FLTMGR.SYS is Windows’ File System Filter Manager, not proof that a Microsoft file is broken. It coordinates file-system filter (“minifilter”) drivers used by antivirus, encryption, backup, cloud-sync and disk utilities. Record the complete stop code first: FLTMGR_FILE_SYSTEM (0x000000F5) is different from another bug check that merely shows What failed: FLTMGR.SYS. The safest fix is to protect your data, remove or update recently changed filter software, repair Windows and the file system, then use crash-dump evidence to identify the actual driver or hardware fault.
What FLTMGR.SYS means
FLTMGR.SYS is a Microsoft kernel driver implementing Windows File System Filter Manager. It provides the infrastructure through which minifilter drivers inspect, modify or monitor file operations. Security products, ransomware protection, encryption, backup and snapshot tools, cloud synchronizers, disk monitors and some storage software can all attach to this path.
Microsoft defines FLTMGR_FILE_SYSTEM, bug check 0x000000F5, as an unrecoverable failure in Filter Manager. That definition does not identify one universal cause. A faulty third-party filter, storage driver, corrupted file system, failing drive, damaged Windows component, unstable memory or a recent update can all be part of the investigation.
Separate the stop code from “What failed”

Photograph the entire screen, including the hexadecimal code. Windows 11 wording and screens vary by release; Reliability Monitor, Event Viewer and dump files provide better evidence than the filename alone. Microsoft’s general stop-code guidance covers these logs and staged checks.

Protect data before repairing
- If Windows starts, copy important files to another drive or a trusted backup immediately.
- Have your BitLocker recovery key available before changing firmware, storage settings or recovery options.
- If the drive clicks, disappears, reports SMART warnings or produces read errors, minimize boots and prioritize imaging, data recovery or drive replacement.
- Never delete
C:WindowsSystem32driversFLTMGR.SYSor download a replacement from an unofficial site. - Avoid intensive repair scans until essential data is safe when hardware failure is plausible.
Try the least-destructive fixes first
Install Windows and manufacturer updates
- Open Settings > Windows Update and select Check for updates.
- Install optional driver updates only when they match the affected hardware or a documented issue.
- Use the PC or motherboard manufacturer’s support page for chipset and storage-controller drivers, NVMe firmware and BIOS/UEFI updates.
- Update or remove third-party antivirus, backup, encryption, sync and disk software through its official installer or uninstaller.
Do not use indiscriminate “driver updater” utilities. They can install an incorrect storage or chipset driver and obscure the original fault.
Undo a recent software or driver change
If crashes began immediately after installing a product, update or driver, roll it back or uninstall it. In Device Manager, open the affected device’s Properties > Driver tab and use Roll Back Driver when available. Keep security protection enabled; if a security filter must be removed for testing, use Windows Security temporarily and reinstall or replace the product promptly.
Boot Windows 11 into Safe Mode
Safe Mode loads fewer drivers and services, so it can reveal whether a startup filter or service is involved. From Windows Recovery Environment (WinRE), follow this path:
- Hold Shift while selecting Restart, or interrupt startup several times to trigger recovery.
- Select Troubleshoot > Advanced options > Startup Settings > Restart.
- Press 4 for Safe Mode or 5 for Safe Mode with Networking.
This recovery sequence is documented by Microsoft in its stop-error troubleshooting guide. In Safe Mode, remove recently changed filter software, inspect Device Manager for storage or chipset changes, and document the original startup configuration before performing a clean boot. Safe Mode working increases suspicion of a third-party driver but does not prove it; Safe Mode may simply avoid a particular workload or hardware trigger.
Repair Windows components
In Windows Terminal (Admin) or Command Prompt (Admin), run:
DISM.exe /Online /Cleanup-Image /RestoreHealth
sfc /scannow

- If DISM repairs the component store, restart and run SFC again.
- If SFC reports repaired files, restart and test.
- If SFC cannot repair some files, review the CBS log and continue with storage and dump analysis instead of endlessly repeating the command.
- When Windows is offline in WinRE, do not assume these online commands are valid; identify the Windows volume and use carefully adjusted offline DISM/SFC paths.
Check the file system with CHKDSK
Start with the less disruptive scan on the Windows volume:
chkdsk C: /scan
If corruption is found or repair is recommended, schedule:

chkdsk C: /f
Windows may run it at the next restart because the system volume is in use. Use the more intensive bad-sector scan only when appropriate:
chkdsk C: /r
/r checks for bad sectors and attempts to recover readable data. It can take much longer and add stress to a failing drive; it is not a substitute for backup or professional recovery. In WinRE, the Windows partition may not be C:. CHKDSK addresses logical file-system problems and some disk-read issues, not buggy minifilters, defective RAM or every storage-driver fault. Microsoft lists these checks in its stop-error procedure.
Check storage, firmware and connections
- Run the SSD/NVMe manufacturer’s health and firmware utility or the OEM’s boot-time diagnostics.
- Investigate SMART warnings, repeated read errors, disappearing disks and thermal or controller alerts.
- On a desktop, power down and check SATA data and power cables; reseat a removable NVMe drive only if you can do so safely.
- Test an external drive or dock separately if crashes occur during backup or file transfer.
- Remove recently installed disk-management, imaging or monitoring software.
A passing CHKDSK result does not rule out firmware, controller, thermal or intermittent hardware problems.
Test memory and overall stability
- Run Windows Memory Diagnostic and, for intermittent failures, test RAM modules individually where practical.
- Return overclocking, undervolting, XMP/EXPO and vendor performance profiles to default settings.
- Check CPU/GPU temperatures and power stability.
- Update BIOS/UEFI only through the hardware manufacturer’s documented procedure.
Memory corruption can make an unrelated kernel module appear in the crash report, so a clean Windows and disk check does not end the investigation.

Use crash dumps to identify the responsible driver
Small dumps normally reside in C:WindowsMinidump. If none are being created:
- Press
Win + R, entersysdm.cpl, open Advanced. - Under Startup and Recovery, select Settings.
- Under Write debugging information, choose Small memory dump (256 KB) or a kernel dump when deeper evidence is needed, and confirm the path.
- Install WinDbg from Microsoft, open the
.dmpfile, then run:
.symfix
.reload
!analyze -v
Inspect BugCheck, Probably caused by, MODULE_NAME, IMAGE_NAME, the stack, loaded filter drivers and driver timestamps. A result naming fltmgr.sys is not conclusive: it may be the point where a third-party driver caused the failure. Microsoft’s dump walkthrough is available in the small-memory-dump documentation.
List file-system filters
For technicians or advanced users, run:
fltmc filters
Correlate the listed antivirus, endpoint, encryption, backup, sync, anti-ransomware, imaging and monitoring products with installation dates and dump stacks. A listed filter is evidence for narrowing the search, not proof of guilt. Never delete its files or registry entries manually; use the vendor’s supported cleanup procedure.

Driver Verifier can expose a defective non-Microsoft driver, but it deliberately stresses drivers and can create boot loops. Create a restore point or ensure WinRE/recovery media is available first. Configure verification selectively for a suspected third-party driver, reproduce the crash, and disable it from Safe Mode or WinRE if instability begins:
verifier /reset
Do not casually run verifier /standard /all; verifying every driver can cause unnecessary crashes and make recovery harder. Microsoft includes Verifier in its advanced stop-error workflow.
If Windows will not boot
Use WinRE to enter Safe Mode, uninstall a recent driver or update, run System Restore when a suitable restore point exists, or select Uninstall Updates to remove a recent quality or feature update. Offline backup or disk imaging should come before repeated repairs when storage failure is suspected. Reset this PC and a clean installation are last resorts: they may not fix defective RAM, storage, firmware or a filter driver that is installed again afterward.

When to replace the drive or seek repair
- SMART or OEM diagnostics report failure.
- The drive repeatedly disappears, produces read errors or causes missing/corrupted files.
- Crashes consistently occur during reads, writes, backups or synchronization.
- Memory tests, firmware checks and driver removal do not stop failures.
- You cannot create a reliable backup or the disk is making unusual mechanical sounds.
At that point, stop treating the filename as the diagnosis. Preserve data, replace suspect hardware or use a qualified recovery service.
Frequently Asked Questions
Can I delete or replace FLTMGR.SYS?
No. It is a protected Windows component, and deleting it can make Windows unbootable. Diagnose the associated driver stack instead.

The genuine file is a Microsoft Windows component. Malware can use similar names, so verify the path and digital signature rather than downloading a replacement.
Does SFC always fix this blue screen?
No. SFC repairs damaged Windows components, but it cannot repair a buggy minifilter, failing storage, incompatible firmware or faulty RAM.
Why does Safe Mode work when normal Windows crashes?
Safe Mode loads fewer drivers and services, which can bypass a third-party filter or a workload trigger; it is a clue, not proof of one cause.
