its larger ingredient cost is acceptable.
- Replace any item used during the trip before starting another expedition.
This approach prevents a common failure mode: having plenty of Nitrate Powder but no Old Sham Sandwiches, or having the sandwiches but no Water at the moment a dose is needed.
Why older guides show different recipes
Recipe information changed between Alpha versions. Alpha 20 replaced Moldy Bread with Old Sham Sandwich in the Antibiotics recipe. Alpha 21 replaced Murky Water with Water in the Chemistry Station recipe. A guide that still lists Moldy Bread or Murky Water is describing an older ruleset, not the current recipe shown in the documented item history.
| Older guide wording | Current documented wording | What to do |
|---|---|---|
| Moldy Bread | Old Sham Sandwich | Search refrigerators, microwaves and kitchen cupboards for Old Sham Sandwiches. |
| Murky Water in the Chemistry Station recipe | Water | Keep Water available when crafting at the Chemistry Station. |
World version, platform build and modded recipes can change what appears in your crafting interface. If the displayed recipe differs from the values here, trust the ingredients shown in your current game and check whether the world is running an older Alpha or a mod.

Troubleshooting
The Antibiotics recipe is missing from the Campfire
- Confirm that the Campfire has a Beaker installed.
- Verify that you are viewing the Antibiotics recipe rather than a similarly named medical item.
- Check that the required Old Sham Sandwiches, Nitrate Powder and Water are in the workstation inventory.
- If the recipe still does not appear, compare your game version with the current recipe history; older or modded versions may use different inputs.
The Chemistry Station will not start the craft
- Check all three inputs: one Old Sham Sandwich, one Nitrate Powder and one Water for each dose.
- Make sure the ingredients are in the Chemistry Station’s inventory, not only in your character inventory if the interface requires workstation storage.
- Confirm that the station is available for use and not occupied by another queued craft.
- Look at the displayed recipe text for a version or mod mismatch before substituting Murky Water or Moldy Bread from an old guide.
You cannot find Old Sham Sandwiches
Stop searching medical containers for this ingredient. Concentrate on refrigerators, microwaves and kitchen cupboards, and search several residential or food-service interiors in one route. Because this is the bottleneck ingredient, use the Chemistry Station recipe whenever possible.
You have Nitrate Powder but no treatment
Search for Old Sham Sandwiches and Water, then craft at a Chemistry Station if available. If only a Campfire is accessible, do not start until you have four sandwiches, three Nitrate Powder and one Water plus a Beaker; otherwise the craft cannot be completed with that route.

Herbal Antibiotics and Honey provide smaller reductions than regular Antibiotics. Check the remaining Infection value rather than assuming the first item failed, then use another appropriate treatment or obtain regular Antibiotics from medical loot or crafting.
Practical examples
Example: one dose needed in a new settlement
You have one Old Sham Sandwich, one Nitrate Powder and one Water, and your settlement already has a Chemistry Station. Craft one Antibiotics there. Searching for a Beaker would only delay treatment because the efficient recipe does not list one.
Example: a Beaker but no Chemistry Station
Your storage contains four Old Sham Sandwiches, three Nitrate Powder, one Water and a Beaker. Use the Campfire recipe to make one dose. When you later gain access to a Chemistry Station, switch to the one-sandwich recipe to preserve your remaining food ingredients.
Example: low Infection and no regular Antibiotics
You have no regular dose, but you do have Herbal Antibiotics and Honey. Choose the weaker item that matches the remaining Infection and keep searching for medical loot. This avoids spending a rare regular dose when a smaller reduction is enough.
