To use an image already in WordPress as a post’s featured image, open the post, use its Featured image control to select the image from the Media Library, choose Set featured image, then save or update the post. An image inserted into the post body is not automatically assigned as its featured image.
Set an existing attachment as the featured image

Open the post you want to change in the WordPress editor.

Find the Featured image control. Its location and wording can vary by editor and WordPress version; look in the editor sidebar or the relevant post settings.

Open the control and choose the Media Library tab to browse images already uploaded to the site.

Select the image you want, then confirm with Set featured image.

Save or update the post. Reopen or preview it to verify that the chosen image appears in the editor’s Featured image control.

The Media Library lets you select previously uploaded items, but selecting an image there does not assign it to a post by itself. The featured-image control creates that post-level association. See the WordPress guide to inserting images into posts and pages and the Classic Editor guide.
Featured image versus an image in the post
A post can contain an image in its body without using that image as its featured image, and it can have a featured image that is not embedded in the body. If you want the same picture in both places, insert it into the post and assign it separately through the Featured image control.

If the Featured image control is missing

Look for a hidden panel. Depending on the editor, available panels or screen options may let you show the featured-image setting.

Check theme support for the post type. WordPress’s Theme Handbook explains that a theme can enable post thumbnails with
add_theme_support( 'post-thumbnails' )and can specify which post types support them. The control may not be available if support is not enabled for the content type you are editing. See the Featured Images & Post Thumbnails section of the Theme Handbook.Rank #4

If the image is assigned but does not appear on the public page
Assignment and display are separate. A featured image can be selected in the editor while the site’s theme template does not render it, or renders it using a different image size or crop. Check the post in the editor first, then review the theme’s template behavior if the image is missing or looks different on the front end. If you are working in code and already have the IDs of the post and image attachment, use WordPress’s set_post_thumbnail() function:

set_post_thumbnail( $post_id, $attachment_id );The function takes a post ID (or WP_Post) and an attachment ID. The attachment must be usable as an image; WordPress checks for an image through wp_get_attachment_image(). Its return value is an integer or boolean. A false result can indicate failure or that the same value was already stored, so do not interpret every false as proof that a different image was assigned. Consult the WordPress Developer Resources reference for set_post_thumbnail().
When the image is already in the Media Library as an attachment, there is no need to create another attachment. wp_insert_attachment() is relevant to a separate workflow that creates an attachment from a file; its ID can then be passed to set_post_thumbnail().
