Before signing or submitting it, confirm that it is the current form for the state handling the claim, check every factual entry against original records, and follow that form’s instructions for signatures, recipients, attachments, and filing.
1. Identify the state and the form’s purpose
Start by determining which state agency handles the claim and what the document is meant to do. Workers’ compensation procedures are jurisdiction-specific: California provides an injured worker’s claim form; Georgia uses Form WC-14; Massachusetts describes a route for an unrepresented worker to file Form 110; and Pennsylvania distinguishes among forms and filing methods. Do not assume a form number, recipient, or process applies nationwide.
Use the relevant agency’s instructions for the particular form. If you are unsure which state process applies, ask the agency or a qualified representative before filing.
2. Match the draft to the current official form
Open the state agency’s official form page and compare the AI draft with the form’s title, number, revision, and instructions. Do not rely on an AI-generated form merely because its layout looks familiar. Iowa warns that using the wrong agency form for certain filings may result in rejection. New York periodically revises forms and may announce that older versions will no longer be accepted.

Use the current instructions for the specific form to establish what information is required and who must complete each section. The examples below illustrate why checking the state-specific process matters; they are not interchangeable rules.

3. Check every factual entry against original records
Compare the draft with reliable records, such as employer documents, insurance correspondence, and the worker’s own records. Do not validate an AI-generated entry by checking it against another AI answer or by accepting a plausible-sounding completion.

Pennsylvania’s Department of Labor & Industry specifically advises filers to “double-check the accuracy of the information provided, such as employee name, address, and Social Security Number, employer name and address, insurance information, injury date, and description of injury.” See its claim forms guidance. This is a Pennsylvania instruction, but the listed fields are useful prompts for reviewing a draft elsewhere.
- Worker’s legal name and contact details
- Employer’s name and address
- Carrier or insurance details
- Injury date and factual description of the injury
- Claim identifiers, entered in the field intended for that identifier
Check claim numbers especially carefully. Pennsylvania explains that its bureau claim number differs from the carrier’s claim number; its guidance directs users to enter the carrier number only where the form asks for it. See Pennsylvania’s claim information page. Follow the relevant form’s labels rather than moving a number into a field because an AI draft suggests it.

Do not let the draft supply missing facts
If a required fact is unknown or disputed, do not invent, infer, or alter it to make the form look complete. Follow the form instructions or ask the responsible agency or a qualified representative how to proceed. Correct an AI-generated statement only when you can support the correction with reliable information.
4. Confirm who completes each section and signs
Check the form instructions to determine which sections belong to the worker, employer, insurer, or representative. A completed-looking form may still be wrong if the wrong person filled in a section. California’s injured-worker guidance, for example, says the worker completes only the employee section of the claim form. That is a California-specific direction, not a general rule for every state.

Verify whether the particular form requires a signature and date, who must provide them, and whether an electronic signature is accepted. New York’s claimant guidance says the Board does not normally accept a claimant’s electronic signature on Board-prescribed forms; check the current form and any applicable exception rather than assuming the rule applies elsewhere.

Before sending the form, use the state’s instructions to confirm where it must go, who must receive copies, what supporting documents are required, and whether the permitted method is electronic, paper, or both. These details differ by form and jurisdiction.

For example, California tells workers to sign and date the claim form, return it promptly to the employer, and keep a copy; it suggests certified mail when mailing so the mailing and receipt dates can be documented. Georgia’s instructions say to file WC-14 with the Board and send a copy to the employer and insurer. Massachusetts’s Form 110 self-filing instructions describe copies and supporting documents for that route. Follow the instructions for the claim you are filing, not another state’s example.
Keep a completed copy and whatever submission confirmation or mailing proof the applicable process provides. The point is to retain a record of what was sent and when, using the method the agency specifies.
6. Inspect the saved or printed file before submission
Review the actual file you plan to submit, not just the form while it is open in an editor. New York’s Board explains that entered data may not transmit unless the user prints to PDF and saves the completed version, and provides instructions for opening a saved form. After saving or printing, open the final PDF and check that the entries and all pages are visible and legible. Iowa’s separate instructions to complete forms using Adobe Acrobat and flatten certain completed PDFs before e-filing apply to Iowa forms only.
- Save or print the completed form using the method required for that form.
- Open the saved file and inspect every page, including any continuation pages.
- Confirm that entries are present, readable, and in the intended fields.
- Check the final file for required signatures, dates, and attachments before sending it.
7. Get human help when the issue is legal or unresolved
Do not treat an AI interpretation as authority to waive a right, change a disputed fact, or disregard a form instruction. The official sources cited here do not establish a general AI-specific legal standard for reviewing workers’ compensation forms. If the instructions do not resolve a question, contact the responsible agency or a qualified representative before filing.
