For Week 4, NFL.com’s QB Index puts Brock Purdy first, Patrick Mahomes second and Dak Prescott third. NFL Fantasy’s start/sit advice is more lineup-focused: it names Tyler Shough its Start of the Week, recommends Purdy and Bryce Young, and makes Justin Herbert its Sit of the Week. These are different editorial lenses, not one agreed fantasy projection. Check your league scoring and each player’s official status before kickoff.

Week 4 quarterback rankings: the top tier
NFL.com’s September 30 QB Index ranks these three quarterbacks at the top. Its order is a football-player ranking, not a universal fantasy-points forecast; the page says its arrows show movement from the Week 3 QB Index.

Who should you start at quarterback in Week 4?
Consider Tyler Shough as a strong start
NFL Fantasy names Shough its Start of the Week, reporting that he scored at least 22 fantasy points in each of his first three games. Its article cites his early volume and upcoming opponent in making the recommendation. That is a source-specific endorsement, not a guarantee or a format-neutral projection; use it alongside your league’s scoring and matchup context.
Purdy and Bryce Young are also endorsed starts
NFL Fantasy calls Purdy and Young notable starts. The split between this advice and the NFL.com index is useful: the index orders quarterbacks as football players, while start/sit advice tries to help managers make a weekly lineup decision. The two sources do not establish a single exact fantasy ranking.

The same article lists Matthew Stafford and Daniel Jones as starts, C.J. Stroud and Geno Smith as deeper streaming options, and Marcus Mariota as a deeper emergency stream. These are the article’s recommendations, not guarantees. Check whether a deeper option fits your roster and league before choosing him over a more established starter.

NFL Fantasy selects Herbert as its Sit of the Week. Treat that as a caution from one editorial source, not a universal instruction: in a close decision, compare his expected volume and matchup with your alternatives, then confirm he is available and that his game has not begun.

Use matchup resources as a cross-check
Pro-Football-Reference’s Week 4 quarterback matchup page describes its matchup ranks as using playing percentages, fantasy points and opponents’ fantasy points allowed. It offers a matchup lens, not a substitute for checking role, scoring rules and availability.
NBC Sports’ October 1 rankings description highlights Purdy despite receiver issues, Jordan Love in a favorable road matchup at Tampa Bay, and Shough as a quarterback seeking to establish himself in the top 12. That description is a useful editorial cross-check, but it is not a complete ranking list.

How to settle a close Week 4 start/sit call
- Check availability first. Confirm each quarterback’s official game status and whether kickoff has passed. The NFL injury report excerpt available for this update includes entries dated September 17 and 20, so it does not establish every player’s final Week 4 status. Verify the relevant team’s current official entry before locking a lineup.
- Compare expected role and recent production. Look at passing volume, touchdowns, interceptions and whether the player has a stable share of the offense. NFL.com’s three-game figures above are recent production, not a forecast.
- Account for rushing and your scoring rules. Rushing yards and touchdowns can materially affect quarterback value, depending on league settings. Standard, half-PPR and PPR labels mainly describe reception scoring for other positions; for quarterbacks, rushing and touchdown values, interceptions, passing-yard thresholds and bonuses are often more consequential. Superflex or two-quarterback rosters also change replacement value because more quarterbacks must be started.
- Weigh opponent and game environment. Use matchup data as one input, not a verdict. Consider likely volume and game script alongside the opponent metric.
- Cross-check projections without inventing precision. FantasyPros’ consensus Week 4 quarterback projections were last updated September 29, 2026, but the available result does not supply individual projection figures to reproduce. Check the live page for numbers rather than inferring them from rankings.
What these Week 4 rankings do—and do not—tell you
The evidence supports a clear top three in NFL.com’s index and several specific start/sit recommendations from NFL Fantasy, but it does not provide a complete, scoring-specific fantasy ranking with comparable projected points for every quarterback. For a standard one-quarterback league, use the recommendations as a shortlist and make close calls with matchup, rushing contribution and verified availability. In superflex or two-quarterback formats, account for positional scarcity instead of applying a one-quarterback start/sit list unchanged.
