The FAA authorized Falcon 9 to return to flight on February 6, 2026, after accepting SpaceX’s investigation of the Starlink Group 17-32 mishap. The probable cause was a second-stage engine that failed to ignite before the planned deorbit burn. SpaceX launched the next Starlink mission the following day.
What happened on Starlink Group 17-32
Falcon 9 lifted off on February 2, 2026, from Space Launch Complex 4 East at Vandenberg Space Force Base in California, carrying 25 Starlink satellites. SpaceX’s mission page records the launch and satellite deployment. The first stage completed its part of the flight and landed; the anomaly involved the second stage later in space. The distinction matters: a payload can reach orbit even if the upper stage encounters a problem afterward. The FAA identified the failure as occurring before the planned deorbit burn, a maneuver intended to dispose of the spent stage. An issue with that post-deployment step is not the same as a launch-site explosion or a failure to deliver the payload.
What the FAA investigation found
The FAA required a mishap investigation, conducted by SpaceX under FAA oversight. This is a technical and safety process: it seeks to identify a probable cause, determine corrective measures, assess public-safety implications and inform whether operations can resume. It is not, by definition, a criminal inquiry or a conventional aircraft accident investigation. (FAA mishap and return-to-flight framework)

Probable cause and public impact
In its February 6 statement, the FAA said the probable root cause was the Falcon 9 second-stage engine failing to ignite before the deorbit burn. The agency reported no public injuries or damage to public property. That public-safety finding does not mean the mission was flawless: payload deployment and the upper stage’s later disposal are separate parts of the flight. (FAA February 6 statement)
Corrective actions
The FAA said SpaceX identified technical and organizational preventative measures. Its public statement does not enumerate those measures or provide a component-level account of the ignition sequence, so it would be premature to claim a particular engine modification, software change or inspection procedure was adopted.

Why Falcon 9 could return to flight
The FAA said it accepted SpaceX’s final report and corrective actions, closed the investigation and authorized Falcon 9 to return to flight on February 6. Investigation closure and flight authorization are related but distinct decisions: the agency may, under its framework, permit a return before an investigation is closed if safety conditions are met, but that was not the status it reported for this mishap. The return-to-flight decision concerns whether continued operations present an unacceptable risk to public safety after the mishap and its corrective measures are considered. It is not a guarantee that every vehicle subsystem is failure-proof, nor does it waive applicable licensing conditions or requirements for a specific mission. Starlink launches resumed the next day
SpaceX’s first post-clearance Falcon 9 mission was Starlink Group 17-33, launched from Vandenberg on February 7 with 25 satellites. SpaceX’s mission page records deployment. Its archive also lists Starlink Group 6-103 on February 16 and Group 6-104 on February 21, showing that Starlink launches continued later that month. These flights establish a resumption of operations, not that every planned launch kept its original schedule or that the mishap had no commercial consequences. - It establishes: The FAA accepted the mishap report and corrective actions, closed this investigation and authorized Falcon 9 to resume flights.
- It does not establish: That Falcon 9 is immune to future anomalies, that all later missions are automatically licensed, or that Starlink’s schedule and business were unaffected.
- It leaves technical detail limited: The FAA’s public statement identifies the probable cause and describes corrective actions broadly, but does not provide a complete engineering breakdown.
This February 2026 decision concerns Starlink Group 17-32. It should not be conflated with separate Falcon 9 investigations involving anomalies in 2024; each event has its own findings and return-to-flight determination. (FAA statements)

For Starlink, Falcon 9’s return allowed launches to resume quickly after a brief interruption. For the FAA, the relevant question was whether identified causes and preventive measures supported safe continued operations—not whether the February 2 mission had been entirely successful.
