Reviewers may lack the author’s context, face an oversized patch, focus on visible style issues, miss behavioral edge cases, trust weak tests, or lack expertise in areas such as security and concurrency. Better review practices can reduce these risks, but no review checklist or approval gate guarantees defect-free code.
Why code review cannot guarantee a bug-free change
A reviewer sees a change at a particular point in time, often without the author’s full understanding of the surrounding system or the assumptions behind the implementation. Even a careful reader can miss a defect that appears only with an unusual input, a particular sequence of events, or an interaction elsewhere in the product.
Evidence about review effectiveness does not establish a universal bug escape rate. A 2018 Google case study combined 12 interviews, a survey with 44 respondents, and review-log analysis of 9 million changes; those figures describe the study’s methods and scale, not the percentage of bugs that code review misses. Google Research’s case study is about review practice in one organization, not a universal defect-detection benchmark.
Common reasons bugs slip through
The reviewer does not have the author’s context
The author has spent time with the change; the reviewer may see only the diff and a short description. A line can look reasonable in isolation and still break a workflow or violate an assumption elsewhere in the module. Google’s review guidance recommends looking beyond assigned lines at the broader file and system context, and asking for clarification when the code is difficult to understand.

Large changes overload attention
As a patch grows, it becomes harder to hold its purpose, interactions, and potential failure paths in mind. Google’s guidance for authors preparing changes warns that large reviews can produce enough back-and-forth that important points get missed or dropped. This is practical guidance, not a controlled estimate of how much a large change raises defect risk.
Visible polish can displace behavioral review
Naming and formatting are easy to spot. A broken state transition, boundary condition, or ordering assumption may be much less visible, particularly when the failure depends on code outside the diff. Google recommends prioritizing design and functionality, considering the user’s perspective, and avoiding blocks based only on personal style preferences in its review standards.
Tests are present but do not challenge the likely failure
A test suite may exercise the happy path while leaving the broken behavior untouched. Tests can also pass for the wrong reason—for example, if assertions do not check the important outcome or if a test can produce a false positive. Google’s review guidance says reviewers should consider whether tests would fail if the implementation were broken. Tests themselves need human review.

Concurrency and specialist risks are hard to spot
Race conditions and deadlocks can be difficult to uncover by simply running a program. Privacy, security, accessibility, and other specialized concerns may likewise require knowledge a general reviewer does not have. Google advises seeking qualified review for complex topics such as concurrency, privacy, and security in its review guidance.

A 2023 study examined 20,995 keyword-selected review comments from OpenStack and Qt; researchers classified 614 as security-related. They reported that security defects were not prevalent in review discussions and that “Not worth fixing the defect now” and disagreement between developer and reviewer were common reasons security defects were not resolved. The study concerns selected projects and comments, not security review effectiveness everywhere. The study’s published paper describes its scope.
In a separate 2022 online experiment with 150 participants, an explicit prompt to focus on security increased the probability of vulnerability detection eightfold in that experiment. The tested security checklist did not significantly improve the result further. That finding is specific to the study’s participants and setup; it is not a guaranteed production effect. How to make reviews more likely to catch defects
1. Keep a change small and self-contained
Split work into changes that can be understood and assessed independently when the task permits. Include related tests and enough context to explain how the pieces fit. Smaller changes make it easier to reason about impact; they do not eliminate the need to inspect surrounding behavior.
2. Explain intent and risk in the review description
State what the change is meant to do, who or what it affects, important assumptions, and the behaviors most likely to fail. This gives the reviewer something concrete to challenge rather than asking them to infer purpose from implementation details.
3. Read for behavior, not only for the diff’s appearance
Review every assigned human-written line, then inspect relevant surrounding code and system behavior. Consider the change from the user’s perspective and ask whether it behaves correctly at boundaries and during failure. Google’s review guidance specifically calls for attention to edge cases, concurrency, and user behavior.

- Inputs: What happens with empty, malformed, missing, or unusually large values?
- State: Are transitions and persisted values correct on both success and failure?
- Permissions: Can the wrong user or component reach the new behavior?
- Ordering and concurrency: Could interleaving, retries, or duplicate operations change the outcome?
- User-visible results: Do errors, loading states, and recovery paths make sense?
4. Challenge the tests themselves
Do not treat “tests added” or a green test run as proof. Ask whether a test would fail if the likely defect were present, whether its assertions check the meaningful result, and whether a change could make it pass incorrectly. A 2023 mutation-testing study analyzed 633 merge requests and 78,000 mutants; code changes or test additions resolved 38% of all mutants and 60% of productive mutants in that dataset. Those are mutant outcomes, not rates of escaped production bugs. The study examines how developers responded to mutants.
5. Match reviewers to the risk
Use reviewers with relevant expertise when a change raises security, privacy, concurrency, accessibility, or other specialist questions. One reviewer need not be an expert in every dimension; the important point is to identify specialized risks instead of assuming a general approval covers them.
6. Use automation as another layer, not a substitute
Automated tests and static analysis can flag classes of problems, but they do not replace understanding the change, its assumptions, and its surrounding behavior. The security-review study recommends combining manual review with automated detection for broader coverage. No tool or approval rule should be treated as proof that every defect has been caught.

7. Balance speed with code health
Time pressure can lead teams to accept shortcuts, while demanding perfection from every change can stall useful work. Google’s review standards recognize both considerations: make sound decisions about quality and progress rather than treating every preference as a blocker.

An approval means the reviewers judged the change acceptable under the context, time, and expertise available. It does not certify that every path was explored or that the code is defect-free. A Microsoft Research paper titled “Code Reviews Do Not Find Bugs. How the Current Code Review Best Practice Slows Us Down” argues for more precise systematization of review practice; its title is the authors’ argument, not a universal finding that review never catches bugs.
For a team, the useful question after a missed defect is not simply “Who approved this?” Ask what made the failure difficult to see: Was the patch too broad? Was intent unclear? Did tests miss the behavior? Was specialist review needed? The answer points to a process improvement without pretending that any process can remove uncertainty from human review.
