The biggest new Apple Watch health changes are more frequent heart-rate and HRV sensing plus a daily readiness score on Apple Watch Series 12 and Ultra 4. watchOS 26 also adds sleep score to several older models and hypertension notifications to a narrower set of watches. Eligibility depends on the watch, paired iPhone, age, health history and—in some cases—region.
1. More frequent heart-rate sensing on Series 12 and Ultra 4
Apple says the Series 12 and Ultra 4 use new optical and electrical heart sensors. Their optical sensor measures heart rate every five seconds throughout the day, which Apple says provides more detail for understanding activity and physiological state. Apple also calls Series 12’s heart-rate sensing the most accurate in a wearable, citing its own study of more than 1,000 participants comparing the watch with leading wearables. That is Apple’s comparative claim, not an independent consensus. Apple’s Series 12 announcement

2. More frequent HRV readings and richer Vitals
On the new sensing system, Apple says heart rate variability (HRV) can be measured as often as every five minutes—24 times more frequently than before. HRV is a measure of variation in the time between heartbeats; it is not a stand-alone verdict on health.

The Heart Rate app separates Recovery HRV, which Apple frames as a signal for daily stress and recovery, from overall HRV, which it presents as a broader health measure. Apple says higher HRV is generally associated with better overall recovery, but an individual reading should be understood in personal context rather than treated as a diagnosis.

Overnight and daytime Vitals
Overnight Vitals adds Recovery HRV in relation to the wearer’s personal baseline, and a new daytime view lets users inspect metrics during the day. Apple says Vitals is for wellness purposes, not medical use. These views can help track patterns; they cannot diagnose a condition. Apple’s health and fitness announcement
3. Readiness score for planning the day
Series 12 and Ultra 4 introduce a daily readiness score from 0 to 10, based on recent activity, vitals and sleep score. The watch presents one of four suggestions: “Recover,” “Pace Yourself,” “Ready” or “Go For It.” Apple says the score can change as new data arrives during the day and identifies factors contributing to the result.

Think of readiness as a planning prompt for a workday or training routine—not a clinical assessment, a command to exercise or rest, or a guarantee of performance. Apple says the algorithm drew on Apple Heart and Movement Study data and collaboration with exercise scientists and physicians at Apple. Apple’s description of readiness
4. Sleep score reaches supported older watches
Sleep score is a watchOS 26 feature, not a Series 12-only upgrade. Apple says the score considers sleep duration, bedtime consistency, wake-ups and time spent in sleep stages. The Sleep app shows an overall score and breakdown; sleep-score history is available in the iPhone Health app.

Compatibility
- Apple Watch Series 6 or later
- Apple Watch SE (2nd generation) or later
- All Apple Watch Ultra models
The watch must be paired with an iPhone 11 or later running iOS 26. A supported watch therefore does not receive the feature independently of its phone and software requirements. Apple’s watchOS 26 sleep-score announcement

5. Hypertension notifications look for patterns, not blood pressure
Hypertension notifications use optical heart-sensor data to look for patterns over 30-day periods that may indicate chronic high blood pressure. They do not take a cuff-based blood-pressure reading. Apple warns the feature will not detect every instance, and a notification does not confirm hypertension.
Eligible models and intended use
Apple lists the feature for Apple Watch Series 9 or later and Apple Watch Ultra 2 or later with watchOS 26. Availability also depends on regional support and regulatory status; check Apple’s current hypertension-notification support page for your location. Apple’s 2025 launch note says the feature is not intended for people under 22, people already diagnosed with hypertension, or use during pregnancy. Apple’s 2025 announcement

What to do after a notification
Apple recommends recording blood pressure with a third-party cuff for seven days and sharing the results with a healthcare provider. The watch is not a blood-pressure monitor, and the notification is a reason to follow up rather than a diagnosis.
Apple says approximately 1.3 billion adults globally are affected by hypertension. It also reports that its feature work involved more than 100,000 participants across training studies and more than 2,000 participants in a validation study. These are figures reported by Apple in its 2025 announcement. Apple’s hypertension feature announcement
Which upgrade requires a new watch?
The two Series 12 and Ultra 4-specific changes in this list are the new sensing system and readiness. Sleep score is available on several earlier watches, and hypertension notifications cover Series 9 and Ultra 2 generations and later, subject to software and regional availability.

Related features that are easy to confuse with the new upgrades
Sleep apnea notifications are not a sleep-apnea diagnosis
Apple Watch sleep apnea notifications analyze wrist accelerometer data to estimate Breathing Disturbances. A notification depends on elevated values over a 30-day period and is intended to identify signs of possible moderate-to-severe sleep apnea in people without an existing diagnosis. Apple explicitly says Breathing Disturbances are not equivalent to the clinical apnea-hypopnea index (AHI); treat a notification as a prompt to discuss clinical evaluation, not as a diagnosis. Apple lists Series 9 or later, Ultra 2 or later, and SE 3, with intended use for people 18 or older without a diagnosis. Apple’s sleep apnea notification information
Blood Oxygen has a U.S.-specific configuration history
Apple’s August 14, 2025 notice said some U.S. Series 9, Series 10 and Ultra 2 units without the feature would receive a redesigned version through iOS 18.6.1 and watchOS 11.6.1, with measurement calculation on iPhone. Apple said units previously purchased with the original feature and units purchased outside the U.S. were unaffected. This does not describe every U.S. watch; check the model and configuration rather than assuming Blood Oxygen behaves identically across devices. Apple’s U.S. Blood Oxygen update
