A Salesforce Flow can fail visibly, never run because its entry criteria are false, or help trigger a transaction-wide rollback after a limit is exceeded. Those are different problems, and the fix depends on which one occurred. Salesforce does not publish an official ranked list of five Flow mistakes; the five below are an editorial synthesis of its troubleshooting, testing, limits, and best-practice guidance.
1. Activating a flow without testing every meaningful path
A flow that works for one representative record may still fail on another decision outcome, an unexpected value, or a boundary condition. Testing only the happy path leaves those failures for production users to discover.
Before activation, exercise each decision outcome, including the default outcome, and test edge values and error behavior. Test with the relevant user contexts too: a flow can behave differently when a user lacks access to a record or field. Salesforce recommends testing in a sandbox so experiments do not accidentally change real records. See Salesforce’s guidance on testing a flow before activation.
Salesforce’s current guidance also describes Test Mode for some autolaunched and record-triggered flow scenarios. Availability and beta status can vary by org and release, so verify the current documentation and the feature in your org before relying on it. 2. Leaving important elements without useful fault handling
When an element fails, a fault connector can route the flow to a useful error message, notification, or recovery action. Without one, users and administrators may have little more than a failed run to investigate. Fault handling is especially worth reviewing around elements that read or change records and around actions whose inputs may be missing or invalid.
Start with the evidence from the failed run rather than guessing. Salesforce error emails can identify the flow name and version, the element, and the error message. For complex flows or Apex actions, a stack trace may help. Then inspect the element’s required inputs, its fault connector, and the values passed into it. A fault path is not a guarantee that a transaction will commit. If the transaction breaches a governor limit, Salesforce says, “The transaction rolls back even if the element has a defined fault connector path.” That is a transaction-level rollback, not simply an unhandled element error. 3. Confusing permissions or entry criteria with an inconsistent flow

- The flow ran but failed: The running user may not have the required access to a record, object, or field, or an element may be missing a required input.
- The flow never ran: The record did not meet the flow’s configured start criteria, or its actual values differed from what you expected when the triggering event occurred.
Check both sides: inspect the record’s values at the time of the triggering change, and verify the relevant user’s access. Use Flow Builder’s debugger to follow the path and examine values in context. Salesforce’s troubleshooting guidance addresses permissions and entry criteria, while its testing guidance recommends testing permissions as part of pre-activation checks.
4. Performing database work inside loops
Repeated record updates or other database operations inside a loop can create duplicate work and increase processing cost. Salesforce’s Flow Builder best-practice guidance cautions against edits in a loop path because they can result in duplicate database changes. Review the loop’s path, identify which records need changing, and organize database work deliberately rather than performing a write on every iteration when it is avoidable.

When the symptom is repeated changes or slow execution, inspect the flow’s element activity and Apex debug logs. Salesforce identifies log events that show Flow interviews and per-element CPU consumption. These can help locate repeated work and determine whether the flow is contributing to CPU use. Flows share a transaction with other automation, including Apex triggers. A governor-limit breach can therefore reflect cumulative work in the transaction rather than one isolated flow. When investigating a rollback or CPU problem, inspect the other automation that ran in the same transaction as well as the flow’s own elements. A fault connector cannot override a transaction-wide limit breach.

Order can matter too. Salesforce permits ordering for record-triggered flows only within specified constraints; it is not a universal way to arrange every flow in an org. Check the active flows for the object, their trigger types, and their configured order values before attributing an unexpected result to one flow. Consult Salesforce’s rules for defining the run order of record-triggered flows.
One specific Salesforce Help article, published June 19, 2026, describes a maximum of 12 duplicate updates in one batch in a scenario involving multiple duplicate scheduled actions or waiting interviews on the same record. That is a scenario-specific error, not a general Flow reliability statistic or a universal limit on Flow use. Its details and workarounds are in Salesforce’s duplicate-updates error article.
A practical troubleshooting sequence
- Identify the failed run. Read the Salesforce error email for the flow name and version, failed element, and message. For complex flows or Apex actions, review the stack trace if available.
- Reproduce and inspect the path. Use Flow Builder’s debugger to see which path ran and what values the flow evaluated.
- Check inputs and access. Confirm required element inputs are present and verify the running user’s relevant permissions.
- Confirm the start conditions. Compare the record’s actual values at the triggering event with the record-triggered flow’s entry criteria.
- Review fault handling and transaction evidence. Inspect the failed element’s fault connector, then review logs and related automation for CPU use, repeated writes, or a transaction-wide limit breach.
- Test the fix safely. In a sandbox, test the affected path, its alternatives, boundary values, error handling, and relevant permission contexts before activation.
