What does the Carruthers record say about Fowler’s role?
The emergency motion says Tennessee’s 2025 execution protocol called for the contracted physician to establish a central line if the IV team could not obtain peripheral access. It says Fowler attempted central access after access could not be established. The motion says Fowler testified under oath in an October 2025 deposition that he had not placed a central IV line since 2013. NPR’s September 30 report, citing legal records, similarly said he had not placed one in 13 years. Details about his prior attempts, technique and hospital privileges are described in the motion’s account of that deposition; the sources reviewed do not independently establish a broader assessment of his medical qualifications.
NPR reported that Fowler told the outlet by phone on September 24: “the doctor does not participate in the execution. The only thing the doctor does is declare the person dead.” That is Fowler’s characterization as quoted by NPR, not an uncontested account of his precise duties under Tennessee’s protocol.

What happened during Pike’s attempted execution?
WSMV reported on October 1 that Pike had received two doses of pentobarbital and was transported to an off-site medical facility. Her attorneys said she was “still alive and snoring.” The Tennessee Department of Correction, by contrast, said the state had followed its established protocol and that the protocol did not allow additional procedures beyond those carried out that evening. These are attributed accounts; the reviewed reporting does not supply an independent final reconstruction of the attempt.
Afterward, Governor Bill Lee said he had ordered a comprehensive third-party review and that the remaining execution scheduled for 2026 would not be carried out that year. The review’s findings were not included in the sources reviewed for this article.

What complaint was filed about the Carruthers attempt?
The Associated Press reported on July 16, 2026, that Carruthers’s sister had filed a complaint accusing Fowler of ethical and procedural violations. The complaint alleged, among other things, that Carruthers experienced severe pain, that lidocaine was given without first establishing whether he was allergic, and that attempts continued despite visible distress. It also challenged Fowler’s qualifications to place a central line. These are allegations, not established findings.
AP reported that Fowler declined to comment. The Tennessee Department of Health would not disclose complaints or investigations before formal charges, so the public reporting cited here does not establish the complaint’s disposition or whether a formal disciplinary action followed.
