Semaglutide and tirzepatide are the focus of much of the attention, but trial averages are not promises for individuals, and the evidence does not establish who should take either medicine.
Why are clinicians paying attention to these medicines?
Semaglutide and tirzepatide belong to a newer wave of medicines that act on gut-hormone pathways involved in appetite and metabolism. The results reported in major obesity trials were markedly greater than the placebo-group results, helping explain the intense clinical interest. That does not mean every patient will lose the same amount, or that weight loss automatically prevents every condition associated with obesity.
Semaglutide acts on the GLP-1 receptor
Semaglutide is a GLP-1 receptor agonist. The FDA’s 2026 Wegovy prescribing information includes use alongside reduced-calorie eating and increased physical activity for long-term weight reduction and maintenance in specified patients. The label also includes certain cardiovascular-risk and metabolic dysfunction-associated steatohepatitis (MASH) indications. Those are distinct, product-specific indications, not a general approval for every patient or every obesity-related condition.
Tirzepatide acts on two receptor pathways
Tirzepatide acts through GIP and GLP-1 receptor pathways. In the SURMOUNT-1 obesity trial, it was studied in adults without type 2 diabetes. Its trial results are one reason newer medicines have drawn attention, but a result from that trial does not by itself determine which drug is appropriate for a particular person.

How much weight did participants lose in the trials?
The following are group averages from separate placebo-controlled trials, measured at different follow-up points. They are not predictions for an individual.

These figures should not be treated as a direct contest between semaglutide and tirzepatide: STEP 1 and SURMOUNT-1 were different trials, not a randomized head-to-head comparison. SURMOUNT-5 did directly compare maximum tolerated weekly doses of tirzepatide (10 or 15 mg) and semaglutide (1.7 or 2.4 mg) over 72 weeks in adults with obesity without type 2 diabetes. It used an open-label controlled design. The cited trial information establishes that design, but does not provide numerical outcome results to report here.

What is the difference between semaglutide and tirzepatide?
The main distinctions established here are their receptor pathways and the trial evidence above. A treatment choice also depends on a product’s labeled indications, dosing and formulation, contraindications, adverse effects, and practical access. A larger percentage from one separate trial is not enough to decide which medicine is better for an individual.
Wegovy is a semaglutide product. On March 19, 2026, the FDA approved a 7.2 mg Wegovy injection dose for weight loss and long-term maintenance in certain adult patients. That specific approval does not mean the dose is appropriate for every adult or interchangeable with another product’s regimen; use depends on the current product label and clinical judgment.

What side effects and safety warnings matter?
Wegovy’s labeled risks
The 2026 Wegovy label lists nausea, diarrhea, vomiting, constipation, and abdominal pain among common adverse reactions. It also has a boxed warning: semaglutide caused thyroid C-cell tumors in rodents, and it is unknown whether this occurs in humans. Wegovy is contraindicated for people with a personal or family history of medullary thyroid carcinoma or with multiple endocrine neoplasia syndrome type 2 (MEN 2). The label contains additional warnings, so this short list is not a substitute for reviewing the full prescribing information with a clinician.
FDA review of suicidal-ideation warnings
On January 13, 2026, the FDA said its review found no increased risk of suicidal ideation or behavior with GLP-1 receptor agonists and requested removal of that warning from affected labels. This update concerns that specific risk signal; it does not mean these medicines have no other risks.

Are compounded versions equivalent or FDA-approved?
No. The FDA says compounded drugs are not FDA-approved and are not reviewed before marketing for safety, effectiveness, or quality. It has reported dosing errors involving compounded injectable semaglutide, including reports requiring hospitalization, and warns that semaglutide salt forms differ from the active ingredient used in approved drugs. The FDA advises patients to obtain a prescription from a doctor and use a state-licensed pharmacy.
On April 30, 2026, the FDA proposed excluding semaglutide, tirzepatide, and liraglutide from the 503B bulks list, saying it found no clinical need for outsourcing facilities to compound them from bulk drug substances. The agency invited comments and said it would consider them before making a final determination. That announcement was a proposal, not a final rule.

Who should consider these medicines, and what about access?
Eligibility depends on the specific product’s current label and a person’s health circumstances; trial results alone do not establish that someone qualifies. A clinician can assess potential benefit, contraindications, other warnings, and which labeled indication applies. Insurance coverage and out-of-pocket cost vary, so confirm coverage and expected cost with the insurer and dispensing pharmacy before starting treatment.
Supply status can change. The FDA records that it determined the semaglutide injection shortage was resolved on February 21, 2025, while noting dated status information for other GLP-1 products and that local supply can vary. That historical update does not establish current availability everywhere; check the FDA’s live shortage information and local pharmacies for current status.
