You can feel completely well and still have high LDL cholesterol. High cholesterol often causes no symptoms, so how you feel cannot tell you whether your LDL is elevated; a lipid panel is the way to measure blood cholesterol. A healthcare professional can help decide when to test and what the results mean for your individual risk.
Myth 1: If I feel fine, my LDL must be fine
High cholesterol often has no symptoms. The American Heart Association says, “Many people do not have symptoms.” That means feeling healthy—or having no obvious warning signs—is not evidence that your LDL is at a safe level. A blood test, typically a lipid panel, measures cholesterol levels. The AHA’s cholesterol guidance explains why testing matters.
This does not mean that a high LDL level predicts an immediate heart attack or stroke. It is one factor in cardiovascular risk, and its significance depends on the broader clinical picture.
Myth 2: If my clinician hasn’t brought it up, I don’t need to ask
It is reasonable to ask a healthcare professional whether you should have a lipid panel and when to repeat it. The right schedule depends on your risk factors, health history, and previous results; there is no single interval in the guidance that fits everyone. The AHA encourages adults to know their cholesterol levels and discuss them with a healthcare professional. Its cholesterol-myth guidance also challenges the assumption that not hearing about LDL means there is nothing to check.

When you ask, mention any prior cholesterol results, family history of early cardiovascular disease or inherited cholesterol conditions, diabetes, and any history of heart attack or stroke. Those details can affect both testing and follow-up.
Myth 3: Only weight, diet, or exercise determines LDL
Diet and physical activity matter, but they do not explain every person’s cholesterol level. Age, overall health, family history, diabetes, previous heart attack or stroke, and inherited conditions can also affect risk and treatment decisions. A person’s body size alone does not establish whether their LDL is high.

That is why cholesterol results are interpreted alongside a person’s health and family history, not as a judgment about how they look or whether they have followed a particular lifestyle.
Myth 4: Cholesterol is only a concern for men or older adults
Cholesterol is relevant across groups and life stages. The AHA says both men and women need to pay attention to cholesterol; levels tend to rise with age, and some life stages can affect women’s levels. These broad observations do not predict an individual’s results, so testing and follow-up should reflect personal circumstances rather than assumptions based on sex or age. Myth 5: Everyone needs the same diet—or the same medication decision
There is no one-size-fits-all response to an LDL result. The 2026 AHA/ACC dyslipidemia guideline replaces the 2018 blood cholesterol guideline and emphasizes personalized assessment. For eligible adults, it promotes the PREVENT-ASCVD equations to help estimate risk; it also highlights once-in-a-lifetime testing for lipoprotein(a), or Lp(a), selective measurement of apolipoprotein B, or ApoB, and coronary artery calcium (CAC) imaging in appropriate cases. These tools are not a checklist that every person needs. The AHA’s clinical practice guideline page and its 2026 dyslipidemia guideline summary describe the update; the summary says recommendations incorporate clinical evidence through late 2024.
In a treatment discussion, a clinician may consider established cardiovascular disease, age, LDL and non-HDL cholesterol, family and medical history, and overall risk—not only one LDL number. The 2026 guideline restores LDL and non-HDL treatment goals, but an appropriate goal and whether medication is warranted depend on the person’s circumstances. Ask how your results fit your risk profile rather than deciding from a number in isolation.

What a heart-healthy eating pattern looks like
AHA dietary guidance published March 31, 2026, emphasizes an overall pattern: vegetables and fruit, whole grains, healthy protein sources, unsaturated fats in place of saturated fats, and minimally processed foods, while limiting added sugars and sodium. For most people, dietary cholesterol is no longer treated as the primary target for cardiovascular risk reduction. That is not a claim that diet is irrelevant; it is a reason to focus on the whole pattern and get individual advice when needed. The AHA’s dietary cholesterol guidance provides more detail.

- Ask about testing. Contact a healthcare professional to discuss whether a lipid panel is appropriate and when it should be done, based on your age, health, family history, and prior results.
- Review the whole result. Ask what your LDL and non-HDL levels mean in combination with your other health information, rather than treating one value as a complete risk assessment.
- Discuss next steps together. Depending on your situation, the conversation may cover lifestyle, follow-up testing, additional risk assessment, or medication. You do not need to assume that every additional test or treatment applies to you.
The AHA reported in its 2026 guideline announcement that 1 in 4 U.S. adults has high LDL-C. That estimate is a population-level figure, not a way to predict any one reader’s result. The AHA announcement accompanies the updated guideline.
