CMS’s GLOBE model is a scheduled experiment in how Medicare calculates inflation rebates for certain drugs covered under Original Medicare Part B—not a price cap on all Medicare prescriptions. Separately, a mid-stage Lilly study reported greater weight loss with a Zepbound–eloralintide combination than with either drug alone among participants who remained on treatment for 48 weeks. Both stories come with important limits: GLOBE initially reaches selected geographic areas, and the study’s results do not establish approval, long-term outcomes, or how well patients generally will tolerate the combination.
What Medicare’s GLOBE plan would change
CMS calls the policy the Global Benchmark for Efficient Drug Pricing (GLOBE) model. It tests an alternative way of calculating inflation rebates for certain separately payable drugs and biological products covered under Original Medicare Part B, using international drug-pricing information. These medicines are generally administered in clinical settings. The model does not set a blanket price limit for every drug in Medicare. CMS’s GLOBE overview describes its scope and design.
CMS says it expects the model to reduce out-of-pocket costs for beneficiaries included in it while preserving or enhancing care quality. Its selected geographic areas are estimated to encompass approximately 25% of Original Medicare beneficiaries; this is the model’s estimated geographic reach, not a promise that all beneficiaries will pay less. The agency says beneficiaries do not need to enroll or take action, and the model is intended to leave Medicare coverage and provider choice unchanged. See the CMS fact sheet.
Three dates that should not be confused

CMS’s model materials provide the operating and coinsurance schedule; the Federal Register notice supplies the rule’s effective date. Keeping those milestones distinct avoids treating the legal start of the regulation as the date beneficiaries first see the coinsurance change.

What the reported savings comparison does—and does not—say
STAT’s October 1, 2026 roundup says the final rule covers four companies and is expected to save 96% less than the initial proposal. Those are STAT’s reported comparisons, not figures independently established here as CMS estimates. The roundup also notes that the companion GUARD pilot remains proposed. The available CMS materials describe GLOBE’s purpose and design, but these reported comparisons should not be read as a general estimate of savings for every Medicare enrollee. STAT’s Pharmalittle roundup is the source for that characterization.
What Lilly’s Zepbound–eloralintide study reported
STAT reported results from a mid-stage study in people with obesity and type 2 diabetes, comparing Eli Lilly’s Zepbound (a GLP-1 drug), investigational eloralintide (which targets the amylin hormone), and their combination. At 48 weeks, the reported weight-loss figures were:

The 23.3% result is specifically for participants who remained on the highest-dose combination. STAT also flags high treatment discontinuation as a tolerability concern, but the accessible report does not give a discontinuation percentage or a detailed adverse-event breakdown. The figures therefore do not describe outcomes for everyone who began treatment, establish long-term results, or guarantee an individual patient’s response. They also do not show that eloralintide or the combination is approved for use. STAT’s account is the source for the reported comparison and its limitations.
