In FY26, organic revenue rose 5.8% while non-GAAP diluted earnings per share increased just 0.7%, and non-GAAP operating margin declined. The first quarter of FY27 brought a sharper revenue increase and higher company guidance, but an extra fiscal week boosted that quarter’s growth. The test now is whether sales growth can translate into stronger earnings and cash generation while Medtronic continues its long dividend record.
Can Medtronic keep raising its dividend?
Medtronic plc (NYSE: MDT) announced a quarterly dividend of $0.72 per ordinary share on June 3, 2026, equal to $2.88 annualized. The company described the increase as its 49th consecutive year of dividend increases. That is a record of past board decisions—not a guarantee of future increases. Medtronic’s June 2026 announcement gives the declared amount and streak.
For a dividend investor, the relevant question is not only whether sales are rising, but whether earnings and cash generation can support the payout over time. Medtronic reported $5.4 billion in FY26 free cash flow, up 4.6%, and $7.3 billion in cash from operations. It returned $4.2 billion through dividends and net share repurchases combined; the release does not make that figure a dividends-only total. These figures show substantial cash generation, but alone they do not establish the future safety or growth rate of the dividend. The FY26 results release reports these measures.
Is growth finally the real story?
There is a stronger top-line case than FY26’s earnings conversion alone would suggest. For the fiscal year ended in 2026, Medtronic reported revenue of $36.4 billion, up 8.4% as reported and 5.8% organically. Organic growth is a company-defined measure intended to reflect underlying performance; it is not the same as reported revenue growth. Over that year, non-GAAP diluted EPS was $5.53, up 0.7%. The company’s adjusted measure should not be confused with GAAP earnings. Medtronic’s FY26 release provides the figures and definitions.

The gap matters: sales grew much faster than adjusted per-share earnings. Non-GAAP operating profit rose 2.4%, while non-GAAP operating margin fell 130 basis points. GAAP operating margin was flat. In other words, the FY26 results showed revenue expansion, but not a corresponding acceleration in profitability.
Growth varied across businesses
Medtronic did not report uniform FY26 growth across its portfolio. The company described organic growth as high-single digits in Cardiovascular, low-single digits in Neuroscience and Medical Surgical, and high-single digits in Diabetes. Those are company-reported descriptors, not precise segment growth rates. Comparisons also need care: Medtronic’s proxy says MiniMed completed an IPO in March 2026 as part of a multi-step separation plan, and FY27 guidance includes Diabetes revenue for the full year. Portfolio reporting changed for FY27, so segment comparisons across the transition may not be like-for-like. What changed in the latest quarter?
For the first quarter of FY27, Medtronic reported revenue of $9.8 billion, up 13.7% both as reported and organically. On September 1, 2026, the company raised its FY27 guidance to organic revenue growth of 7.25%–7.75% and non-GAAP diluted EPS of $5.94–$6.00. These are management’s expectations, not achieved results. The guidance includes Diabetes revenue for the full fiscal year. The Q1 FY27 release gives the quarter’s performance and revised ranges.
The headline quarterly growth rate needs a calendar qualification. FY27 has 53 weeks, and Medtronic estimated that the extra week added approximately $570 million to Q1 organic growth. That contribution makes 13.7% a poor standalone estimate of the business’s recurring quarterly pace; it should not be annualized without adjusting for the extra week. Management’s guidance is a full-year expectation and also reflects the year’s changed business scope.

Management’s explanation
In the September 1 release, Medtronic CFO Thierry Piéton said: “The combination of strong operating performance and disciplined financial management drove revenue and adjusted EPS ahead of expectations, enabling us to raise our fiscal 2027 guidance.” This is management’s characterization of the quarter. The reported growth, revised guidance and calendar effect are the figures investors can use to assess that claim.
What would make growth the central story?
Medtronic has entered FY27 with stronger reported momentum and higher guidance, but a single quarter cannot establish that the improvement is durable. The key evidence to watch in subsequent results is whether revenue gains produce faster earnings growth and better margin conversion, and whether free cash flow continues to support both investment in the business and shareholder returns.

- Compare sales and earnings: FY26 organic revenue growth of 5.8% significantly exceeded non-GAAP diluted EPS growth of 0.7%. Future results can show whether that spread narrows.
- Track margins: FY26 non-GAAP operating margin fell 130 basis points even as revenue increased. Stronger sales alone do not resolve that profitability question.
- Check cash generation against capital returns: FY26 free cash flow was $5.4 billion, while the $4.2 billion return figure combines dividends and net share repurchases. These are useful context, not a dividend payout ratio.
- Use comparable periods: Account for FY27’s extra week when interpreting Q1 growth, and remember that full-year guidance includes Diabetes revenue for the full year.
How strong is the case today?
The evidence supports a cautiously improved growth outlook, not a confirmed turnaround. FY26 revenue growth was real but converted only modestly into adjusted EPS growth, while non-GAAP margin contracted. Q1 FY27 and raised guidance strengthen the case that momentum has improved, but the extra week and the full-year inclusion of Diabetes complicate direct comparisons. Whether growth becomes the defining story depends on the next stage: sustained earnings and cash-flow conversion, not sales growth alone.
The available company results and guidance establish what Medtronic reported and expects; they do not establish whether FY27 targets will be met, whether the dividend streak will continue, or whether the shares offer attractive future returns.
