“Add a screenshot to a PDF form” can mean three different things: put a fixed screenshot on a PDF page, let someone provide an image while filling out a form, or add an image to a source document before exporting it as a PDF. The documented workflows below cover building a form in Word and preparing fillable PDF fields in Acrobat. They do not establish the exact steps for placing a static screenshot directly onto an existing PDF page, so the right method depends on which result you need.

Choose the right workflow first

Before editing, decide whether the image is fixed or supplied by the person filling out the form, whether the PDF already exists, and which software you can use. If accessibility matters, plan for an image description and for usable field labels and reading order.

Make a Word form with an image placeholder

Microsoft documents Picture Content Control as an option for a form created in Word. It provides a place in the Word form for a picture; it is not proof that a converted PDF will offer a universally compatible image-upload field.   1. Open the Word document you are using as the form.
  2. Place the insertion point where the image placeholder should appear.
  3. Choose Developer > Picture Content Control.
  4. Add a nearby text label describing what image is expected, such as “Attach a screenshot of the error message.” Keep the label in the document rather than relying on the placeholder alone.
  5. If the form is meant to be completed in Word, save and test the Word file with the intended users.
  6. If you export it as a PDF, open the resulting PDF and verify the appearance and behavior yourself. Do not assume the picture control remains an interactive upload area after conversion.

For an informative image already placed in a Word document, give it concise alt text that communicates its purpose or relevant evidence. In the Word workflow described by Microsoft, an image should be in line with text to be read correctly by screen readers. Microsoft also provides instructions for inserting an image from a phone’s photo library or camera and for adding alt text in Word: Use a screen reader to insert a picture or image in Word. Those are Word-document instructions, not a documented method for editing a PDF on a phone.

Create or edit fillable fields in Acrobat

For a scanned paper form or an existing document that needs interactive fields, Adobe documents Acrobat’s All tools > Prepare a form workflow. It can start from an existing file or scan, identify likely fields, and let you adjust the resulting form. Adobe’s page was last updated September 23, 2025: Convert documents and scans into fillable PDF forms in Acrobat.

  1. Open the source PDF or scan in Acrobat.
  2. Select All tools > Prepare a form.
  3. Choose the existing file or scan to use as the form source and create the form.
  4. Review the fields Acrobat identifies. Resize, rearrange, or delete fields as needed, and add form components where necessary.
  5. Save the form, then inspect and test its fields and navigation.

This workflow prepares form fields. It does not, based on the cited instructions, tell you how to insert a static screenshot as page artwork or guarantee a general-purpose image-upload field for respondents. If you need a fixed screenshot on a page, locate a PDF editor feature explicitly intended to add an image to page content; verify the result in the saved file. If you need users to submit screenshots, confirm that your chosen form format and receiving workflow actually accept the image files users must provide.

Adobe Experience League also describes making fillable forms from scanned paper or documents authored in Word, Excel, InDesign, or another application: Create fillable forms. Some accessibility features described in Adobe’s tutorial are available only in Acrobat Pro, Premium, and Studio; edition availability is not universal. Insert an image in the source document before exporting

If the screenshot should be a fixed part of a form, placing it in the source document before exporting can be simpler than editing page content in a PDF. Insert the screenshot using the source application’s image command, position it with the form text, add a concise alternative description when it conveys information, then export to PDF and inspect the result. Microsoft’s Word guidance covers inserting images and adding alt text, including on mobile: Word image insertion and alt text.

Keep the screenshot near its explanation or label, and avoid placing it over instructions, response areas, or existing controls. If the source file is a scanned paper form rather than an editable document, use a PDF workflow to create fields; Acrobat’s Prepare a form route is documented above.

Make the form accessible and check the finished PDF

A screenshot can be meaningful evidence, so provide concise alt text or an equivalent description when readers need its content to understand the form. For PDF forms, WebAIM recommends four practical tasks: add appropriate form fields, give each field a descriptive tooltip, tag the fields, and check or repair tab order.   • Check that the screenshot is legible at normal viewing size.
  • Confirm it does not cover labels, instructions, fields, or controls.
  • Check that the description explains the image’s purpose rather than repeating nearby text without adding context.
  • Test that interactive fields can be reached and used, and check their reading and tab order.
  • Open the exported PDF in the software your intended audience is likely to use and verify the form behaves as intended.

Troubleshooting common problems

The picture control is missing in Word

Confirm that you are editing the form in Word desktop and that the Developer tab and form controls are available in your version. Microsoft’s instructions cover supported Word versions and identify the control under Developer > Picture Content Control. If the document is actually a PDF, the Word control is not a PDF editing command.

The control stopped behaving like an image placeholder after PDF export

The cited Word guidance does not establish that Picture Content Control survives conversion as a fillable PDF image-upload area. Test the exported file rather than assuming this behavior. If respondents must supply images, choose a submission workflow that explicitly supports image uploads, or keep the form in a format that supports the control.

Acrobat found fields in the wrong places or missed one

Review the automatically identified fields after choosing All tools > Prepare a form. Adobe’s documented workflow allows fields to be resized, moved, deleted, or added. A scanned or complex source may need more manual correction than the automatic identification suggests.

The screenshot covers a field or is hard to read

Reposition or resize the image in the source document or the PDF editor’s page-content tools, then inspect the saved PDF at normal viewing size. Keep form labels and controls unobstructed; an image that looks clear only when heavily zoomed may not serve readers well.

Screen-reader users cannot identify the image or navigate the form

Add an informative description where the authoring workflow supports it, provide descriptive labels or tooltips for fields, tag fields, and check tab order. Word image guidance and WebAIM’s PDF form recommendations address these as separate concerns: image description alone does not make interactive fields accessible.

Frequently asked questions

Can I add a screenshot to a PDF form on my phone?

The cited phone guidance describes inserting an image in Word, not placing one directly into a PDF. For a PDF, use a PDF editor that explicitly supports page-image insertion, or edit the source document on a device and export it.

Does Acrobat Prepare a form create an image-upload box?

The reviewed Adobe instructions describe preparing and editing PDF form fields but do not establish a universal image-upload field. Verify that the exact PDF software and submission process you plan to use support image files from respondents.
