The safest way to clean up a WordPress Media Library is to separate four jobs: reviewing attachment records, finding files in wp-content/uploads that have no database record, regenerating or removing obsolete thumbnails, and reducing image-file sizes. Back up the database and uploads folder first, then scan, review every proposed deletion, use trash or quarantine where available, and test important pages before permanently removing anything.
What “clean up” means in WordPress
WordPress stores media in more than one place. An attachment record appears in the Media Library database, while the original file and its derivatives live under the uploads directory. A file can therefore be present without a Media Library entry, and a Media Library item can be marked “unattached” even though a theme, builder, widget, custom field, or external page still references it.

Why “unattached” does not mean “unused”
WordPress’s unattached filter is useful for creating a review list, but it is not proof that an image is safe to delete. A file may be inserted by direct URL, assigned as a featured image, stored in post metadata, used in a widget or template, or embedded in a page builder’s data. A plugin’s scanner can only detect the reference patterns it knows how to inspect.
The same distinction applies in reverse: an orphan-file scan looks for physical files without attachment records. The Cleanup Orphan Images listing says its files may still be referenced directly by themes or plugins and gives generated WebP files as an example of files that may not be registered as ordinary Media Library items.

Prepare a recoverable cleanup
- Back up the database and uploads. Keep a restorable copy of the database and the complete uploads directory before a bulk change. The Cleanup Orphan Images documentation specifically warns that its deletion is permanent and advises backing up uploads: plugin documentation.
- Record the scope. Decide whether the problem is admin clutter, storage usage, orphan files, obsolete thumbnails, or oversized images. Do not run a thumbnail command when your actual problem is unknown attachments.
- Prefer staging for complex sites. Test on a staging or backup copy when the site uses builders, custom fields, commerce, dynamic templates, a CDN, or image-optimization software.
- Plan a recovery window. Keep plugin trash, quarantine, hosting snapshots, or your backup until important pages have been checked.
Review the Media Library before scanning
Start with the native Media Library. Inspect attachment dates, file types, dimensions, and where an item is assigned. WordPress.org’s orphan-cleanup listing notes that WordPress provides a built-in filter for unattached images, but also distinguishes those database entries from orphan files in the uploads directory. Use the filter as a lead for investigation, not as an automatic deletion queue.
For a small site, open each candidate and check posts, pages, menus, logos, galleries, and featured images that could use it. For a large site, export or scan candidates in batches so a mistaken deletion is easier to identify and reverse.

Choose a scanner for unused media
Usage scanners inspect attachment records and, depending on the product, additional content or filesystem locations. Their feature lists are publisher claims rather than a universal accuracy guarantee, so compare their coverage with your site’s actual stack.

Review every flagged item
- Open the image in its original context, including templates and builder layouts.
- Search for direct URL references if the filename is used in custom HTML, CSS, JavaScript, feeds, or external systems.
- Check featured-image fields, site branding, widgets, menus, and custom post types.
- Delete in small batches and note which batch changed which pages.
Find files that are not Media Library records
An orphan-file scan asks a different question: “Does a file exist under uploads without an attachment record?” The Cleanup Orphan Images listing advertises this check and manual selection before deletion. It also warns that direct-URL references in themes or plugins may not be detected and that deletion is permanent.

Do not assume every unregistered file is junk. Image-optimization and hosting tools may generate WebP or other derivative files outside the normal attachment list. Review filenames, modification dates, the software that created them, and references in source code or configuration before removing them.
Regenerate or remove obsolete thumbnails with WP-CLI
Thumbnail maintenance is not an unused-media scan. Use it when image-size registrations changed, thumbnails are missing, or you need current derivatives. WordPress documents wp media regenerate for regenerating thumbnails for one or more attachments and for targeting a named image size: WP-CLI regenerate documentation.

wp media regenerate
To regenerate only a selected size, use the documented image-size option:
wp media regenerate --image_size=medium
When old image sizes are no longer registered, --delete-unknown targets thumbnails for those sizes. The documentation also provides --skip-delete, which preserves original thumbnails that might be linked outside your control:

wp media regenerate --delete-unknown
wp media regenerate --skip-delete
Confirm that the active theme and plugins no longer need an old size before deleting it. The broader wp media reference covers listing registered sizes and related media commands.
The third-party Media Library Tools listing also describes batch regeneration and removal of orphaned thumbnail files. Treat those capabilities as plugin features, not as WordPress core behavior.

Compression and format conversion solve a storage or performance problem without removing an image from the site. Media Library Tools describes compression controls, while MediaPilot AI describes local WebP or AVIF conversion and storage analytics: Media Library Tools and MediaPilot AI. The available documentation does not establish a universal percentage of savings or a quality result, so test representative images and check visual quality, responsive sizes, CDN behavior, and backups before processing the whole library.
Verify the site after cleanup
- Open key posts and pages in an incognito browser window.
- Check featured images, galleries, product pages, landing pages, logos, Site Icon, menus, widgets, and builder templates.
- Inspect responsive layouts and image URLs served through a CDN or optimization layer.
- Watch server logs, broken-link reports, and user reports for missing assets.
- Restore a flagged file or attachment immediately if a broken image appears, then identify the reference pattern the scan missed.
- Keep the backup or recoverable trash until the review period is complete; only then permanently delete the remaining items.

Bottom line
Clean the Media Library as an investigation, not a one-click purge. Back up first; distinguish attachment records, physical uploads, and thumbnail derivatives; select a tool that checks the references your site actually uses; review proposed deletions; and verify the public site before permanent removal. That process takes longer than deleting everything marked unattached, but it is the practical way to reduce clutter and storage without turning missing images into a new maintenance problem.
