The dependable way to create a reusable PDF template is to separate layout from interactivity: design the page in LibreOffice Writer (or another document editor), export it as a PDF, then use a PDF form authoring tool such as Acrobat to place real fields. If you already have a document or scan, convert that source; if you are starting with a blank form, create the form canvas first. A printed blank line alone is not a fillable field.

Choose your starting route

Your first decision is whether the page already exists.

Adobe documents both workflows: Acrobat can use Word, Excel, scanned material, or a blank page as the source (PDF forms guide). The interface is being updated, so look for the equivalent of Prepare a form if your menus differ.

Route A: turn a Writer document into a template

1. Design the static page in LibreOffice Writer

  1. Set the page size, margins, header, footer, logo, and typography.
  2. Write every prompt exactly as recipients should see it. Leave visible space for answers, but do not treat that space as interactive yet.
  3. Use tables or aligned paragraphs for consistent rows. Avoid manually typed spaces for alignment.
  4. Export with File > Export as PDF (or the PDF toolbar button), and open the resulting file to confirm that page breaks and fonts survived.

2. Open the PDF in a form authoring tool

In Acrobat, choose Prepare a form and select the existing PDF. Acrobat can recognize static blanks in an existing document or printed form and propose fillable fields (Adobe’s PDF forms documentation). Automatic detection is a starting point, not a finished template.

3. Review every detected field

  • Delete fields placed over headings, decorative lines, or the wrong row.
  • Resize fields so the complete value is visible at normal zoom.
  • Replace an incorrect type: a yes/no choice may need radio buttons, while independent selections need checkboxes.
  • Give each field a clear, unique name. Names such as customer_email are easier to manage than repeated names such as Text1.
  • Set required or optional status deliberately. Use read-only fields for values recipients must not change.
  • Enable multiline input for addresses, explanations, and other answers that may wrap.
  • Add a tooltip that explains what belongs in an ambiguous field; tooltips are especially useful for screen-reader users.

4. Preview, save, and test

Use the authoring tool’s preview or recipient view. Tab through the controls, type long and short values, open every menu, select and clear every checkbox or radio group, and test keyboard-only completion. Save the completed template under a new filename so the editable source remains available.

Route B: create a form from a blank page in Acrobat

Adobe’s instructions, last updated November 5, 2025, specify this sequence: choose Menu on Windows or File on macOS, select Prepare a form, choose Start from blank page, and optionally indicate whether a signature is required (Adobe’s create-from-scratch instructions).

  1. Choose Text to place headings, instructions, and labels.
  2. Add the controls your process needs: text fields, image fields, checkboxes, radio buttons, drop-down lists, or list boxes.
  3. Align controls and labels, then set field names and properties.
  4. Preview the recipient view, correct spacing and tab order, and save the PDF.

That workflow creates a reusable template without first composing a Writer file. It is useful for short forms, while a carefully designed Writer source can be faster for a letterhead-heavy or multi-page layout.

Use the right interactive control

Adobe distinguishes these interactive controls from static lines and labels (PDF forms guide). A PDF that merely looks like a form will not behave like one in a viewer.

Make the template usable and accessible

Labels and instructions

Put a visible label next to each field, not only inside a placeholder that disappears while typing. State units, accepted formats, and whether a field is required. Keep instructions short and place them before the control they describe.

Keyboard order and focus

Set a logical tab sequence that follows the visual reading order. Give every control a visible focus state where the authoring tool supports it, and verify that a user can complete the form without a mouse.

Contrast, structure, and reading technology

Use sufficient contrast, meaningful heading structure, and text that remains selectable. Test with a screen reader or accessibility checker when the form matters to the public. Adobe publishes a seven-step accessible-forms tutorial, but notes that some capabilities are limited to Acrobat Pro, Premium, and Studio (Create Accessible Forms). The exact controls available depend on your Acrobat edition.

Distribute and collect completed PDFs

Decide how responses will arrive before you publish the template. Acrobat supports distribution and response collection workflows, and users can fill and save forms in Acrobat or Reader (Adobe’s forms guide). Do not assume identical save, submit, calculation, or attachment behavior in every third-party viewer: test the actual software your recipients use.

  • Send a copy when each recipient should work independently.
  • Use a controlled collection workflow when you need centralized responses or auditability.
  • Keep an untouched master and a version number in the document metadata or footer.
  • Open a returned file in the intended viewer and verify that values, signatures, and formatting are preserved.

Troubleshooting common failures

The PDF has lines but nobody can type

The lines are static artwork. Reopen the file in Prepare a form, add text fields, and save a new copy.

Fields appear in the wrong places

Automatic detection inferred fields from visual spacing. Delete false positives, drag fields to the correct labels, and inspect every page at 100% zoom.

Typing is cut off

Increase the field height or enable multiline. Set an appropriate font size and test the longest realistic answer.

Only one checkbox can be selected

The controls were created as a radio group. Replace them with separate checkboxes when choices are independent.

The form works in Acrobat but not another viewer

Viewer support varies. Test a filled copy in the recipient’s application, avoid unsupported calculations or scripts when broad compatibility matters, and provide a plain-PDF fallback if necessary.

Layout changed after export

Check Writer page styles, embedded fonts, image anchoring, and page breaks. Export again, then compare the PDF page by page before adding fields.

  • Every intended answer area is an actual interactive field.
  • Field names, required status, multiline behavior, and tooltips are correct.
  • Tab order follows the page and all controls work from the keyboard.
  • Long values, empty values, dates, choices, and signatures have been tested.

Can LibreOffice Writer itself export a fillable PDF?

Writer is well suited to designing the visible page, but the workflow described here adds interactive PDF fields in a form authoring tool after export. Verify the capabilities of your exact LibreOffice version before relying on PDF form controls created directly in Writer.

Should I start with a scan or rebuild the form?

Use a scan when preserving the paper layout matters and you can carefully review detected fields. Rebuild the layout when the scan is skewed, low quality, or difficult for recipients to read.

Do recipients need Acrobat to complete the form?

Acrobat and Reader support filling and saving PDF forms, but behavior can differ in third-party viewers. Test with the software your audience actually uses.
