A 2026 study identified 40 outbreak cases from May 2019 through June 2026. Control measures lowered the reported incidence, but investigators said the strain remained in the water system.
What happened at the hospital?
Three months after the specialist hospital moved into a new building, two cases among lung transplant recipients prompted an investigation. The study, published online on 29 September 2026, describes cases identified between May 2019 and June 2026. The authors defined an outbreak case as a patient with a positive culture whose isolate matched the strain found in water samples by whole-genome sequencing (WGS).

The hospital’s water system was associated with the outbreak strain, which persisted and spread through the system. The study supports a link between the water and patient infections; it does not establish one exposure route for every patient. The study in Clinical Infectious Diseases and its PubMed record summarize the investigation.
Who was affected?
| Patient group or outcome | Cases reported |
|---|---|
| Lung transplant recipients | 11 |
| Patients with cystic fibrosis | 11 |
| Patients with non-CF bronchiectasis | 14 |
| Other patient groups | 4 |
| Diagnosed with nontuberculous mycobacterial pulmonary disease | 19 |
The 19 pulmonary-disease diagnoses are a subset of the 40 outbreak cases, not a description of every case. The study’s counts are specific to this hospital outbreak, not estimates of risk for other hospitals or patients.

How did investigators link cases to the water?
The investigation combined three kinds of evidence:
- Clinical cultures: Patient samples established which individuals had a positive culture.
- Environmental sampling: Investigators sampled the hospital water system from 2019 to 2025.
- Whole-genome sequencing: WGS was used to compare clinical and water isolates and identify matches under the study’s outbreak-case definition.
The authors also conducted a case-control study among lung transplant recipients. It found no specific risk factors associated with lung transplant surgery or bronchoscopy. That finding is limited to the factors examined in that analysis; it does not show that no risks existed or identify the exposure route for each patient.

Did the control measures stop the outbreak?
No. The authors report that extensive control measures—including point-of-use filters, biocide treatment and enhanced flushing—were followed by a decline in incidence from 12.8 to 2.2 cases per year. The abstract reports these measures collectively and does not give separate effect sizes, so it cannot establish how much each measure contributed.
No further case had been identified since June 2025, coinciding with a review of enhanced flushing, according to the study’s status at publication. That is not a guarantee about surveillance after publication. The authors’ conclusion is that “Control measures reduced the incidence of infection but did not eliminate the outbreak strain from the hospital water.”

What the findings do—and do not—establish
- Established by the study: Patient and water isolates were linked by WGS, and reported incidence fell after a package of control measures.
- Not established in the abstract: A single route of exposure for every patient, the independent effectiveness of each measure, or detailed clinical outcomes for all 40 cases.
- Scope: The findings concern one specialist hospital in England. The study’s observation period ran through June 2026; its no-new-case status since June 2025 reflects what had been identified by publication.
