The changes are proposed for 2028, not yet in effect. The CDC is accepting public comments through October 20, 2026.
What is changing in the NHIS?
The NHIS is a U.S. household health survey that the CDC says has collected and analyzed health data since 1957. Its questionnaires are being redesigned for 2028. The agency says the proposal aims to address rising collection costs and difficulty reaching households, while making the survey more flexible and reducing respondent burden through shorter, less complex instruments. The previous major redesign was in 2019.
The proposal includes revised adult and child questionnaires and a sequential mixed-mode collection design with self-response options. STAT reported that the proposed adult questionnaire is reduced from 482 questions to 150 and that roughly 25,000 households are expected to receive a mailed questionnaire. Those figures are from STAT’s October 1, 2026 report, not the CDC redesign page.
The CDC has invited public comment on potential effects on data quality, usability and trends over time. The proposal and comment process are described on the CDC’s NHIS questionnaire redesign page.

Which disability questions remain in the draft, and which are reported removed?
The August 2026 public draft adult questionnaire still asks about difficulty in six functional areas: seeing, hearing, walking or climbing steps, remembering or concentrating, self-care, and communicating. It also asks about difficulty running errands alone, social participation, work limitations, and missed work because of illness, injury or disability. The wording can be reviewed in the August 2026 draft adult questionnaire.
STAT reports that other disability-related questions are being removed, including questions about hearing aids, fatigue, cognition and mobility supports such as wheelchairs and scooters. The distinction matters: the draft retains questions about functioning, but some contextual and support-related details are not represented in the same way, according to STAT’s comparison. It would be inaccurate to say the proposal eliminates all disability measurement.

Why do the response options and classification rule matter?
The CDC’s current adult disability composite is based on six functional domains: seeing, hearing, mobility, communication, cognition and self-care. Each item has four response levels: no difficulty, some difficulty, a lot of difficulty, and cannot do at all. Under the current composite, a respondent is classified as having a disability if they report “a lot of difficulty” or “cannot do at all” in at least one domain. The CDC explains this measure on its adult disability methodology page.
This is a functional measure, not a complete count of diagnoses, support needs or every form of disability. Although the draft retains questions across the six domains, that similarity alone does not establish that the redesigned questionnaire will preserve the current response options, classification rule or analytic comparability. The CDC’s 2019 redesign described functioning and disability as annual-core content; its 2019 redesign information and 2019–2027 adult content schedule provide historical context for that approach.

What are advocates concerned about?
Advocates and researchers quoted by STAT worry that losing detail could make it harder to identify or understand some groups, including people with intellectual and developmental disabilities. Katy Neas, CEO of The Arc, said the survey has historically undercounted people with disabilities and called the changes a “head-scratcher,” as reported by STAT. Syracuse University sociology professor Scott Landes said, “You’re making us choose between bad and worse.” These are stakeholder concerns, not findings from a survey of the proposed instrument.
The reviewed sources do not quantify how the redesign would change disability estimates or establish the magnitude of any future undercount. The proposed questionnaire has not yet been fielded, so effects on subgroup estimates, response rates and continuity with past trends remain unknown.

How can the public comment?
The redesign remains a proposal, and the final 2028 instrument may change. The CDC’s stated comment deadline is October 20, 2026. The official redesign page provides the proposal details and instructions for submitting written comments.
